Synonyms: Abdominal muscle deficiency anomalad, abdominal muscle deficiency syndrome, abdominal musculature aplasia syndrome, abdominal wall aplasia syndrome, absence of abdominal muscle syndrome, aplastic abdominal muscle syndrome, defective abdominal wall syndrome, prune-belly-sequenz, prune belly syndrome. Associated persons: Norman Rupert Barrett J. F. Eagle, Jr. F. Fröhlich William Obrinsky Description: A syndrome combining malformation of the urogenital tract with aplasia of the abdominal musculature. There is a congenital absence of muscles of the anterior abdominal wall, and the lax excessive skin of the abdomen looks like the wrinkled skin of a prune. Other features include persistent furrowlike umbilicus, cryptorchism; and other anomalies, including malrotation of the bowel, pigeon breast, hip dislocation, polydactyly, and heart defects. The male/female rate is 2: 2. The aetiology is unknown. Affected girls show only absence of abdominal muscles. About 20% are stillborn or die within first 4 weeks of life; 50% die within 2 years. Diagnosis can be made in utero by ultrasound by 21 weeks gestation. It can occur sporadically, as an X-linked transmission, associated with chromosomal abnormalities, and as a familial occurrence and is then associated with congenital deafness and mental retardation. First described in 1839 by F. Fröhlich, Würzburg. Summarizing presentation in 1949 by W. Obrinsky.

Triad of deficient or absent abdominal musculature, undescended testicles, and dysplasia of the urinary tract. Physical exam shows a wrinkled anterior abdominal wall. Etiology/Pathophysiology:
Etiology is unknown.Twenty percent are still born or die as neonates, and an additional 50% die in the first 2 years of life. Other associated anomalies include hip dislocations / club foot / limb deformities (40% ), cardiac defects(30% ), and malrotation (30% ). Pathology:
Aplasia of the muscles of the lower medial abdominal wall. The kidneys have a decreased number of functioning nephrons with variable cystic dysplasia. The dilated ureters and bladder have a patchy absence of smooth muscle. Imaging Findings:
The ureters and bladder are dilated. Urethral abnormalities that can be seen include posterior urethral dilation, diverticula, stenosis / atresia, and megaurethra. They can have umbilical-bladder attachment or a patent urachus. DDX:

Prune belly syndrome
Definition: A disease characterized by three major findings (triad): deficient abdominal muscles (causing the skin of the abdomen to wrinkle like a prune), undescended testicles , and a dilated abnormal urinary tract.
Alternative Names: Eagle-Barrett syndrome
Causes, incidence, and risk factors: The underlying causes of prune belly syndrome are unknown. There is blockage to the flow of urine from the kidney to the outside of the body during fetal life. For example, dilatation of the urinary tract may develop as a result of urethral obstruction in the womb. The incidence is approximately 1 in 30,000 to 40,000 births. Boys are almost solely affected.
Expectant mothers carrying affected infants may develop varying degrees of oligohydramnios (insufficient amniotic fluid ) that predisposes the infant to lung problems (see Potter syndrome ). The newborn infant has a wrinkled abdomen (it looks like a prune). This appearance results from swelling with fluid in the womb then loss of that fluid after birth leading to wrinkles of excess skin. The appearance is accentuated by the lack of adequate abdominal musculature. Genital abnormalities may be prominent as undescended testes . Approximately one third of the infants are either stillborn or die within the first few weeks of life from severe lung, kidney, or combined problems.

LOS ANGELES, CA (Oct. 30, 2002) About as complex as it getsthat's how pediatric urologist Andrew Freedman, M.D., director of pediatric urology at Cedars-Sinai Medical Center's Endourology Institute, describes the surgery he performed this summer on then 17- month-old Jalen Brown, born with Prune Belly Syndrome. That surgery required reconstructing the toddler's urinary system in a nearly 10-hour procedure.
Prune Belly Syndrome, also known as Eagle-Barrett Syndrome, is a very rare occurrence: just one in 80,000 births. An estimated 99 percent of those affected are boys. The cause is still unknown, though some theories suggest urinary tract obstructions and resulting distention or developmental defects in utero.

The Eagle-Barrett syndrome is a relatively rare condition in which there is failure of normal development of the abdominal muscles and the smooth muscle of the ureters and bladder. Bilateral cryp torch idism is the rule. At times, talipes equinovarus and hip dislocation are also noted. Because the smooth muscle of the ureterotrigonal complex is deficient, reflux is to be expected; advanced hydroureteronephrosis is present.
