Diarrhoea and vomiting often appear together, but may appear separately. Most times, although you feel terrible, there is little that a doctor can do for you, other than give advice similar to that below. For most intents and purposes the approach to the treatment is similar:
Do
  • Only drink clear fluids eg purpose-made electrolyte solutions, or "pop", such as lemonade.
  • Start by just taking sips of fluid, and when the sickness subsides slowly build up the quantity.
  • You may take anti-diarrhoea medications which can help the symptoms, but they do not cure you any quicker.
Do not
  • Do not eat until at least 24 hours after the last sickness or diarrhoea subsides.
  • Do not drink milk or drinks containing particles, as these take more digestion, and irritate the bowel.
  • Do not take any tablets for headache or fever until the vomiting subsides.
Diarrhoea
Diarrhoea involves frequent and loose motions. It is usually caused by inflammation of the large intestine, which is where the body removes most of the fluid from digested food. The major cause, in the context that we are referring to, is infection, but there are various other rarer causes which occasionally occur. If diarrhoea persists, and is not subsiding after a week, you should consult your doctor. It may be that you think it is due to a treatment you are taking from your doctor or dentist, and it is wise to consult your doctor immediately if this is the case. In some cases bleeding occurs, and this also should prompt a consultation with your doctor, but as long as the blood loss is not heavy, this can await a daytime consultation.

Diarrhoea is not as dramatic an illness as AIDS. However, the above groups of viruses are of great economic importance, causing millions of lost working days each year, as well as much discomfort. Diarrhoea continues to be a major cause of morbidity and mortality worldwide resulting in an estimated 1000 deaths among children each day, the highest incidence being in developing countries of the world. One well documented source of infection is the consumption of shellfish (polluted by sewage) - and therefore, they also have economic consequences for fishermen and the food industry. Such viruses often cause mini-epidemics in families, hospital wards, etc and are potentially very dangerous to seriously ill hospital patients. More importantly, these viruses contribute to the massive mortality caused by infantile diarrhoea in developing countries and are responsible for uncounted millions of deaths each year.
The human pathogens in these groups have been poorly studied since for the most part, they do not grow in culture. Only recently has molecular information on the nature of their genomes become available. The taxonomy of these viruses is still somewhat confused and they have previously been grouped on the basis of their appearance in the EM (unreliable). It is common for diarrhoea to be accompanied by stomach pains, bloating, nausea, vomiting, fever and loss of appetite. Diarrhoea as a drug related side-effect Diarrhoea has been reported as a side-effect of all the protease inhibitors as well as ddI and abacavir in the NRTI class, and with some antibiotics. With some drugs, diarrhoea goes away after the first few weeks or months of treatment, however for some people it becomes a permanent feature of living with the drug. The severity of the diarrhoea also varies. Severe diarrhoea, involving several trips to the toilet each day, large, uncontrollable liquid bowel movements, and feelings of weakness and dizziness as a result of the loss of fluids and electrolytes is experienced by about a quarter of people starting treatment with nelfinavir and a fifth of people starting saquinavir. Similar levels have been reported in people taking amprenavir, lopinavir, ritonavir and indinavir. Less serious problems may be experienced by many other people taking protease inhibitors. Changes in diet have little effect on protease inhibitor and other drug-related diarrhoea. However a variety of treatments are available to doctors to try and control diarrhoea caused by drugs. In fact, it is the commonest cause of incapacity in air crew, one study suggesting that 29% of all crew suffered at least one incident of incapacitation in flight, and 58 times out of a 100 that incapacitation would be....you've guessed! Most diarrhoea is caused by ingesting food or water contaminated with faeces or chemicals, such as alcohol or chlorine. "Delhi Belly" is usually acquired in the Tropics or Sub-Tropics where food hygiene is poor and water is polluted by inadequate sewage disposal, though altered diet and sunbathing can be major contributing factors. Bacteria are the commonest bugs to cause diarrhoea, which can affect half of all travellers, many of whom end up as serious visitors to the toilet and 1% even in the Krankenhaus. E.Coli lives up to its faecal name and is responsible for about half the cases. Shigella, Campylobacter, Salmonella (can cause typhoid) and Vibrio Cholerae are other performers, though the "rice water" stools of the latter are thankfully uncommon amongst present day travellers. Vibrio Parahaemolyticus, although infrequent in the UK, is the commonest form of food poisoning in Japan where seafood forms a high percentage of the staple diet. Viruses are now an increasingly common cause of poisoning from shellfish, and the Protozoa Giardia is responsible for 11,000 reported cases annually in the UK, although most originated abroad. Clay coloured stools suggest viral hepatitis which three in every 1,000 travellers catch unless they have been wise enough to have had their Hep A jab.

Summary of Meeting Presentation Peter Sullivan from the Department of Paediatrics, University of Oxford presented a paper on 'the Management of Persistent Diarrhoea and Malnutrition (PDM) at the International Child Health Group Meeting on Managing Childhood Malnutrition: in Birmingham. Dr Sullivan explained, how a pressing concern amongst paediatricians is the continuing high mortality rates in children with PDM (between 3-20% of diarrhoea becomes persistent). This, he attributed to faulty case-management. For example, inappropriate diets that do not take account of metabolic adaptations to malnutrition are commonplace and may account for many deaths. Another crucial aspect is inadequate treatment of microbial infections which significantly affect the outcome of nutritional rehabilitation in children with PD. Studies have shown that following treatment and cure for PDM many children relapse, and that even after one year the state of the gut is similar to when the child first presented. It was explained how successful nutritional rehabilitation of children with persistent diarrhoea (PD) requires an understanding of the biology of malnutrition and of guidelines for provision of appropriate diets. MN is an important factor in determining the duration of PD and in the success of treatment. In order to treat PD and MN effectively one needs to have a good understanding of:

Diarrhoea
Diarrhoea kills over 1 million children every year through dehydration and malnutrition. Children are more likely than adults to die from diarrhoea because they become dehydrated more quickly. About one in every 200 children who contract diarrhoea will die from it. Diarrhoea is caused by germs that are swallowed, especially germs from faeces. This happens most often where there is unsafe disposal of faeces, poor hygiene practices or a lack of clean drinking water, or when infants are not breastfed. Infants who are fed only breastmilk seldom get diarrhoea. If families and communities work together, with support from governments and non-governmental organizations (NGOs), they can do much to prevent the conditions that cause diarrhoea. What is diarrhoea?
Diarrhoea is the passing of increased amounts (more than 300g in 24 hours) of loose stools. It is often caused by a virus or bacteria and can be acute (short term) or chronic (long term) - lasting more than two to three weeks. Most people are affected by diarrhoea at some time in their lives. It is often accompanied by stomach pains, feeling sick and vomiting. It is usually due to consumption of drinking water contaminated with bacteria, undercooked meat and eggs or inadequate kitchen hygiene - in other words, an infection.
Diarrhoea normally only lasts for a few days, but it can be very stressful when it occurs.

Causes of travellers' diarrhoea
The toilet roll is not supposed to be your most important travelling companion. Think before you eat and drink as it is very easy to pick up a bout of diarrhoea while holidaying abroad. The most common cause of holiday or travellers' diarrhoea is the different types of virus and bacteria at the destination.
The local drinking water in particular is an obvious source of risk in many places and should, therefore, be completely avoided. Replace it with water from previously unopened bottles for drinking as well as for cleaning teeth and making ice for drinks.
Approximately 40 per cent of all cases of travellers' diarrhoea are due to infections with ETEC (enterotoxin-forming Escherichia coli bacteria). It is also possible to be infected with other and more specific and unpleasant bacteria and parasites, such as

An effective vaccine for diarrhoea could be available within 10 years, according to scientists at the forefront of research in Bangladesh. An estimated 1.5 million people throughout the world die of diarrhoea each year. Millions more are hospitalised and require urgent medical care to stop them suffering chronic dehydration and dying. We would hope to have a vaccine in about 10 years' time

The centre includes a hospital and treats in the region of 140,000 people with diarrhoea each year. Saving lives Recent studies have looked at possible vaccines for cholera and rotavirus - the leading cause of childhood diarrhoea which kills up to 600,000 children each year. Almost everyone has diarrhoea at some point in his or her life. The majority of diarrhoea diseases results from infection and is a short-lived self-limiting condition. Terms such as "stomach flu" and gastroenteritis are used to describe the same condition.
Most diarrhoea will stop without treatment in a few days. Certain drugs can cause or worsen diarrhoea.
"Acute diarrhoea" is defined as a short episode of diarrhoea that comes on suddenly and goes away within a couple of weeks. Most people with acute diarrhoea recover on their own.
Diarrhoea that lasts more than three weeks is thought of as "chronic diarrhoea." Typically, chronic diarrhoea requires medical attention and treatment.
Infants and the elderly are more prone to dehydration from diarrhoea.
Symptoms

Many parents wonder if their child has diarrhoea because bowel movements in a newborn tend to occur frequently. Your baby's ordinary bowel movements will depend very much on whether she's being breast or formula fed. The bowel movements of breastfed babies are usually yellowish, on the soft or liquid side and may occur up to five times daily. Sometimes a breastfed newborn will have a bowel movement during or immediately after each breastfeeding session. This occurs because as her stomach fills up, the milk stimulates her entire intestinal tract, prompting a bowel movement. Within a month, most breastfed babies are down to one or two bowel movements a day. The bowel movements of formula-fed babies tend to occur once a day and are fairly firm. fever or appears to be losing weight, it may be diarrhoea, so consult your GP or health visitor.
