The objective of this study was to determine whether a specific genetic profile is associated with a higher risk of developing a postoperative thromboembolic complication. Specifically, we examined if the Factor V Leiden (FVL) mutation or the deletion polymorphism of the angiotensin-converting enzyme (ACE) gene increased a patient's risk for postoperative thromboembolic events. The FVL mutation has been associated with an increased risk of idiopathic thromboembolism and the deletion polymorphism of the ACE gene has been associated with increased vascular tone, attenuated fibrinolysis and increased platelet aggregation.

    I'm a 37 yr old woman who was diagnosed at 31 as having had a silent heart attack, although the doctors don't know when it
    happened. I was having PVCs(benign), which I mentioned to my Internist during a routine exam. An ekg showed abnormal results, and an echo showed a small area of damage. I next had a thallium stress test, and a heart catheterization showed damage from a moderate heart attack, although I had normal coronary arteries. I had taken the Pill for 5 yrs prior to diagnosis, and my cardiologist is assuming that the MI was caused by a blood clot, possibly related to the pill, although I guess I will never know for sure what caused it. I've had several echos and stress tests since diagnosis, with no change.I have never smoked or taken drugs, drink rarely and am otherwise healthy,although I am 35 lbs overweight. My EF is 50, blood pressure normal, all other tests have come back negative/normal:clotting/coagulation tests,diabetes,lupus, Lyme,excess iron,hepatitis,HIV, lipoprotein a, anti-phosholipid antibody, anti-nuclear antibody tests,etc. I currently take 1 mg folic acid, 81 mg aspirin, 25 mg atenolol, fish oil and a multivitamin. My total cholesterol:186, HDL:43 LDL:130 Trig:68.

    Bleeding after cardiac surgery is a major clinical problem, consuming up to 20 percent of the nation’s blood supply. Despite some understanding of the risk factors that contribute to increased blood loss, there is still considerable patient-to-patient variability, said Dr. Brian S. Donahue, assistant professor of Anesthesiology at Vanderbilt University Medical Center. Donahue and colleagues are the first to show that a common genetic variation contributes to a patient’s tendency to suffer bleeding complications. The investigators report Feb. 25 in Circulation that cardiac surgery patients who have a different genetic form of one clotting factor lose less blood and are less likely to receive blood transfusions. The findings herald a new era in medicine, one in which therapies will be customized for individual patients, based in part on the genes they carry. That is the hope anyway, Donahue said. “If we can outline a patient’s specific risk factors for given complications, then we can better tailor our therapeutic approach,” he said. The current study focuses on factor V (“factor five”), one of the proteins essential to normal blood clotting. A variant form of the gene, called factor V Leiden, is present in about five percent of the white population in the United States, making it one of the most common inherited blood coagulation mutations. Individuals with factor V Leiden are at increased risk of forming blood clots, especially deep venous thromboses.

    General Information:
    The Factor V Leiden mutation (R560Q) is present in 20-40% of patients with venous thrombosis, 40-65% of adults from families with unexplained thrombophilia, and approximately 95-100% of patients with the phenotype of activated protein C (APC) resistance. The Factor V Leiden mutation is a significant risk factor for venous thrombosis, particularly in the homozygous state or in combination with additional risk factors such as pregnancy, use of oral contraceptives or surgery. Methodology: This test includes preparation of DNA from peripheral blood and PCR based analysis for direct detection of the common Factor V Leiden mutation (R506Q). PCR is performed pursuant to a license agreement with Roche Molecular Systems, Inc. Use: Identification of individuals heterozygous or homozygous for the Factor V Leiden mutation. Indications: Venous thrombosis or history of a thrombotic event, activated protein C resistance by laboratory analysis, presence of another hereditary coagulation disorder (protein C deficiency, protein S or antithrombin III), family history of thrombosis or known Factor V Leiden mutation.     ABSTRACT Background Venous and arterial thromboembolism occurs in only about one third of patients homozygous for homocystinuria, which suggests that other, contributory factors are necessary for the development of thrombosis in these patients. Factor V Leiden, an R506Q mutation in the gene coding for factor V, is the most common cause of familial thrombosis and could be a potentiating factor. Methods We determined activated partial-thromboplastin times in the presence and absence of activated protein C and tested for the factor V Leiden mutation in 45 members of seven unrelated consanguineous kindreds in which at least 1 member was homozygous for homocystinuria.

    Abstract
    Objective: To determine the prevalence of factor V Leiden in a Canadian blood donor population. Design: Cross-sectional laboratory study. Setting: Hamilton Centre of the Canadian Red Cross Society. Participants: Volunteer donors who attended Hamilton Centre blood donor clinics over a 4-day period in August 1994; blood samples from 356 people were evaluable. Outcome measures: Presence of factor V Leiden. Results: Factor V Leiden was detected in 19 of the 356 people, for a prevalence rate of 5.3% (95% confidence interval 3.0% to 7.6%). All 19 people were shown to be heterozygous for the mutation. Conclusion: Factor V Leiden is common in the Canadian population. Its prevalence is similar to that reported in other Western countries. These data are relevant in the clinical management of patients at risk for venous thrombosis and those with recurrent thrombotic disorders.

    by Atef Shrit, M.D. Prior to 1993, the diagnosis of hereditary thrombophilia could be established in less than 5% of patients under 50 years of age presenting with idiopathic (without identifiable precipitating factors) venous thromboembolism. The major disorders known at that time were deficiencies of antithrombin-III, protein C and protein S. With the discovery that many patients with unexplained venous thrombosis have activated protein C (APC) resistance due to Factor V Leiden mutation or a prothrombin gene mutation, it became possible to define genetic risk factors in many younger patients with idiopathic venous thromboembolism. Activated Protein C Resistance Due To Factor V Mutation (Leiden)
    Functional assay and DNA testing (Factor V Leiden) for resistance to activated protein C are locally available at CompuNet Clinical Laboratories.     Thrombophilia Panel - DNA Analysis Genetic risk factors are involved in the predisposition of individuals to venous thrombosis. These factors include inherited resistance to activated protein C (APC) as well as deficiencies in antithrombin III, protein C and protein S. APC resistance is a major basis for familial thrombosis and is common in the general population (2-5%). APC resistance is frequently associated with factor V Leiden, a point mutation in the factor V gene. Individuals heterozygous for this mutation have an increased risk for venous thrombosis (approximately 5- to 10-fold). Individuals homozygous for this mutation have an even higher risk for venous thrombosis (approximately 50- to 100-fold). Prophylactic anticoagulation therapy should be considered for carrier individuals at risk in situations where venous stasis is likely (i.e. prolonged bed rest after surgery). This is a common inherited abnormality in the Factor V protein which is important in clotting. It is named after the city in the Netherlands that it was first identified in. How Do You Get It Factor V Leiden is the most common inherited blood abnormality that results in the development of blood clots. Because it is inherited, it is important that family members of affected persons be identified and tested. What Should You Do About It Persons who are found to have this abnormality but have not had any history of blood clots should receive preventative measures during times when there is a greater risk of developing blood clots. Because there is an increased risk of the development of blood clots in women with high hormone levels, those using birth control pills, pregnant, or on hormone replacement therapy may be at even greater risk for developing complications.
