  • What is dry eye syndrome? Dry eye syndrome is the decline of the quality or quantity of tears bathing the eye. The patient experiences constant pain from eye irritation, and a sandy or gritty sensation that, if untreated, can lead to scarring or ulceration of the cornea, and thus loss of vision.
  • What do we know about tears? In many cases, dry eye results from disorders of the various glands which work together to produce normal tears. Tears themselves are a complex combination of substances which form three layers on the eye. The very thin outer layer contains lipids from the meibomian glands in the eyelid, to reduce evaporation. The lacrimal glands produce the middle watery layer that keeps the salinity and the acidity of the tears at proper levels. This middle layer also carries antibodies and other immune defense agents to defend the eye against infection. The inner mucous layer helps the tear film "stick" to the cornea and stay intact.

    Frequently Asked Questions about
    Dry Eye
    Q: What is dry eye?
Our natural tears protect our eyes and give them moisture that is absolutely necessary for clear and comfortable vision. Some people are unable to produce enough of these tears, leaving the eyes dry and easily irritated. In this condition that affects millions of Americans, blinking no longer leaves a moist "tear film" to wash and soothe the eye. The irritation and discomfort that result may make a difference in one's ability to see. In rare cases, dry eye can become serious—even blinding—without proper care and treatment.
    Q: What causes dry eye?
It's not clear why some people are unable to produce enough natural tears. However, one cause of dry eye is Sjogren's Syndrome, a disease involving mild to extreme dryness in both the eyes and the mouth. This disorder may be connected with menopause and arthritis. However, dry eye can also be caused by other eye disorders.
    Q: What treatment is available?

VOL 103 / NO 5 / MAY 1998 / POSTGRADUATE MEDICINE Do your eyes sting and burn? Do they feel gritty and irritated? You may have dry eye. This condition is just what it sounds likeyou don't have enough tears coating your eyes to keep them comfortable. A reduction in the amount of tears normally accompanies aging. But it can also be caused by use of some prescription and nonprescription drugs, by eye infection or injury, or by a certain type of arthritis. Your primary care doctor or ophthalmologist can help determine the cause and best treatment of the problem. How is dry eye treated?
Unfortunately, there is no way to increase production of tears. Either lubricating drops called "artificial" tears must be used to replace the missing ones, or the tears you produce naturally must be conserved.
    • You can buy artificial tears in a drugstore without a prescription and can use them as often as necessary to make your eyes feel better. Those that do not contain preservatives are recommended. Eye ointments are available too. Ointments keep your eyes moist longer than drops, so many patients use them before they go to bed. To put medication in your eyes, first wash your hands. Then pull your lower eyelid down with a fingertip. Look up. Squeeze a drop or a short strip of ointment into the lower lid. (Do not touch your eye with the applicator.) Close your eye for a few seconds to let the medication absorb.

Every wakening second your eyes produce tears containing a complex mixture of infection fighting components. Blinking spreads this over the eye's surface before being sucked through two small holes in the nasal corner of your eyelids. These are the tear duct openings or puncta that drain tears to the nose and throat. If your eyes make insufficient tears, or they drain away too quickly, the tear film is spread too thinly over the eye's surface. If your tears are chemically imbalanced, they evaporate too quickly before the next blink comes along. Either way dry spots appear and cause irritation.

Who gets dry eyes? Dry eye is a condition that can develop under many circumstances and affects millions of people of all ages and races worldwide. In some, it may be associated with underlying medical problems. The dry eye syndrome is caused by the alteration in one's natural tear film, a thin layer of tears protecting the surface of our eyes. Damage to the surface of the eyes (cornea and conjunctiva) is responsible for the symptoms of dry eyes. How do I know if I have dry eyes? If you feel a foreign body sensation in your eyes, then an eye physician (ophthalmologist) can tell you if you have dry eyes by examining your eyes and testing for the amount of tears that your eyes make. How are dry eyes treated? Artificial tears, which may be purchased at a local drug store, are the first line of therapy for dry eyes. Artificial tears come in several different brands. Some solutions are thicker than others. The ones which are preservative-free are recommended. The thicker solutions might relieve symptoms for a longer period of time; however, they may cause slight blurriness of vision for a short time after they are applied. A humidified environment also helps dry eyes. Several other modalities of treatment, including occlusion of the tear drainage system with a plug, exist for people with severe dry eyes. Most importantly, people with dry eyes should be examined by an eye physician to be treated for any possible causes of their symptoms.

Overview Dry eye syndrome is one of the most common problems treated by eye physicians. Over ten million Americans suffer from dry eyes. It is usually caused by a problem with the quality of the tear film that lubricates the eyes. Tears are comprised of three layers. The mucus layer coats the cornea, the eye’s clear outer window, forming a foundation so the tear film can adhere to the eye. The middle aqueous layer provides moisture and supplies oxygen and other important nutrients to the cornea. This layer is made of 98 percent water along with small amounts of salt, proteins and other compounds. The outer lipid layer is an oily film that seals the tear film on the eye and helps to prevent evaporation. Tears are formed in several glands around the eye. The water layer is produced in the lacrimal gland, located under the upper eyelid. Several smaller glands in the lids make the oil and mucus layers. With each blink, the eyelids spread the tears over the eye. Excess tears flow into two tiny drainage ducts in the corner of the eye by the nose. These ducts lead to tiny canals that connect to the nasal passage. The connection between the tear ducts and the nasal passage is the reason that crying causes a runny nose. In addition to lubricating the eye, tears are also produced as a reflex response to outside stimulus such as an injury or emotion. However, reflex tears do little to soothe a dry eye, which is why someone with watery eyes may still complain of irritation.

Dry Eye Back to Other Specialities Substation What is "Dry Eye"? The tears your eyes normally produce are necessary for overall eye health and clear vision. Dry Eye occurs when your eyes do not produce enough tears or produce tears which do not have the proper chemical composition. What causes "Dry Eye"? Dry Eye symptoms can result from the normal aging process, exposure to environmental conditions, problems with normal blinking or from medications such as antihistamines, oral contraceptives or antidepressants. Dry Eye can also be symptomatic of general health problems, other eye diseases, or can result from chemical or thermal burns to the eye. Contact lens wear can exaggerate symptoms. What are signs/symptoms of "Dry Eye"? The most common signs/symptoms include stinging, scratchy and uncomfortable eyes; and sometimes having a burning feeling of something foreign within the eye. You may experience increased Dry Eye symptoms upon awakening. People sometimes experience an intermittent overly watery eye. This is a natural reflex to try to comfort eyes that do not produce enough of the basic secretions to protect the eye. How is "Dry Eye" diagnosed?

If you have sandy, gritty irritated eyes or burning eyes , and these symptoms get worse as the day goes on, you’re probably one of the 14 million Americans with the problem eye doctors call “ dry eye Now, after years of research, the good news is that we finally understand dry eye , and there is something you can do about it. Why Does Your Eye Need Tears Anyway?
Did you ever wonder why the eye is covered by tears? It’s because the delicate living tissue on the surface of your eye has no blood supply—it has a tear supply instead. So rather than getting important things like oxygen and electrolytes from the blood, your eye surface gets them from the watery layer of your tears. What Happens In Dry Eye?
In dry eye the tear film on the eye surface loses water because of either decreased tear production or increased evaporation. And as evaporation continues throughout the day, your eyes feel drier and drier. What Causes Dry Eye?
