This condition results from the bacillus Corynebacterium diphtheriae, which multiplies on the throat or skin during infection. It causes a sore throat and fever, and it can be fatal. Children in the U.S. are vaccinated in their first year of life with DPT vaccine (against diptheria, pertussis, and tetanus); however, in poor developing countries, immunization is not routine and presents a hazard for these countries as well as for nonimmunized people who travel to these countries.

A substantial proportion of U.S. adults are not sufficiently protected against diphtheria and tetanus because they haven't kept up with their booster shots, a study found. On average, only 60 percent of adults were protected against diphtheria and 72 percent against tetanus, according to the study in Tuesday's Annals of Internal Medicine. "In this country we have a real emphasis on childhood immunizations," said the lead author, Geraldine M. McQuillan of the National Center for Health Statistics. "What was a big surprise about this data was that though we're doing OK for kids, we really have a problem in adults." Antibody levels were poorest among people 70 and older, with only 30 percent immune to either disease, researchers found. Antibodies are produced by the immune system to fight off threats to the body.

Corynebacteria are Gram-positive, aerobic, nonmotile, rod-shaped bacteria related to the Actinomycetes. They do not form spores or branch as do the actinomycetes, but they have the characteristic of forming irregular shaped, club-shaped or V-shaped arrangements in normal growth. They undergo snapping movements just after cell division which brings them into characteristic arrangements resembling Chinese letters. Figure 1. Stained Corynebacterium cells. The "barred" appearance is due to the presence of polyphosphate inclusions called metachromatic granules. Note also the characteristic "Chinese-letter" arrangement of cells. The genus Corynebacterium consists of a diverse group of bacteria including animal and plant pathogens, as well as saprophytes. Some corynebacteria are part of the normal flora of humans, finding a suitable niche in virtually every anatomic site. The best known and most widely studied species is Corynebacterium diphtheriae , the causal agent of the disease diphtheria.

What is diphtheria?
Diphtheria is an acute infectious disease caused by a bacterium called Corynebacterium diphtheriae . This disease affects the upper respiratory tract and very occasionally the skin. Virulent strains of C. diphtheriae produce a toxin which can damage heart and nervous tissues, although non-toxigenic strains can cause a localised infection. A diphtheria-like illness is sometimes caused by another toxin-producing bacterium Corynebacterium ulcerans . Immunisation against diphtheria was introduced on a national scale throughout the United Kingdom in the early 1940s and resulted in a dramatic drop in the number of cases and deaths from the disease. For example, in 1940, 46,281 cases and 2,480 deaths were notified, compared with 37 cases and six deaths in 1957. Diphtheria is now rare in England and Wales, nearly all new cases are acquired abroad, and only two deaths due to diphtheria have been recorded since 1972.

Diphtheria is an infectious disease spreading from person to person by respiratory droplets from the throat through coughing and sneezing. The disease normally breaks out 2 to 5 days after infection. Diphtheria usually affects the tonsils, pharynx, larynx and occasionally the skin. Symptoms range from a moderately sore throat to toxic life-threatening diphtheria of the larynx or of the lower and upper respiratory tracts. Diphtheria is often complicated by diphtheric myocarditis (toxic damage to heart muscles) and neuritis (toxic damage to peripheral nerves). The disease can be fatal - between 5% and 10% of diphtheria patients die, even if properly treated. Untreated, the disease claims even more lives. Untreated patients are infectious for 2 to 3 weeks. Treatment consists of immediate administration of diphtheria antitoxin and antibiotics. Antibiotic treatment usually renders patients non-infectious within 24 hours. Unless immunized, children and adults may repeatedly be infected with the disease.
