    Contagious Epididymitis
    Definition
    Brucella ovis is responsible for the disease known as Contagious Epididymitis in rams. The disease has never been recorded in the United Kingdom, but has been reported from Australia, New Zealand, the United States of America, South Africa and parts of Europe. Prevalence is thoufht to be high in the Southern European states. Infected flocks may suffer considerable economic loss because of poor fertitlity. There are no records of human infection, and the disease is not classed as a zoonosis.
    Animals affected
    Under natural conditions, Brucella ovis affects only sheep. Goats may be infected experimentally, but most laboratroy species are resistant.
    Clinical signs
    Infection in rams may produce an acute systemis response with fever and depression. The organism may subsequently localise in the epididymis. Inflammatory cells then appear in the semen from 2 - 8 weeks after infection, and the semen quality deteriorates. When infection becomes established in the epididymis B. ovis may be cultured from semen samples from about 3 weeks post-infection: by about 5 weeks most of the rams are positive on semen culture. Excretion of organisms in the semen then probably continues indefinately. Palpable lesions in the epididymis may develop from about 9 weeks post-infection.

    The testicles and epididymis
    Men have two testicles - one in each side of the scrotum - which produce sperm cells and hormones. The sperm is stored and allowed to mature in the epididymis (a convoluted glandular tube the top and side of each testicle) before it is conveyed to the spermatic ducts.
    Although they are positioned close to each other anatomically and share a few common membranes, inflammation in the testicles and in the epididymis can have quite different symptoms.

An increased risk is associated with sexually active men who are not monogamous and do not use condoms . Men who have recently had surgery or have a history of structural problems involving the genito-urinary tract are also at increased risk (regardless of their sexual behaviors).
    Epididymitis may begin with a low grade fever and chills and a heavy sensation in the testicle. The testicle becomes increasingly sensitive to pressure or traction. There may be lower abdominal discomfort or pelvic discomfort, and urination may cause burning or pain. On occasion, there may be a discharge from the urethra, blood in the semen, or pain on ejaculation. The testicle may enlarge significantly and produce severe pain. Epididymitis is an important clinical syndrome because it has similar symptoms as testicular torsion. Testicular torsion is a surgical emergency and should be treated as soon as possible. Acute testicular pain should never be ignored.

    This is pretty rare before puberty. It is an acute inflammation or infection of the epididymis, a structure that lies along the spermatic cord in the scrotum. It presents as sudden painful scrotal swelling. There usually is pus in the urine. The germ that causes it is usually bacterial; we are not always successful in figuring out exactly which germ it is. The major problem with epididymitis lies in the fact that differentiation from torsion of the testicle can be very difficult. Surgical exploration is often the only way to be sure, and must be undertaken very quickly to save the testicle if torsion is present. If the problem is just infection, bed rest and antibiotics are curative.

    Presentation
    An adult male complains of dull to severe scrotal pain developing over a period of hours to a day, and radiating to the ipsilateral lower abdomen or flank. There may be a history of recent urethritis, prostatitis or prostatectomy (allowing ingress to bacteria), straining with lifting a heavy obiect, or sexual activity with a full bladder (allowing reflux of urine). There may be fever, nausea, or urinary urgency or frequency. The epididymis, is tender, swollen, warm, and difficult to separate from the firm, nontender testicle. Increasing inflammation can extend up the spermatic cord and fill the entire scrotum, making examinations more difficult, as well as produce frank prostatitis or cystitis. The rectal exam therefore may reveal a very tender, boggy prostate.
    What to do:
    • Ascertain that the testicle is normal in position and perfusion. Doppler ultrasound may help pick up a drop-off in arterial flow from spermatic cord to testicle in testicular torsion.
    • Palpate and ausculate, the scrotum to rule out a hernia. Gently palpate the prostate once. Culture urine and/or any urethral discharge to identify a bacterial organism.

The epididymis is a soft, coiled tubular structure on the back of the testicle. What is going on in the body? Infections generally spread to the epididymis from the bladder or the urethra. The urethra is the tube that allows urine to pass from the bladder to the outside of the body. In young children or older men, bacteria that cause urinary tract infections are often the cause. Sexually transmitted diseases are more likely to be the cause in sexually active young men. Infection in the epididymis causes severe pain and swelling. The infection may spread to the testicle. When both structures are inflamed, the condition is known as epididymo-orchitis. It is rare that an infection will occur in the testicle alone. The bacteria will usually infect the epididymis first. What are the signs and symptoms of the disease? Symptoms of the condition include: pain in the scrotum. This can range from moderate to severe. tenderness in the epididymitis and scrotum. Only the epididymis is tender at first. The entire scrotum can become swollen and tender as the problem progresses.

    Epididymitis refers to inflammation of the epididymis, which is a tube system, lying on top of the testicle.
    The most common causes of this condition are infection by Chlamydia or Gonorrhoea and it represents a complication of infection from the urethra (and you might consider it the male equivalent to pelvic inflammatory disease ). If the infection involves the epididymis on both sides there is a (small) threat of sterility. The condition can be very painful and can even be a "Waiting Room" diagnosis on seeing a male patient limping and grimacing into the clinic. It may occur in conjunction with a discharge but this is not always the case. The infection may occur as a complication of a urinary tract infection, highlighting that sexual transmission is not always necessary. A rare cause of the problem is the virus that causes mumps, and even then infection of the testicle itself is more likely. Testing The diagnosis is made by clinical examination by the doctor, feeling for tenderness around the epididymis and testicle. In conjunction with this, swabs are taken from the urethra for the routine testing of chlamydia and gonorrhoea. Treatment will often be started on the basis of the clinical examination alone as the infection swabs will take up to a week to process.
