Veterans' Entitlements Act 1986 The Repatriation Medical Authority under subsection of the Veterans' Entitlements Act 1986 (the Act): (a) revokes Instrument No.137 of 1996 (Statement of Principles concerning chronic airflow limitation); and (b) determines in its place the following Statement of Principles.
Kind of injury, disease or death
. (a) This Statement of principles is about chronic bronchitis and/or emphysema and death from chronic bronchitis and/or emphysema. (b) For the purposes of this Statement of Principles,
(i) "chronic bronchitis" means a respiratory tract disorder characterised by excessive mucus production sufficient to cause cough and sputum production with expectoration for at least three months of each of at least two consecutive years which is not attributable to other respiratory diseases, attracting ICD code 491. The bronchitis may be be present alone or may be accompanied by chronic airways obstruction or limitation, with or without a reversible component. There are four categories of chronic bronchitis: chronic simple bronchitis, chronic mucopurulent bronchitis, asthmatic bronchitis and chronic bronchitis with pulmonary obstruction. This definition specifically excludes bronchiolitis and chronic obstruction from bronchiolitis;

For unknown words see the On-Line Medical Dictionary See this site (for a summary of potassium and copper nutrition) INTRODUCTION I have previously proposed that rheumatoid arthritis is accentuated by losses of potassium in our food supply and is largely a chronic potassium deficiency [Weber 1974] which causes alterations inside the cell of free amino acids [Iacobellis], interference by sodium with enzymes inside the cell as a result of the increased sodium there [Rubini], alterations of the potassium-sodium regulating hormone patterns which then affect other physiological processes, or some combination of these. I have come to believe that a disturbance of copper metabolism is the most serious symptom of these other processes. The aspect of copper physiology which is most potentially dangerous is its role in activating lysyl oxidase, the enzyme which cross links collagen and elastin connecting tissue [Sandberg, et al]. Copper's effect on elastin is especially important because elastin gains its strength primarily from cross linking [Carnes1971(discusses copper aminases)] and because elastin is the main material of several important organs, which include blood vessels, spinal discs, lungs, and skin [Carnes, 1977].

Web posted at: 10:09 p.m. EDT (0209 GMT) WASHINGTON (CNN) For the first time, researchers have reversed emphysema in lab animals using a derivative of vitamin A. It is the first promise for a non-surgical treatment of the chronic, debilitating disease that destroys the air sacs of the lungs. Drs. Gloria De Carlo Massaro and Donald Massaro of Georgetown University, who conducted the study, treated rats with retinoic acid, a form of vitamin A. The damaged air sacs in the lungs of the rats were restored to normal size and number. The research is published in the May issue of Nature Medicine. Dr. Claude Lenfant, director of the National Heart, Lung, and Blood Institute (part of the NIH), says "this is the first time that anyone has identified a means of reversing emphysema. It represents the first step in improving our understanding of the role of retinoic acid and similar agents" in this type of research. Dr. Lenfant cautions, "a great deal more basic research is needed before we can even begin to think about applying this to humans." Emphysema which leads to shortness of breath and, eventually, death affects about 2 million Americans and kills 17,000 in the United States each year. Currently, the only treatments for emphysema are lung transplant and surgery to remove diseased portions of the lung.

What Is Emphysema?
Emphysema is a condition in which there is over-inflation of structures in the lungs known as alveoli or air sacs. This over-inflation results from a breakdown of the walls of the alveoli, which causes a decrease in respiratory function (the way the lungs work) and often, breathlessness. Early symptoms of emphysema include shortness of breath and cough.
How Serious Is Emphysema?
Emphysema is a widespread disease of the lungs. In 1993, 1.9 million people in the U.S. had emphysema. It is estimated that 50,000 to 100,000 Americans living today were born with a deficiency of a protein known as alpha 1-antitrypsin (AAT) which can lead to an inherited form of emphysema. Emphysema ranks 15th among chronic conditions that contribute to activity limitations: over 43 percent of individuals with emphysema report that their daily activities have been limited by the disease. Many of the people with emphysema are older men, but the condition is increasing among women. Males with emphysema outnumber females by 22 percent.

NHLBI-Funded Emphysema Study Finds Certain Patients
at High Risk for Death Following Lung Surgery
Emphysema patients who have severe lung obstruction with either limited ability to exchange gas when breathing or damage that is evenly distributed throughout their lungs receive little benefit from lung volume reduction surgery (LVRS) and are at high risk of death from the procedure, according to early results from the National Emphysema Treatment Trial (NETT). NETT is a five-year, multicenter, randomized study to evaluate the role of LVRS in the treatment of severe emphysema. This is the first time that researchers have identified scientifically based selection criteria for patients seeking LVRS as a potential treatment for advanced emphysema. As a result of the findings, patients with these high-risk characteristics are no longer being enrolled in NETT; however, the study will continue to recruit other eligible patients until its scheduled completion in 2002. "This is exactly the kind of insight we hope to gain from NETT," said Dr. Claude Lenfant, director of the National Heart, Lung, and Blood Institute (NHLBI), the primary sponsor of the study. "Identifying which patients should not undergo this surgery is a key goal of this study. We expect the final results of NETT will guide us further in determining if and when this procedure should be used to treat emphysema."

Emphysema: Abnormal irreversible enlargement of air spaces distal to terminal bronchioles caused by destruction of alveolar walls, resulting in decreased elastic recoil properties of lungs. Emphysema is the most common cause of death from respiratory disease in the United States. Cause: Cigarette smoking, deficiency of alpha,-antitrypsin.
Recurrent inflammation associated with release of proteolytic enzymes from lung cells causes bronchiolar and alveolar wall damage and, ultimately, destruction. Loss of lung supporting structure results in decreased elastic recoil and airway collapse on expiration. Destruction of alveolar walls decreases surface area for gas exchange. Symptoms: Dyspnea (labored or difficulty breathing) Chronic cough Anorexia Weight loss Malaise Barrel chest Use of accessory muscles of respiration Prolonged expiratory period with grunting Pursed - lip breathing Tachypnea (rapid respiration) Complications includes: recurrent respiratory tract infections, cor pulmonale, and respiratory failure Treatment: Bronchodilators - to promote mucociliary clearance Antibiotics to treat respiratory infection; Influenza vaccine to prevent influenza; and Pneumococcal vaccine to prevent pneumococcal pneumonia

A study published in the Journal of the American Medical Association suggests that critically ill patients with mild anemia fare better with less blood, not more. Statistics show that patients receiving redcell transfusions for anemia run a greater risk of dying than those where blood was withheld. No one is quite sure why, but it may be that the introduction of additional red cells weakens the immune system in some cases. What is Emphysema? Emphysema is a condition in which there is over-inflation of structures in the lungs known as alveoli or air sacs. This over-inflation results from a breakdown of the walls of the alveoli, which causes a decrease in respiratory function and breathlessness. In Emphysema, the lungs loose elasticity and are unable to fully expand and contract. This occurs because the air sacs cannot completely deflate, thus unable to fill with fresh air for adequate ventilation. Emphysema in children is usually caused by congenital abnormalities of the lung and alpha-1 antitrypsin deficiency. In emphysema, the patient can breathe in but breating out is difficult and inefficient. The seriousness of emphysema varies greatly. Some persons never reach a stage of incapacity and go through life with relatively litte inconvenience, while in others, the disease worsens until final degeneration of the ability to breath occurs.

Peer Review Status: Externally Peer Reviewed by Mosby The assessment of cigarette smokers with dyspnea and airway obstruction is difficult due to the number of primary lung diseases (emphysema, bronchitis, asthma, or bronchiectasis) that can present with a similar constellation of symptoms and objective data. [40] In addition, cardiac failure and pulmonary embolus can mimic primary airway disease. The American Thoracic Society [43] has defined emphysema as a condition of the lung characterized by abnormal, permanent enlargement of airspaces distal to the terminal bronchiole, accompanied by the destruction of their walls, and without obvious fibrosis. The association between exposure to cigarette smoke and emphysema is well documented. [44] However, there also are data suggesting a relationship between fibrosis and smoking. [44, 45] This has raised important questions about the pathogenesis and definition of emphysema. Using a scanning electron microscope, Nagai and Thurlbeck [46] have demonstrated the presence of alveolar wall fibrosis in areas of centrilobular emphysema. Emphysema is classified into three subtypes based on the portion of the acinus that is involved.
