If you are a woman experiencing symptoms such as pain during sexual intercourse or pain during your periods that interferes with your full life, you have arrived at the right place. Pain during sexual intercourse or during menstruation are common conditions. Sometimes these conditions, known as dyspareunia (pain during intercourse) and dysmenorrhea (pain during menstruation), are due to a retroverted uterus or a prolapsed uterus. Dyspareunia and dysmenorrhea as well as other related conditions can cause physical as well as emotional distress and can effect intimate relationships. Although you may think you are alone, a surprisingly large number of women experience these pelvic pain conditions regardless of marital status, age, income, race or child bearing history.

Many women do not seek treatment due to fear or embarrassment. However, changing social attitudes coupled with an increasing number of therapeutic options make it possible for physicians to help more women.     Dyspareunia is the clinical name for painful intercourse. This condition can occur at any age, in both sexes, and the pain can appear at the start of intercourse, midway through, at the time of orgasm, or after intercourse is completed. The pain can be felt as burning, sharp, searing or cramping. It can be external, within the vagina, or deep in the pelvic region or abdomen. The causes of dyspareunia, as with most sexual dysfunction, can be classified as either organic (physical or medical factors such as illness, injury or drug effects) or psychosocial (including psychological, interpersonal, environmental and cultural factors). The cause of a sexual dysfunction in a given individual may be a combination of several factors, and in some cases, the precise cause may not be identifiable at all.

    Uterine Suspension in the Management of Collision Dyspareunia Background Dyspareunia is a multi-faceted problem that can involve a number of factors and disease states. Women rarely report it on their own but may discuss the issue upon direct inquiry. Evaluation of a patient responding affirmatively should involve a complete and careful sexual history and a comprehensive physical exam. It is important to identify the severity, character and location of pain during intercourse. This information can be used to determine whether the patient is experiencing superficial or deep dyspareunia or both. In cases of deep dyspareunia, particular attention should be paid to uterine position during the pelvic exam. If the pain experienced during intercourse can be reproduced with a digital exam, the patient is a good candidate for uterine suspension. Some investigators have advocated a pessary trial prior to uterine suspension. However, others have noted that the trial is unnecessary and that pessary use can cause sexual dysfunction and further gynecological concerns. Uterine suspension represents a long-term solution for women with dyspareunia secondary to a retroverted uterus.

D yspareunia is genital pain experienced just before, during or after sexual intercourse. Although this condition has historically been classified as a sexual disorder, an integrated and pain-model approach to the problem is gaining support. The current thinking about pain initiation and promulgation suggests an initial instigating factor that is then perpetuated by confounding factors. These factors may be physical or psychologic. Patients with dyspareunia may complain of a well-defined and localized pain, or express a general disinterest in and dissatisfaction with intercourse that stems from the associated discomfort. Although dyspareunia is present in both sexes, it is far more common in women, with the pain initiating in several areas, from vulvar surfaces to deep pelvic structures. Patients with dyspareunia are more likely than the general public to report pain with use of a tampon or digit, or during a gynecologic examination.
