Farsightedness or hyperopia occurs when an eye is too short for the cornea's curvature. Light rays entering the eye focus behind the retina, and as a result a blurred image is produced. Farsighted individuals, however, can use their focusing muscles to 'pull' the image forward onto the retina. In a young person with severe hyperopia, or once presbyopia sets in around age 45, distant objects are seen more clearly than near objects. Certain laser technologies, such as holmium lasers and hyperopic LASIK may correct this condition.

Normal Eye Farsighted Eye
Compared to the normal eye, the farsighted eye usually has grown too short. The refractive point of incoming light rays is behind rather than on the retina.
The farsighted eye differs from the nearsighted eye insofar as up to a certain age farsightedness can be accommodated by the lens of the eye. The eye lens is able to change shape and thereby increases its refractive power (accommodation), which makes up for the decreased refractive power of the farsighted eye for distance vision.
Farsighted individuals usually are able to see images at a distance well, even at an older age. Eyeglasses for younger people are needed only in severe cases of farsightedness. On the other hand, the inability to read " presbyopia " occurs at a younger age. Eyeglasses or contact lenses for correction of farsightedness usually have collective lenses with a correction value in "plus" (convex) diopter, for example +3.5 dptr.

Hyperopia (farsightedness) without astigmatism means that the near vision is blurred and distance vision may or may not be blurred depending on the patient's age and other factors. This is because light is focused behind the retina secondary to one or more of the following reasons:
  • The eye may be too small so that light is focused behind the retina.
  • The cornea (clear window of the eye) is too flat, thus not focusing the light far enough forward so the focus is behind the retina.
  • The lens in the from of the eye is too weak thus not focusing the light far enough forward so that the focus is behind the retina. In hyperopia, vision can be corrected by altering the curve on the front of the eye to make the curve steeper. However, this is somewhat more difficult to do than making the curve flatter as in myopia.     What is a Farsightedness?

    In the normal eye, light bends through the cornea and lens to focus on the retina. With Farsightedness (place cursor over image) the eye is shorter (or cornea is flatter) causing the focal point of light to be behind the retina. The image is therefore out of focus on the retina and vision is blurred.
    The Treatment
    1. A very thin flap of cornea is created and folded back. 2. Excimer laser is applied to reshape (ablate) the cornea. 3. The cornea flap is replaced over the treated cornea. 4. The "steeper" cornea now bends to become focused light on the retina.
    The Result

It is caused by a defect of refraction in which the image is focused behind the retina of the eye rather than upon it, either because the eyeball is too short or because the refractive power of the lens is too weak. Presbyopia, a similarly faulty vision, is attributable to physiological changes in the lens brought on by age. Corrective eyeglasses with convex lenses compensate for the refractive errors.

People with this condition have trouble seeing things that are close. This condition is the most common reason for reading glasses or bifocals. If this is the only vision problem an affected person has, he or she will be able to see far away objects without difficulty. In young people, the focusing power of the eye is strong. Many younger people with mild cases of this condition can overcome the problem by intentionally focusing harder. As people get older, the focusing power of the eye decreases as a natural part of aging. This is why most farsighted people need bifocals or reading glasses around age 40 to 50. What are the signs and symptoms of the condition? The main symptom is a decreased ability to see things that are close to the eyes. Some people experience headaches, especially after reading. What are the causes and risks of the condition? This condition occurs when light rays do not focus well on the back of the eye, which is where the brain detects images. This most commonly occurs in older people because the focusing power of the eye decreases. It can occur in younger people if the eye is too short for their focusing power.

Farsightedness, or hyperopia, as it is medically termed, is a vision condition in which distant objects are usually seen clearly. But close ones are not brought into proper focus. Why does farsightedness occur? If the length of your eyeball is too short or the cornea has too little curvature, near objects cannot be brought into a sharp and clearly focused image. Some theorists believe that farsightedness is hereditary and others believe that it may result from environmental factors. How does farsightedness affect vision? If you are farsighted, you involuntarily exert extra effort to maintain clear distance vision and even greater effort to see clearly at close range. This extra effort can cause fatigue, tension and discomfort. If the crystalline lens of the eye cannot bring the object into focus, blurred vision occurs. How common is farsightedness? Many people have some degree of far sightedness. The condition is only a problem if it significantly affects a person's ability to see. It is estimated that over half the people who wear glasses are wearing them because of a focusing problem due to farsightedness or presbyopia, a natural decrease in focusing ability.

    FARSIGHTEDNESS Farsightedness is a condition in which you may have no problem seeing objects in the distance, but may have difficulty focusing clearly on near objects. On the other hand, you can be farsighted and see everything clearly, but be straining your eyes unnecessarily. Farsightedness is caused by an eyeball that is too short, causing light rays to focus behind the retina, instead of directly on the retina, which is necessary for normal vision. The focusing apparatus of the eye must work continuously to compensate for the problem and focus images. This form of farsightedness is called hyperopia. Its symptoms include eyestrain, blurred vision, or headache especially when reading or at the end of the day. The condition can become more symptomatic over time. Another form of farsightedness, called presbyopia, occurs as a normal result of aging. Common in people in their forties, presbyopia occurs when the lens inside the eye loses flexibility, preventing accurate focusing of near objects. People with this condition may notice eye fatigue when reading in poor lighting or at the end of the day, trouble changing focus from distance to near, or constantly repositioning reading material in an attempt to find the right focus. Reading glasses are available to make activities like reading and computer work easier and more comfortable.

    Stanford tests experimental treatment for farsightedness BY MITCH LESLIE An experimental method to correct farsightedness is being tested at Stanford in the nation's first clinical trial of the procedure. The fast, painless outpatient treatment uses radio-frequency energy to reshape the front of the eye. For patients treated thus far, the technique has provided immediate improvement in vision, said Edward Manche, MD, assistant professor of ophthamology and director of cornea and refractive surgery. Vision continued to sharpen in the weeks after the procedure as the eye settled into its new shape, added Manche, who is national medical monitor of a four-center FDA clinical trial of the effectiveness, safety, and stability of the technique. The procedure has been previously tested only in Italy and Mexico. Since starting Stanford's trial about six weeks ago, Manche has treated four patients, all of whom now have at least 20/25 vision, he said. Before the therapy, none had vision better than 20/40 the limit to drive legally without corrective lenses, Manche said. Manche presented preliminary data on these patients April 11 at the annual meeting of the American Society of Cataract and Refractive Surgery, held in Seattle, Washington.

    LASIK being tested as treatment for farsightedness BY RUTHANN RICHTER Ophthalmologists at Stanford have begun a clinical trial of a new laser treatment to correct vision in people who are naturally farsighted, with or without astigmatism, or who have become farsighted as a result of earlier eye surgeries. The trial is believed to be one of the first in the United States to test the value of the technique in patients with these vision problems, said Edward Manche, MD, assistant professor of ophthalmology and director of refractive surgery. The technique is called LASIK, or laser in-situ keratomileusis. "There were many people who had radial keratotomy [RK] and who are farsighted because of the unrecognized consequences of the progression toward farsightedness. Up until now, there was nothing we could offer them. This offers a solution to patients who have had RK in the past and are farsighted as a result," said Manche, the principal investigator in the new trial. Radial keratotomy is a once-popular procedure in which doctors reshaped the cornea through a series of tiny incisions. The technique faded from the scene with the advent, in 1985, of the excimer laser, now the tool of choice for refractive surgeons, Manche said. In the meantime, however, thousands of once-nearsighted patients developed farsighted vision as a result of overcorrection by RK, he said.

    New York, NY (August 3, 2000) A new procedure has been approved by the U.S. Food and Drug Administration (FDA) to correct farsightedness. Laser Thermal Keratoplasty (LTK) takes less than three seconds per eye and involves no cutting or removal of corneal eye tissue. Currently, the only site in the northeastern U.S. that offers LTK is the Weill Cornell Medical Center of NewYork-Presbyterian Hospital at its Laser Vision Correction Center. According to Sandra Belmont, MD, FACS, the Director of the Weill Cornell Laser Vision Correction Center, and one of the principal investigators for the FDA study, farsightedness, or hyperopia, is the most common refractive error among Americans. There are 60 million farsighted Americans over the age of 40 in the U.S. During the LTK procedure, two concentric rings of eight simultaneous spots of laser energy are applied to the mid-periphery of the cornea (outside of the optical zone). MONOVISION Hyperopia, or farsightedness, is the condition of the eye whereby the corneal curvature is too flat for the length of the eye, or the eye is too short for the corneal curvature (Figure 1). This results in light rays not yet coming to focus by the time they strike the retinal tissue in the back of the eye. A blurred, rather than sharp image of both distance and near objects is produced. Our VISX and Autonomous lasers are FDA approved for the treatment of hyperopia and astigmatism. From the patients perspective, the procedure for the treatment of farsightedness with or without astigmatism is essentially the same as treatment of nearsightedness with or without astigmatism. However, using the excimer laser, rather than flattening the central cornea as is done with the treatment of nearsightedness, the peripheral cornea is flattened allowing the central cornea to become relatively steeper (Figure 2). The relatively steeper central cornea adds refracting power to the surface of the cornea allowing the light rays to focus closer to the central retina. This improves the distance vision of farsighted individuals, and if monovision is performed in one eye, may also help improve the reading vision in the monovision eye. Because regression back towards hyperopia is slightly more common in the treatment of farsightedness than in nearsightedness, we purposely overcorrect patients in the early post-operative period. This means that we actually induce a small amount of myopia (nearsightedness) in both eyes during the early post-operative period. Therefore, the patient will be slightly nearsighted in one or both eyes for a few months after the laser surgery for farsightedness. Glasses for good distance vision may be required in the early post-operative period. Although this induced nearsightedness may be permanent and require surgical correction, in most cases, the early post-operative nearsightedness is temporary.

    Hopkins Offers Non-Laser Correction Farsightedness
    Surgeons at Johns Hopkins' Wilmer Eye Institute are now offering conductive keratoplasty, or CK, to correct low-level farsightedness in selected patients over age 40. The procedure, approved in April by the U.S. Food and Drug Administration, is the first non-laser treatment for hyperopia, a condition in which people can see objects far away but have trouble focusing on those nearby. It is an outpatient surgery performed under local anesthesia in just a few minutes . Unlike laser treatments, which use light waves as an energy source, CK uses radiofrequency waves, a form of electromagnetic energy, to re-shape the peripheral cornea. The energy is similar in some respects to the microwaves that power CB radios and cell phones. CK employs a pen-shaped instrument with a tip as thin as a human hair that releases the radiofrequency energy. The tip is applied in a circular pattern on the outer layer of the front of the eyeball to shrink small areas of tissue. The result is a constrictive band of tissue, similar to a tightened belt, that increases the overall curvature of the cornea.
