GENERAL INFORMATION Epididymitis is an inflammation of the epididymis, an oblong structure attached to the upper part of each testis. The inflammation may be so great that the testicle itself feels greatly enlarged and hardened. However, this does not mean that the testicle itself is infected.
Some rarer types of bacteria create a different picture altogether. Tuberculosis, for instance, is a bacteria that can spread to the epididymis in unusual circumstances. Tuberculosis epididymitis develops very slowly over days and weeks. Non bacterial epididymitis simply means an inflammation of the epididymis not caused by a bacteria. The causes are many and include viruses, trauma, and unknown causes. The unknown causes are commonly referred to in medicine as 'idiopathic' - an unknown cause. Non-bacterial epididymitis can be just as painful and with a very quick onset. The viral types may be associated with fevers and generalized weakness and ill feeling. Epididymitis caused by trauma or the 'idiopathic' types are not usually associated with the fevers and chills and generalized symptoms. Many believe that these types of epididymitis are caused by a rupture or blow-out of the sperm ducts causing a leakage of sperm fluid into the tissue. The body tissues can be quite irritated by this leakage and hence the redness and swelling. The treatment for these types of epididymitis may include antibiotics just to prevent bacteria from infecting the area whose resistance has been weakened.

Definition Epididymitis refers to inflammation of the epididymis, which is a tube system, lying on top of the testicle. The most common causes of this condition are infection by Chlamydia or gonorrhea and it represents a complication of infection from the urethra (and you might consider it the male equivalent to pelvic inflammatory disease ). If the infection involves the epididymis on both sides there is a (small) threat of sterility. The condition can be very painful and can even be a "Waiting Room" diagnosis on seeing a male patient limping and grimacing into the clinic. It may occur in conjunction with a discharge but this is not always the case. The infection may occur as a complication of a urinary tract infection, highlighting that sexual transmission is not always necessary. A rare cause of the problem is the virus that causes mumps, and even then infection of the testicle itself is more likely.
Signs and Symptoms Infection with this virus causes severe pain in the testicular region, and is often accompanied by inflammation.

Peer Review Status: Externally Peer Reviewed Among sexually active men aged less than 35 years, epididymitis is most often caused by C. trachomatis or N. gonorrhoeae. Epididymitis caused by sexually transmitted E. coli infection also occurs among homosexual men who are the insertive partners during anal intercourse. Sexually transmitted epididymitis usually is accompanied by urethritis, which often is asymptomatic. Nonsexually transmitted epididymitis associated with urinary tract infections caused by Gram-negative enteric organisms occurs more frequently among men aged greater than 35 years, men who have recently undergone urinary tract instrumentation or surgery, and men who have anatomical abnormalities. Although most patients can be treated on an outpatient basis, hospitalization should be considered when severe pain suggests other diagnoses (e.g., torsion, testicular infarction, and abscess) or when patients are febrile or might be noncompliant with an antimicrobial regimen. Diagnostic Considerations
Men who have epididymitis typically have unilateral testicular pain and tenderness; hydrocele and palpable swelling of the epididymis usually are present. Testicular torsion, a surgical emergency, should be considered in all cases but is more frequent among adolescents. Torsion occurs more frequently in patients who do not have evidence of inflammation or infection. Emergency testing for torsion may be indicated when the onset of pain is sudden, pain is severe, or the test results available during the initial examination do not enable a diagnosis of urethritis or urinary tract infection to be made. If the diagnosis is questionable, an expert should be consulted immediately, because testicular viability may be compromised. The evaluation of men for epididymitis should include the following procedures:

And before I go any farther, I should briefly review the structures of the male reproductive system because they are all potentially involved in your problem.
The epididymis empties into the vas deferens. This tube which you can feel within your scrotum above the testicle carries mature sperm from the epididymis through the inguinal canal (the hernia area) and across the inner surfaces of the pelvis to the prostate gland. The prostate mixes its own fluid and that from the seminal vesicles and other sources with the sperm to produce the ejaculate of the male reproductive system.
Another common cause of repeated epididymitis, and the most likely cause for you, is chronic infection in the prostate gland. The low-grade infection spreads down the vas deferens and causes the new episodes of epididymitis. Unfortunately, chronic prostate infections are difficult to eradicate. It often takes special antibiotics that must be taken for four or more weeks to clear up the problem.
Infections of the epididymis or the prostate do not cause cancer. Once the infection has been cleared up, your discomfort should be over. However, you still have some risk of developing prostate cancer and testicular cancer just like every man. Performing monthly testicular self-exams is your best protection against testicular cancer.

Epididymitis is typically caused by bacterial organisms associated with urinary tract infections , sexually-transmitted diseases (such as chlamydia and gonorrhea ), prostatitis (infection of the prostate) or prostatectomy (removal of the prostate). An increased risk is associated with sexually active men who are not monogamous and do not use condoms . Men who have recently had surgery or have a history of structural problems involving the genito-urinary tract are also at increased risk (regardless of their sexual behaviors).
Epididymitis may begin with a low grade fever and chills and a heavy sensation in the testicle. The testicle becomes increasingly sensitive to pressure or traction. There may be lower abdominal discomfort or pelvic discomfort and urination may cause burning or pain. On occasion there may be a discharge from the urethra, blood in the semen or pain on ejaculation.
Prevention:
Epididymitis may be prevented by early diagnosis and adequate treatment (plus reporting if applicable) of the infectious diseases associated with it. Prophylactic (preventive course) antibiotics are frequently given at the time of surgeries.

Epididymitis can result from several causes. A urinary tract infection can spread through the sperm ducts to the epididymis. This disorder can also be a result of prostatitis (infection of the prostate gland), and it can occur after a prostatectomy (surgical removal of the prostate). Organisms that commonly infect the region this way include staphylococci, streptococci, and Escherichia coli. Epididymitis is also a common complication of sexually transmitted diseases, primarily chlamydia, but also gonorrhea and syphilis. In some cases, epididymitis develops as a complication of long-term use of an indwelling catheter.
If you think you have epididymitis, contact your physician immediately. Diagnosis is made by analysis of a urine sample and, sometimes, prostate secretions, to identify the source of the infection. In sexually active men with multiple partners, epididymitis is very often the result of urethritis (infection of the urethra) caused by chiamydia (Chlamydia). If it is determined that the disorder was sexually transmitted, both partners should be treated to prevent them from passing the infection back and forth. It is important thatthe infection be adequately treated. If it becomes chronic, then epididymitis can cause the formation of

Epididymitis
Bacterial invasion of the epididymis results in classic acute inflammation. Tissue is congested, edematous, with a neutrophilic progressing to monocytic and lymphocytic infiltrate. Infection is initially limited to the intersitial connective tissue, but extension into the tubules occurs. Frank abscess formation or suppurative necrosis may result. This specimen demonstrates edema in the connective tissue of the tunica and pus in epididymis. The testicle is relatively uninvolved.
Epididymitis in childhood is usually associated with a congenital genito-urinary abnormality and infection with gram-negative rods.
In sexually active males, Chlamydia and Neisseria are the most frequent causes of epididymitis.
In men „ 35 years of age, gram negatives such as E. coli and Pseudomonas are responsible for most infections.
