Baclofen Toxicity In Patients With End Stage Renal Disease - A Clinical Profile

Abstract Baclofen, a centrally acting Gamma amino butyric acid (GABA) agonist causes neurotoxicity. Solely excreted by the kidney, dosage, modification is recommended in renal failure. Baclofen toxicity was clinically evaluated in dialysis-dependent chronic renal failure patients. Introduction Baclofen is a p Chlorophenyl derivative of the neuro transmitter gamma - amino butyric and (GABA). This centrally acting GABA agonist represents the elective pharamacotherapy for spasticity of spinal cord origin. It inhibits monosynaptic and polysynaptic reflexes, possibly through hyperpolarisation of different fiber terminals. It may also act at supra spinal sites. Baclofen decreases the number and severity of spasms and relieves associated pain, cramps and muscle rigidity and therefore improves mobility . Baclofen is rapidly and extensively absorbed from the Castro - intestinal (Gl) tract, but is subject to individual variations.

Based on published data of the Centers for Medicare and Medicaid Services ("CMS"), the European Dialysis and Transplantation Association, and various industry sources, the number of ESRD patients worldwide requiring dialysis was estimated to be 1,000,000 in 2000, 260,000 of whom reside in the U.S. This population is estimated to be growing at an annual rate of 8%. This growth is primarily attributable to the aging population, increasing life expectancy of patients with a high risk of ESRD and the growth of population subsets with a greater risk of ESRD. Current Treatments: Only a small percentage (5%) of ESRD patients can benefit from organ transplantation, due both to the lack of donor organs and a high rate of transplant rejection. Consequently, the majority of U.S. patients rely on one of two therapies: 1. Hemodialysis: Limitations: Sub-optimal clinical outcomes and high incidence of side effects due to oscillation in toxins, electrolytes and body fluid levels; relatively high U.S. mortality rate. 2.
