Mission Statement: To improve supportive care and quality of life for ESRD patients and their families. The End-Stage Renal Disease (ESRD) Workgroup worked to improve supportive care and quality of life for dialysis patients from Spring 2000 – Spring 2002. Chaired by Alvin Moss, MD (Director of the West Virginia University Center for Health Ethics and Law), this workgroup consisted of 23 members including nurses, social workers, nephrologists, and kidney transplant recipients. To accomplish its goals, the workgroup divided into three subgroups Education, Quality of Life, and Quality of Dying. The ESRD Workgroup made considerable progress in bringing palliative care issues to the attention of the nephrology community, including an invitation from the Chair of the American Board of Internal Medicine Subspecialty Board on Nephrology to submit questions on end-of-life care for the board certification examination. In addition, the Renal Physicians Association (RPA) and the American Society of Nephrology's (ASN) joint Quality Patient Care Committee meeting revised their position statement on Quality Care at the End-of-Life to incorporate workgroup findings in the Fall of 2001. This revised position statement has been widely circulated. The ESRD Workgroup completed its work with the support of the Robert Wood Johnson Promoting Excellence Program in the winter of 2002, with finalization of a

There are two forms of dialysis. They are Hemodialysis and Peritoneal Dialysis.
Hemodialysis is a procedure that cleans and filters your blood. It rids your body of harmful wastes and extra salt and fluids. It also controls blood pressure and helps your body keep the proper balance of chemicals such as p otassium, sodium, and chloride.
Hemodialysis Dialysis
Hemodialysis uses a dialyzer, or special filter, to clean your blood. The dialyzer connects to a machine. During treatment, your blood travels through tubes into the dialyzer. The dialyzer filters out wastes and extra fluids. Then the newly cleaned blood flows through another set of tubes and back into your body.
Before your first treatment, an access to your bloodstream must be made. The access provides a way for blood to be carried from your body to the dialysis machine and then back into your body. The access can be internal (inside the body usually under your skin) or external (outside the body).

What is renal failure? Renal failure refers to temporary or permanent damage to the kidneys that results in loss of normal kidney function. There are two different types of renal failure - acute and chronic. Acute renal failure has an abrupt onset and is potentially reversible. Chronic failure progresses slowly over at least three months and can lead to permanent renal failure. The causes, symptoms, treatments, and outcomes of acute and chronic are different. Conditions that may lead to acute or chronic renal failure may include, but are not limited to, the following:

Pathogenesis of renal osteodystrophy Ari Kostadaras, M.D. Changes in mineral metabolism and bone structure are an almost universal concomitant of progressive renal failure. There are two classic types of renal bone disease, with many patients showing evidence of both disorders (called mixed osteodystrophy): Osteitis fibrosa, in which bone turnover is increased due to secondary hyperparathyroidism. Osteomalacia, in which bone turnover is reduced with an increased volume of unmineralized bone (osteoid); this problem is in most, but not all patients due primarily to aluminum deposition in bone. Symptoms due to these disorders, such as fractures and bone pain with osteomalacia or bone pain in osteitis fibrosa, generally do not occur until the patient is already on maintenance dialysis. However, subclinical disease begins early, particularly with osteitis fibrosa. SECONDARY HYPERPARATHYROIDISM AND OSTEITIS FIBROSA ­ A tendency to phosphate retention, beginning early in the disease as the fall in glomerular filtration rate (GFR) decreases the filtered phosphate load, plays a central role in the development of secondary hyperparathyroidism . Over the long-term, however, the hyperparathyroidism is maladaptive, producing bone disease and contributing to dysfunction in other organs. Furthermore, there is a changing effect of PTH on phosphate balance in advanced renal failure.

What causes End-Stage Renal Disease?
Diabetes is the most common cause of ESRD, resulting in about one-third of new ESRD cases. Even when drugs and diet are able to control diabetes, the disease can lead to nephropathy and ESRD. Most people with diabetes do not develop nephropathy that is severe enough to cause ESRD. About 15 million people in the United States have diabetes, and about 50,000 people have ESRD as a result of diabetes. Other causes include:

Renal Dialysis Facility - a unit which provides dialysis services directly (i.e., not through contracted providers).
