DISEASES OF THE ESOPHAGUS
Organ of transport, by peristalsis
Upper Esophageal Sphincter (UES) is an actual muscular sphincter. Upper 1/3 of the esophagus has skeletal muscle, while the distal 2/3 has smooth muscle. Lower Esophageal Sphincter (LES) is a zone of resting high pressure in the distal esophagus, approximately 20 mm Hg above gastric baseline. Peristalsis, the progression of a pressure wave through the body of the esophagus, can be measured by esophageal manometry. It has a normal velocity and amplitude.
  • Thus, simultaneous or multiphasic esophageal contractions are not peristaltic.
  • A single wave of peristalsis doesn't have to clear the esophagus. A second wave come through to clear anything remaining.
Normal Swallow
Begins with relaxation of UES, allowing food into the upper esophagus. Peristaltic waves push the food down to the LES, which relaxes to gastric baseline, allowing food into the stomach. Coordination of swallowing in the upper 1/3 of the esophagus occurs centrally while in the lower 2/3 it is coordinated by local nerve plexuses.

TRAINING PROGRAMS PROGRAM DESCRIPTION Brown University and Lifespan Affiliates (Rhode Island Hospital, Providence Veteran's Administration Hospital) offer a three-year accredited Training Program in Gastroenterology the goal, of which, is to prepare Board-eligible internists for a career in clinical or academic Gastroenterology; the Program fulfills all training standards as mandated by the A.C.G.M.E., A.B.I.M., A.S.G.E., A.G.A., and A.C.G. The atmosphere is scholarly, and provides wide exposure to clinical GE medicine, and clinical/basic scientific research. CAREER DEVELOPMENT
Clinical Track:
The GE Division provides a strong background in hepatology, gastrointestinal motility, inflammatory bowel disease, and GE malignancy. Extensive experience in specialized endoscopic techniques is available at the PGY-6 level to include diagnostic/therapeutic biliary endoscopy, sphincterotomy, pancreatic/esophageal stent placement, and ultrasound. Fellows routinely perform +2000 endoscopic procedures within the three-year tenure.
Clinical Scholar Track:
In addition to the above, extensive experience in clinical investigations is offered to the Fellow who wishes to assume an academic position as Clinical Teacher at a University, or hospital.

Rates of Helicobacter pylori infection are decreasing, but that may not be good news.
August 2000 WASHINGTON, D.C. - Although Helicobacter pylori increases the risk for ulcer disease and gastric cancer, its presence may decrease the risk of esophageal diseases including adenocarcinoma, according to Martin J. Blaser, MD. "Having H. pylori is a trade-off. If you have it, it's bad for your lower stomach. If you don't have it, it's bad for your esophagus," Blaser said at a press conference here sponsored by the National Foundation for Infectious Diseases. Gastric cancer was the leading cause of cancer deaths in the West, explained Blaser, who is chairman of the department of medicine at New York University School of Medicine, New York City. "But it has been declining over the course of the 20th century. And we now know that that decline is in part due to the disappearance of H. pylori ." said Blaser, who is also the Frederick H. King Professor of internal medicine and professor of microbiology at NYU.

Increasing cancer rates
Although squamous cancer of the esophagus also has been declining, adenocarcinoma of the esophagus has been increasing dramatically. "It is the fastest increasing cancer in the United States. It is increasing at a rate of 11% a year, compounded annually," Blaser said. The rate of increase is even higher in other parts of the world.

    The Division of Esophageal Surgery is responsible for the management of patients with disorders of the esophagus and gastric cardia. Specialist services include the assessment and treatment of patients with swallowing problems, esophageal motility diseases, gastroesophageal reflux, perforation, diverticulum, caustic substance ingestion, and benign and malignant tumors of the esophagus. The Division provides the following specialist services:
    • Diagnostic and therapeutic upper GI endoscopy Endoscopic ultrasonography GI physiology including esophageal manometry and 24-hour ambulatory pH monitoring Multidisciplinary treatment of esophageal cancer including chemo-radiotherapy Multidisciplinary treatment of cancer of the gastric cardia including regional chemotherapy Minimally invasive surgery for cancer as well as benign esophageal diseases
    DIAGNOSTIC AND THERAPEUTIC UPPER GI ENDOSCOPY
    Diagnostic upper endoscopy is performed for patients with a variety of upper gastrointestinal symptoms including dysphagia, odynophagia, heartburn and regurgitation, and epigastric pain. Endoscopic ultrasonography is routinely performed for tumor staging.
