Summary
The authors examined the prevalence of complement-fixing autoantibodies using indirect immunofluorescence in a large clinic-based population of patients (n = 394) with Type 1 diabetes (median duration of disease, 28 years; range 6 months to 73 years) and compared it with that in a cohort of age- and sex-matched non-diabetic patients (n = 160). Autoantibodies to sympathetic ganglia (CF-SG), vagus nerve (CF-VN) and adrenal medulla (CF-ADM) were determined.
The authors conclude that complement-fixing autoantibodies to autonomic structures have an extensive presence in Type 1 diabetes patients but that their role in causing or predicting diabetic autonomic neuropathy remains to be determined. Comment
There is growing evidence that Type 1 diabetes patients have a higher prevalence of autoantibodies, not only to b-cells but also to cells in the peripheral nervous system. This report adds to other observations by Zanone et al. [1] on the prevalence of CF-SG antibodies or at least one of the autoantibodies directed to autonomic nervous system structures (CF-SG, CF-ADM or CF-VN autoantibodies) in patients with diabetic autonomic neuropathy. There is also a high frequency of antibodies to phospholipids and glutamic acid decarboxylase in diabetes patients with neuropathy [2], further supporting the notion that there is a relationship between autoantibodies to nerve structures and neuropathy in Type 1 diabetes. The authors correctly state that ‘a major research goal will now be to identify the molecular target’.

The Department of Neurology provides comprehensive outpatient and inpatient services to the USC-affiliated hospitals. The department's clinical programs includes the Bowles clinic for Alzheimer's disease and Related Disorders; Parkinson's Disease and Movement disorders clinic; Neuromuscular disease Program; Epilepsy Program (including surgery for patients with intractable seizures); Multiple Sclerosis and Immunologic disorders Program; and Cerebrovascular Disease (Stroke) Program, which emphasizes early intervention The department has a strong reputation in clinical neurophysiology and has active research programs in epilepsy, neuromuscular disease, intraoperative monitoring, and coma. Its EEG and Evoked Potential Laboratories are fully accredited and offer complete services in EEGs and evoked potentials including intraoperative monitoring. Board certified electromyographers perform expert EMG and nerve conduction studies. Evaluation and treatment of patients with neuromuscular disorders using muscle biopsy studies (eledronmicroscopy, histochemistry, culture techniques, and special studies) are available. The Department has established a two-bed telemetry unit at USC University Hospital in which to evaluate patients who are candidates for surgery for intractable Epilepsy. This is part of a comprehensive program that includes PET scanning, electrophysiology, neurophysiology, language localization, and therapeutic protocols in relation to both standard and experimental antiepileptic drugs.

Abstract Diabetic neuropathy occurs in a majority of patients with type 1 or type 2 diabetes mellitus. Clinical presentation varies based on the distribution and types of nerves involved. Many patients have signs and symptoms that do not fall neatly into any one neuropathic category. A thorough history and physical examination, as well as appropriate diagnostic studies, should be performed when diabetic neuropathy is suspected. Central to the treatment of diabetic neuropathy is close glycemic control with ancillary drugs and therapies directed at symptom relief. Reducing chronic hyperglycemia can help reduce neuropathic pain and slow the progression of neuropathies. The Physician Assistant Recertification Series provides a definitive review and update of major areas of primary care medicine and the opportunity to earn Category 1 CME credit. Article selection and a blinded peer review are conducted by a distinguished panel of PA educators and clinicians. The series is designed to help PAs maintain current clinical standards.

    EPIDEMIOLOGY OF DIABETIC NEPHROPATHY
    Diabetic neuropathy The dialysis treatments neither improve nor stabilise the neuropathy, even if this occurs less frequently and is less severe in diabetic patients treated with PD. However, it has been observed (M. Briganti et al., Blood Purification, 1989) that thanks to an adequate correction of uraemic acidosis, which ensures suitable levels of pCO (decreases correlated to the marked anomalies in EEG signalling), several types of dialysis can improve intra-dialysis tolerance, both clinical and neurological. The best correction of acid-base status attainable in AFB allows for stable levels of pCO , both plasmatic and cerebral.

This condition is thought to be caused by immune complexes consisting of mycobacterial antigen, immunoglobulin and complement that deposit in the cutaneous tissues. ENL differs from classic erythema nodosum, which is usually painless, and limited to the lower extremities, by being widely distributed even affecting the face; being painful; and having nodular lesions. Iritis, neuritis, lymphadenitis, arthritis, and proteinuria may also be present and fever is common. ENL can be the first manifestation of leprosy or it can develop during the course of treatment.
