These vary the abnormality may appear in one part of the lung at one time, but the next X-ray may show no pathology or a problem in a different part of the lung. The abnormal X-rays are accompanied by a rise in the number of eosinophils (a type of white blood cell which is probably involved in allergies) in the blood. The disease is usually self-limited so it often resolves itself without treatment. Causes and Risks Loffler's syndrome appears to be caused by an allergic reaction. A common cause is the migration of the parasitic worm Ascaris lumbricoides through the respiratory tract. Proteins on the surface of the worm probably incite this allergic reaction. Other parasites of the Ascaris family may also cause the syndrome. Additional possible causes include allergies to medications, for example, sulfonamide antibiotics. Ency. home

40-year-old woman with infiltrates and pulmonary eosinophilia
PG Noone, MD FCCP.
A 40 year old woman with malaise, night sweats, and pulmonary infiltrates. She had had two separate courses of antibiotics (azithromycin and levofloxacin) with no benefit. She had malaise, occasional sweats, a non-productive cough, and nasal obstruction. There was also a central soreness across her chest. Past history included upper airway allergy symptoms requiring occasional courses of anti-histamines, and an intermittent cough but no clear-cut history consistent with asthma. She also had depression requiring fluoxetine (prozac). Physical examination was normal apart from slight pallor. There were no crackles or wheezes. There was no hepatomegaly. Laboratory investigations showed a slight anemia (Hgb 10g/dl), an elevated ESR (118 mm/min), a white cell count of 10,000/L , with an absolute eosinophil count of 0.9 x 109/L. P-ANCA and ANA were weakly positive, C-ANCA, and RF were negative. Serum IgE was normal. Urinalysis was clear. Radiographs are as shown. Dense infiltrates are visible both on the plain radiograph (postero-anterior view, and lateral view), primarily posteriorly in the upper lobes, which is confirmed on the CT scan as shown.

Tryptophan and 5-HTP

The Eosinophilia-Myalgia Syndrome
The eosinophilia-myalgia syndrome (EMS) afflicted thousands, killed dozens and lingers in hundreds of victims to this day. It resembles a chronic autoimmune disease, with aching joints and muscles, elevated white blood cells (eosinophils), fever, skin rash and fibrosis (hardening), swollen limbs, weakness and such long-term complications as lung disease (including pulmonary hypertension) and cognitive impairment. Based on overwhelming evidence, there is a virtual consensus among authorities that the EMS epidemic was due not to tryptophan itself, but to contaminated lots of tryptophan from a major Japansese manufacturer, Showa Denko. Unfortunately, this was not the only time that EMS or EMS-like symptoms occured in users of tryptophan and 5-HTP. Eosinophilia, scleroderma-like lesions, and even primary pulmonary hypertension have made rare appearances before and since. In
