Prune Belly Syndrome, also known as Eagle-Barrett Syndrome, by Jill Todd II. Chromosomal Location Prune Belly mimics an X-linked disease. The exact cause of Prune Belly Syndrome is not known; although, urinary obstruction (a common symptom), has been associated with the Ring X chromosome which lacks the XIST locus. The XIST is the inactivation center. Failure for the Ring X chromosome to become inactive causes many abnormalities, such as urinary obstruction (Wiley, 1). III. Type of inheritance Prune Belly Syndrome is an autosomal dominant disease; however, it mimics X-linked inheritance. This disease is seen in families – only in sons. The disease is thought to be caused by a primary defect in mesoderm development (Dept of Radiology, 1).
  • Incidence
  • Approximately 1/40,000 live births are born with Prune Belly Syndrome. 20% are still born or die before birth; 50% die in the first two years of life (D’Alessandro, 1). The disease is predominately in males.
  • Symptoms
      Absence or partial stomach muscles
    Abdomen appears as a "potbelly" (Pedbase, 2)

    • absence of abdominal muscles
      undescended testicles - a condition seen in newborns whereby one (or both) of the male testes has not passed down into the scrotal sac
      an abnormal, expanded bladder and problems in the upper urinary tract, which may include the bladder, ureters, and kidneys
    Because of the substantial involvement of the urinary tract, children with prune belly syndrome are usually unable to completely empty their bladders and have serious bladder, ureter, and kidney impairment. A child with prune belly syndrome may also have other birth defects. Most commonly, these defects involve the skeletal system, intestines, and heart. Girls may have defects in their external genitalia, as well. Some infants who have prune belly syndrome may be stillborn or die within a few months of birth.

    Prune Belly Syndrome, also known as Eagle-Barrett syndrome, is awell known triad of abdominal muscle deficiency or hypoplasia, urinarytract anomalies, and bilateral cryptorchidism. Tracheo-esophagealfistula is a sporadically occurring defect estimated to occur in 1 out4425 live births. Only one case of Prune Belly Syndrome, tracheo-esophagealfistula associated with VATER syndrome, and urethral atresia occurringin the same individual has been reported. We present a uniquecase involving a 33 week fetus with bladder distention and bilateral hydroureteronephrosisdiagnosed in utero who postnatally was diagnosed with Prune Belly Syndrome,tracheo-esophageal fistula, and urethral atresia.
    CASE REPORT After the placement of a 8 Fr. percutaneous suprapubic tube, a cystogramwas performed which demonstrated a large bladder with severe trabeculation.After instillation of 200 milliliters of cystograffin under gravity, bilateralgrade 5 ureteral reflux was evident by the presence of contrast in therenal pelvis bilaterally On day four of life, the patient was noted to have a creatinine of 1.0mg/dl. Urine output during this time was between 1.5-3 ml/kg/hr. Repeatabdominal ultrasonography demonstrated worsening hydroureteronephrosis.The patient was taken to the operating room for an exploratory laparotomyand urinary diversion. The bladder was noted to have two large
