The kidneys are located below the rib cage in the middle of the back. The function of the kidneys is to filter the blood and form urine. The kidneys have four major functions:
  • Remove waste products from the blood
  • Remove extra fluid
  • Adjust levels of minerals and other chemicals
  • Produce hormones
When the kidneys fail they can no longer remove the wastes and extra fluid from the body. This causes the patient to feel sick. There are many reasons why kidneys fail. Some kidney diseases are inherited, while others are caused by other underlying health problems. Some causes include: diabetes, hypertension, glomerulonephritis or nephritic syndrome. End-Stage Renal Disease (ESRD) occurs when the kidney no longer function. With ESRD, normal kidney function does not return. There are three treatment options that should be considered including: hemodialysis, peritoneal dialysis or transplantation. The patient’s nephrologists and renal care team will meet with the patient and family to guide them through the process of selecting the best treatment modality that meets the patient’s needs and life choices.

The patient with end-stage renal disease (ESRD) is a diagnostic and therapeutic challenge for the emergency physician. Clinical presentations range from the nearly asymptomatic patient with laboratory evidence of mild hyperkalemia to the patient with fulminate pulmonary edema or cardiac arrest. Within a narrow window of time and with limited data, the emergency physician must initiate potentially treacherous therapeutic maneuvers for a patient functioning within narrow metabolic boundaries. By definition, ESRD occurs when a patient’s kidneys can no longer sustain life without the aid of dialysis therapy or a renal transplant. It is at the end point of chronic renal failure where the majority of clinical complications occur. Chronic renal failure (CRF) implies that an irreversible reduction in renal function exists, and recovery of that function is unlikely. Nearly every organ system suffers in ESRD. If profound enough and not treated accordingly, ESRD results in the uremic syndrome. The uremic syndrome is a constellation of multisystemic complications that result from the accumulation of nitrogenous wastes and metabolic by-products. Symptoms may range from fluid overload and congestive heart failure to encephalopathy and coma. To prevent this and other systemic derangements, patients are started on either peritoneal dialysis or hemodialysis. Dialysis therapy can be life-saving but can also cause its own complications. In this chapter, the authors review the complex constellation of problems associated with ESRD from the perspective of the emergency physician.

End-Stage Renal Disease Attributable to Diabetes Mellitus
Annals of Internal Medicine , 15 December 1994; 121: 912-918. Thomas V. Perneger, MD, PhD; Frederick L. Brancati, MD, MHS; Paul K. Whelton, MD, MSc; and Michael J. Klag, MD, MPH Objective: To determine the proportion of end-stage renal disease associated with diabetes mellitus in a biracial population, using population-attributable risk estimates. Design: Case-control study. Setting: Population-based study in Maryland, Virginia, West Virginia, and Washington, D.C. Participants: 716 newly treated patients with kidney failure aged 20 to 64 years and 361 age-matched controls. Measurements: Self-reported history of diabetes mellitus, including type, duration, treatment, and complications. Results: Persons with insulin-dependent diabetes (odds ratio, 33.7) and non-insulin-dependent diabetes (odds ratio, 7.0) were at greater risk for end-stage renal disease than were persons without diabetes. The odds ratio was only slightly increased for diabetes lasting less than 15 years, but the ratio increased more than 20-fold for diabetes lasting 15 years or more. The population-attributable risk for kidney failure was 21% for insulin-dependent diabetes and 21% for non-insulin-dependent diabetes (42% overall). A similar proportion of end-stage renal disease was attributed to diabetes in whites (44%) and in blacks (41%). Insulin-dependent diabetes had a relatively greater effect on the incidence of kidney failure in whites; in contrast, non-insulin-dependent diabetes had a relatively greater effect on kidney failure in blacks.

Acute HCV infection does not occur frequently. However, end-stage renal disease (ESRD) patients on chronic hemodialysis, due to the constant parenteral exposition, remain at risk for this condition. The early diagnosis and treatment of the acute infection decrease the risk of HCV chronic evolution.
The aim of the present study was to describe clinical, laboratorial and virological features of acute HCV infection in ESRD patients on chronic hemodialysis.
Patients on chronic hemodialysis presenting acute increase of ALT levels followed by anti-HCV seroconversion were included between March1999 and May 2002. Both are done monthly in the dialysis centers. HCV-RNA and genotyping were performed by Amplicor (Roche) and sequencing methods, respectively.
At the time of the acute HCV infection diagnosis, the duration of hemodialysis treatment, the presence of signs or symptoms and the ALT features (peak levels and the length between the ALT peak level and anti-HCV seroconversion) were analyzed.

Although hypertension and low income already are linked to an increased risk of end-stage renal disease (ESRD) for both African-American and white men, the two factors may help explain the four-fold higher incidence of ESRD found in blacks, compared to whites, according to a study led by Johns Hopkins researchers published in the April 23 issue of The Journal of the American Medical Association. These results also may help physicians design more effective treatment strategies, says Michael J. Klag, M.D., Hopkins associate professor of medicine and principal investigator for the study. "These results indicate that interventions designed to address the markedly higher incidence of end-stage renal disease in African-American men need to focus on ethnic disparities in blood pressure and socioeconomic status," he says. In the study, data were analyzed from 332,544 men (300,645 white, 20,222 African-American, 11,677 other ethnic groups) aged 35 to 57 years who had been screened for the Multiple Risk Factor Intervention Trial, a study designed to examine the effects of various interventions on coronary heart disease. The incidence of ESRD was assessed using the Health Care Financing Administration national ESRD Treatment Registry. Death from renal disease was assessed using data from the National Death Index and the Social Security Administration.

Renal failure refers to temporary or permanent damage to the kidneys that results in loss of normal kidney function. There are two different types of renal failure - acute and chronic. Acute renal failure has an abrupt onset and is potentially reversible. Chronic failure progresses slowly over at least three months and can lead to permanent renal failure. The causes, symptoms, treatments, and outcomes of acute and chronic are different. Conditions that may lead to acute or chronic renal failure may include, but are not limited to, the following: Acute Renal Failure Chronic Renal Failure Myocardial infarction - a heart attack may lead to temporary kidney failure. Diabetic nephropathy - diabetes can cause permanent changes, leading to kidney damage. Rhabdomyolysis - kidney damage that can occur from muscle breakdown. This condition can occur from severe dehydration, infection, or other causes.

Reprinted From The New England Journal of Medicine - September 20, 2001 The prevalence of end-stage renal disease is increasing at an alarming rate. In 2000, chronic kidney failure developed in ver 90, 000 people in the United States; the current population of patients no dialysis numbers about 300, 000, and 80, 000 patients are living with transplanted kidneys. Both the prevalence and the incidence of end-stage renal disease are approximately twice what they were 10 years ago. Indeed, in 2000, the number of patients with newly diagnosed renal failure exceeded the number who died of any single type of cancer except lung cancer. Nephropathy due to type 2 diabetes accounts for almost all of the increasing number of patients with kidney failure. If the trends of the past two decades persist, approximately 175, 000 new cases of uremia will be diagnosed in 2010. Since the number of kidney transplantations has all but stalled at about 13, 000 per year, the majority of these patients will be treated with dialysis. Improvements in dialysis and transplantation are surely needed. However, caring for patients with end-stage renal disease already consumes more than $18 billion per year in the United States. Six percent of the Medicare budget is used for this service. Shortages of nursing and technical staff for dialysis centers are currently severe, and a shortage of nephrologists is projected. Hence, the prevention of end-stage renal disease is critical. Type 2 diabetes itself may be preventable. Glycemic and blood-pressure control blunt its renal complications.
