61. Low Desire, Vaginismus, And Dyspareunia
dyspareunia is another disorder that can occur in both men and women. Itis defined as genital pain that is associated with sexual intercourse.

Hypoactive sexual desire disorder (HSDD) is a condition in which individuals (either men or women) are persistently and recurrently uninterested in sexual expression, and who report the absence of sexual fantasies. In addition, there is little motivation to seek stimuli and diminished frustration when deprived of the opportunity for sexual expression. The individual usually does not initiate sexual activity or may only engage in it reluctantly when it is initiated by the partner. As with all sexual disorders, HSDD can be classified as lifelong (existing for the entirety of the person’s adult life) or acquired (developed after a period of normal functioning). It can also be classified as generalized (occurring across all partners, sexual activities, and situations) or situational (limited to certain partners, sexual practices, or situations). One important factor in the diagnosis of HSDD as with all sexual disorders is that the condition is distressing to the individual and/or is interfering with their life in some way. The following are the diagnostic criteria for HSDD as provided by the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV, 1994):

A case of superficial dyspareunia in a 46-year-old women [sic] is described. Clinical suspicion, examination and investigations supported a diagnosis of Sjogren's syndrome. Dyspareunia is a common symptom that may cause anxiety and lead to problems in sexual relationships. A community of women aged 35-59 identified dyspareunia in 8% (30/379) and vaginal dryness in 17% (73/434). Patients with dyspareunia might not volunteer this symptom.
Causes of superficial dyspareunia include a rigid hymen, vulvovaginitis due to thrush or, less commonly, trichomoniasis, a poorly repaired episiotomy, and, in postmenopausal women, narrowing of the vagina due to atrophic changes. Vaginal dryness may cause superficial dyspareunia. Various factors lead to vaginal dryness: drugs such as anticholinergic agents, antihistamines, antidepressants, antispasmodics, antiparkinsonian agents, potent progestogens such as norethisterone, and antioestrogenic drugs such as danazol and tamoxifen; diabetes mellitus;

Dyspareunia Causes
Any of the genital parts can cause pain during sex. Some conditions affect the skin around the vaginal entrance, some cause muscle spasm at the vaginal entrance and some women have a bacterial, viral or yeast infection. In many cases the cause is unknown and orthodox medicine is unable to offer a treatment. The pain from these conditions is usually felt when a tampon or penis is inserted into the vagina.     rev. 8/02 Dyspareunia (Pain with Sexual Relations) It is not uncommon for women to experience pain or discomfort during and/or after sexual relations (dyspareunia). Menopausal women are particularly susceptible to developing dyspareunia. During the reproductive years, the ovaries produce high levels of estrogen and progesterone. Among other things, estrogen maintains the thickness and health of the vagina, which leads to normal functioning of the vagina during sexual relations. Menopause leads to lower estrogen levels, which may cause the vagina to become relatively thin and dry (atrophic). Lower estrogen levels also inhibit the normal vaginal sexual response, e.g. with regard to lubrication. In some cases, vaginal atrophy and lack of sexual response may lead to discomfort during and/or after sexual relations. Generally speaking, dyspareunia may occur during superficial, mid, and/or deep penetration. Dyspareunia due to vaginal atrophy may occur at any level, and often occurs along the entire course of the vagina. Other common causes of dyspareunia include:
    • cystocele or rectocele may cause discomfort during sexual relations. The dyspareunia is typically anterior with cystoceles, and posterior with rectoceles.

    D yspareunia is genital pain experienced just before, during or after sexual intercourse. Although this condition has historically been classified as a sexual disorder, an integrated and pain-model approach to the problem is gaining support. The current thinking about pain initiation and promulgation suggests an initial instigating factor that is then perpetuated by confounding factors. These factors may be physical or psychologic. Patients with dyspareunia may complain of a well-defined and localized pain, or express a general disinterest in and dissatisfaction with intercourse that stems from the associated discomfort. Although dyspareunia is present in both sexes, it is far more common in women, with the pain initiating in several areas, from vulvar surfaces to deep pelvic structures. Patients with dyspareunia are more likely than the general public to report pain with use of a tampon or digit, or during a gynecologic examination. This article reviews the various causes of dyspareunia and describes the historical and physical clues leading to these diagnoses. Treatment options are beyond the scope of this article.
