In addition to its original linkages with Burkitt's lymphoma and infectious mononucleosis, Epstein-Barr virus is now believed to be the cause of nasopharyngeal cancer, T-cell lymphoma, natural killer (NK) cell lymphoma, Hodgkin's disease and gastric carcinoma. As usual, the anti-smokers ignore the role of this virus in order to falsely blame smoking and passive smoking.

Current Activities and Past Findings

Epstein-Barr is just one of several flight experiments that has monitored the reactivation of latent viruses during space flight. The stresses of space flight can also reactivate viruses such as herpes simplex-1 (a virus related to Epstein-Barr and chicken pox), which most commonly causes sores around the mouth, nose, chin, and cheek, but can also cause pharyngitis and tonsillitis. Detailed Supplementary Objective (DSO) research focusing solely on Epstein-Barr virus reactivation began during STS-108 (December 2001). The Expedition 5 crew will be the first participants to provide information on virus reactivation during long-duration space flight. Earlier research by the Epstein-Barr researchers (see R.P. Stowe et al, 2000; 2001) indicated that physical and psychological stress, measured through stress hormones present in postflight urine samples, resulted in decreased virus-specific T-cell immunity and reactivation of Epstein-Barr virus (EBV) in Shuttle astronauts. Their findings also have shown that reactivation begins, due to stress, before flight and acute lytic replication occurs during flight. These findings are the basis of the current investigation. EBV was first discovered as part of research into a cancer, later called Burkitt's Lymphoma, found in children from Africa. EBV, however, is one of the most common human viruses, affecting people worldwide. Although it frequently remains latent throughout the carrier's lifetime, it can be reactivated, causing moderate to severe illnesses and long-term, life-threatening complications. People with compromised immune systems, such as organ transfer and AIDS patients, run a particular risk of the virus becoming active. Stress can also reactivate it.

Epstein-Barr virus linked to invasive breast cancer and lymphoma
U-M scientists find virus releases molecular brake on roaming cancer cells ANN ARBOR, MI Scientists in the University of Michigan Medical School have found a molecular link between aggressive breast and lymphatic cancers and the Epstein-Barr virus, which causes infectious mononucleosis. In a paper published in the March 2001 issue of Nature Medicine, U-M scientists show how the Epstein-Barr virus alters the function of a cellular protein that normally suppresses the movement of malignant cells. When this natural brake on cell migration is disabled by the virus, cancerous breast and lymphatic cells are free to metastasize or spread. "This is the first evidence of a human virus associated with the development of cancerous tumors targeting a cellular protein to promote the migration of malignant cells," says Erle S. Robertson, Ph.D., who directed the study. Robertson is an assistant professor of microbiology and immunology in the U-M Medical School and a researcher in the U-M's Comprehensive Cancer Center. "The Epstein-Barr virus is associated with many human cancers including Burkitt's lymphoma, nasopharyngeal carcinoma, Hodgkin's disease and invasive breast cancer," Robertson says.

High Levels of Epstein-Barr Virus Antibodies in Women Linked to Risk of Multiple Sclerosis

Boston, MA— Researchers from the Harvard School of Public Health have found that elevated levels of specific antibodies that fight a range of Epstein-Barr Virus (EBV) antigens are associated with the onset of multiple sclerosis (MS). The study results appear in the December 26, 2001 issue of the Journal of the American Medical Association (www.jama.com).
Blood samples from more than 62,000 women were collected between 1989 and 1999. The researchers documented 144 cases of MS including 18 cases with blood samples collected before the onset of the disease. For each woman with MS, two without MS, who were the same age were randomly chosen as controls. Compared to their controls, the women with MS had significantly higher average blood levels of antibodies for EBV antigens before the onset of the disease. The strongest association was found for antibodies for Epstein-Barr Nuclear Antigen-2 (EBNA-2). These levels increase soon after EBV infection but normally decline within a few months. In this study, elevated levels of EBNA-2 antibodies were associated with a four-fold increase in risk of MS.

5.1 Virus-host interactions
Epstein-Barr virus (EBV) is a gamma-1 herpesvirus found throughout all human populations, with a prevalence of over 90% in adults. Primary infection usually occurs in early childhood and is asymptomatic, whereas delayed primary infection may cause a self-limiting lymphoproliferative disease, infectious mononucleosis. Infection results in the establishment of a life-long carrier state characterized by the persistence of antibodies to several viral gene products and the secretion of infectious virus in saliva. Saliva is the usual vehicle of transmission. EBV can induce growth transformation of human and primate B-lymphocytes in vitro Cellular and perhaps humoral immune responses contribute to the control of primary EBV infection and mediate the transition to asymptomatic persistence of the virus in healthy carriers. The humoral responses include the production of neutralizing antibodies directed to the CD21-binding protein gp350/320 and antibodies directed against lytic and latent gene products. Cells expressing type-III latency, which are detected during primary infection, are promptly eliminated by CD8 major histocompatibility (MHC) class I-restricted cytotoxic T lymphocytes which recognize epitopes from all the latent viral proteins except EBNA-1. Accordingly, only EBNA-1 appears to be expressed in the latently infected B cells that persist in healthy virus carriers. The failure of this protein to elicit cytotoxic T-lymphocyte responses could be instrumental in allowing persistence of virus-infected cells in immunocompetent hosts. CD4

These viruses are closely related members of the gamma herpesvirus family and are similar to each other with respect to genomic structure and gene organisation. The EBV genome is composed of linear, double-stranded DNA, which is approximately 172 kb in length (see Fig. 1a; ), and regions of the EBV genome have become known by their position on a Bam HI restriction endonuclease map of the genome (see Fig. 1b EBV is orally transmitted and infectious virus can be detected in oropharyngeal secretions from infectious mononucleosis (IM) patients, from immunosuppressed patients and, at lower levels, from healthy EBV-seropositive individuals (Ref. ). Epstein-Barr Virus. Disease type: Viral infection. Figure V.02.01 - The EBV attaches to the outside of the cell, and is internalised by the cell. Once inside the virus releases its DNA content, which enters the nucleus and integrates into the hosts DNA. The viral proteins will then be expressed, to begin the formation of new virus.
Pathology
With EBV this process is not 100% effective, and a small population of EBV +ve cells will remain. This gives rise to life-long infection. Once EBV has been caught, it cannot be removed. Figure V.02.03 - Translocation of EBV through the genome. This erroneous copying can lead to cancer.
Aetiology
EBV by itself does not cause disease. But its constant gene translocations can. B cells, of the immune system, will secrete Antibody at a constant level throughout the cells life-span. B cells can become cancerous if a oncogene is placed next to the Antibody gene. Antibodies are produced in great excess, and as the oncogene is next door to the antibody, it too will be produced in excess. The Oncogene will result in a leukemia. EBV has been linked positively with the incidence of Hodgkins disease, Burkitts Lymphoma, and Nasophangeal carcinoma, to name but a few. Figure V.02.02 -

Epstein-Barr virus
All tumor-associated viruses have evolved strategies for survival and spread as part of their physiologic infection, which affects cell cycle and apoptosis control. By inducing transient proliferation in their target organ/cells they prepare for lytic replication by engaging more cells, as an amplification mechanism. Virus infections per se induce apoptosis, which may be regarded as a defence mechanism by the host/host cell. Progression from cells at risk to overt tumor only takes place if late replicative events are truncated or blocked (e.g. by integration, accidental infection of wrong cell type), if the infection cannot be handled by the immune system and/or if the infection raises an inappropriate host reaction.
Epstein-Barr virus (EBV) infection in man, illustrates these points, due to extenisve though incomplete knowledge on several of these issues. EBV is the most common virus in the human population (>90% of adults carry the virus), it normally gives rise to an inconspicuous or benign infection, paradoxically, however, under rare circumstances it induces tumors and in vitro it is very efficient in transforming its natural host cell, the B-lymphocyte.
