Introduction Diabetic neuropathy is the most frequent symptomatic complication of diabetes and encompasses a wide range of abnormalities affecting both the peripheral and autonomic nervous systems. These neuropathies may be disabling and even contribute to mortality in diabetic patients. Other than improved glucose control, current therapies are primarily palliative and supportive. Peripheral Neuropathy Distal, symmetric polyneuropathy is the most common form of diabetic peripheral neuropathy and usually occurs in a stocking-glove distribution. Patients with peripheral neuropathy are at risk for long-term complications of ulceration, infection, and amputation. All diabetic patients should receive an annual foot exam, including visual inspection, peripheral pulses, and sensation. The Semmes-Weinstein monofilament exam is currently the best screening test to detect clinically significant neuropathy. Patient education regarding foot care and daily monitoring for skin breakdown is essential. Pharmacologic therapies for painful peripheral neuropathy include, but are not limited to, tricyclic antidepressants, antiepileptics, and topical capsaicin.

Introduction Diabetic neuropathy (DN) is the most common and troublesome complication of diabetes mellitus, leading to the greatest morbidity and mortality and resulting in a huge economic burden for diabetes care(1;2). It is the most common form of neuropathy in the developed countries of the world, accounts for more hospitalizations than all the other diabetic complications combined, and is responsible for 50-75% of non-traumatic amputations (2;3). DN is a set of clinical syndromes that affect distinct regions of the nervous system, singly or combined. It may be silent and go undetected, while exercising its ravages, or it may present with clinical symptoms and signs that although nonspecific and insidious with slow progression but also mimic those seen in many other diseases. It is, therefore, diagnosed by exclusion. Classification DN is not a single entity but a number of different syndromes, ranging from subclinical to clinical manifestations depending on the classes of nerve fibers involved. According to the San Antonio Convention (10), the main groups of neurologic disturbance in diabetes mellitus include: 1) subclinical neuropathy, determined by abnormalities in electrodiagnostic and quantitative sensory testing, 2) diffuse clinical neuropathy with distal symmetric sensorimotor and autonomic syndromes, and 3) focal syndromes.

These tests are based on detection of heart rate and blood pressure response to a series of maneuvers. Specific tests are used in evaluating disordered regulation of gastrointestinal, genitourinary, pseudomotor function, and peripheral skin blood flow induced by autonomic DN (52). Biopsy of nerve tissue may be helpful for excluding other causes of neuropathy and in the determination of predominant pathologic changes in patients with complex clinical findings as a means of dictating choice of treatment (35;55). Skin biopsy has some clinical advantages in diagnosis of small-fiber neuropathies by quantification of PGP 9.5, when all other measures are negative (20;56). Diabetes as the cause of neuropathy is diagnosed by exclusion of various other causes of neuropathy (28;57). Figure7.     There is considerable evidence that alpha lipoic acid can be used to prevent and treat diabetes, both type I (juvenile diabetes) and type II (mature onset diabetes) and the complications of diabetes. Among the the most serious complications of diabetes is nerve damage, especially in the eye (retinopathy) and heart attacks resulting from atherosclerosis. The two most popular mechanisms of action proposed as the causes of these consequences of diabetes are excessive free radical damage and glycosylation (or glycation), which involves glucose (blood sugar) induced modifications in proteins leading to molecular cross linking of these proteins in the lens of the eye, cell membranes, and in connective tissues such as collagen and elastin.
