Epididymitis Epididymitis is a condition in which the epididymis becomes swollen, painful and usually infected. The epididymis is one of a pair of tightly coiled ducts in the testicles, through which sperm pass as they exit the body. If you have epididymitis, you will probably be in considerable pain. You will notice that one testicle is intensely tender, swollen and red. You may have a fever and chills. It is important to make sure that your symptoms are due to epididymitis and not from another condition, called testicular torsion. Because they have similar symptoms, they are often mistaken for each other, especially in younger men. Testicular torsion occurs when the testicle twists in the scrotum. This cuts off blood supply to the testicle. Testicular torsion requires emergency treatment. If you have symptoms of epididymitis, it is crucial that you seek medical advice to verify that the symptoms are not due to testicular torsion because early treatment of testicular torsion can save the testicle.     WHAT YOU SHOULD KNOW
    Epididymitis (EP-ih-DID-ee-MY-tis) is an infection of the epididymis, a tube located behind the testicle. The infection is common in men 19 to 35 years old.
    Causes
    The problem may start with an infection of the bladder or prostate gland. The infection is sometimes contracted through sexual contact.
    Signs/Symptoms
    The usual symptoms are fever, pain, redness, and swelling of the scrotum and painful urination. You also may feel a lump in your scrotum.
    Care
    Your doctor will prescribe an antibiotic to fight the infection. Rest until you feel better, and follow the directions listed below.
    WHAT YOU SHOULD DO
    • Rest in bed until fever, pain, and swelling go down. The testicle may stay swollen and hard for several days or even a few weeks.
    • If your doctor prescribes an antibiotic to fight the infection, take it exactly as directed and finish the entire prescription. If you stop taking the drug too soon, a few germs may survive and re-infect you.

    Epididymitis is an infection of the epididymis. The epididymis is a soft, coiled tubular structure on the back of the testicle. What is going on in the body? Infections generally spread to the epididymis from the bladder or the urethra. The urethra is the tube that allows urine to pass from the bladder to the outside of the body. In young children or older men, bacteria that cause urinary tract infections are often the cause. Sexually transmitted diseases are more likely to be the cause in sexually active young men. Infection in the epididymis causes severe pain and swelling. The infection may spread to the testicle. When both structures are inflamed, the condition is known as epididymo-orchitis. It is rare that an infection will occur in the testicle alone. The bacteria will usually infect the epididymis first. What are the signs and symptoms of the disease?

The pain may have a rapid onset and may radiate along the spermatic cord and even reach the flank. The scrotum is usually swollen and red. The enlarged hardened epididymis may be palpable, but usually cannot be clinically distinguished from the testis. Acute epididymitis Two main types of acute epididymis are recognized: a sexually transmitted form seen in young men, due to chlamydia or gonococci and a nonsexually transmitted form in older men with prostate hyperplasia, prostatitis or urinary tract infection due to enterobacteria or pseudomonas . Older men who have undergone transurethral prostatectomy may also develop epididymitis, since resection in the region of the verumontanum may allow reflux of infected urine into the vas deferens.

      In 1 series, 64% of children less than 1 year old age and 21% of older children with epididymitis had vesicoureteral reflux, ureteral ectopia, or urethral strictures. Although the foreskin may be an important risk factor for infection, all children with epididymitis should have a complete urological working.

    The epididymis is a crescent shaped structure. It runs down the middle of the back of the testis from top to bottom. It functions to carry the sperm from the testes (where they are produced) to the vas deferens, which is a muscular tube that carries the sperm. The epididymis is more susceptible to infection than the other structures in the scrotum and this infection is called epididymitis. Infections can subsequently lead to scar tissue, which can block the sperm from moving through it and trap it in the testes (or the part of the epididymis before the blockage.) The sperm are still being produced; they are just not able to come through. If this happens on both sides a man may have no sperm in the ejaculate.     EPIDIDYMITIS Common Name: None Specified Description: Infection or inflammation of the epididymis, which is a tubular structure attached to the upper posterior part of each testicle, and results in scrotal pain. Causes: Causes depend on the age of the patient: In younger men ( less than 35 )- Sexually transmitted disease e.g.. Chlamydia or Gonorrhea and E. coli infection, associated with anogenital sex- can be seen in gay men. Older than age 35- Non-sexually transmitted disease associated with prostatitis, post prostatectomy, and benign prostatic hypertrophy (obstruction leads to high voiding bladder pressures and causing reflux into the epididymis). Infection is usually due to gram-negative bacteria ,namely Eschericia coli (E. coli) and Pseudomonas aeruginosa. In children , the infection is due to E. coli . Children presenting with epididymitis should always be investigated for underling congenital abnormalities, such as vesicouretral reflux ( reflux of urine from the bladder into the ureter) or an ectopic ureter (abnormal position of ureter). Also,epididymitis may occur in patient on treatment with a drug called Amiodarone (this medication is used in treatment of an irregular heart beat). This is a non-infectious epididymitis, that resolves on decreasing the drug dose.

    • In sexually active men under 35 years old, usually bacteria get in during sexual intercourse and travel to the epididymis. Sometimes it is caused by the chlamydia bacteria or gonorrhea. In men older than 39 years, it may be related to rectal intercourse. In men over 35 years, it is also related to problems with the prostate gland and those who have a urinary catheter put in.     • You will need to take antibiotics. Sometimes you will need to be admitted to the hospital to get the antibiotics through an IV (needle that goes into a vein). Your sexual partner may also need to take antibiotics.
