Considerations
Pectus excavatum is a congenital abnormality that can be mild or fairly marked. The child typically has a depression in the center of the chest over the sternum, and this may appear quite deep. It is caused by excessive growth of the costosternal cartilage (the connective tissue joining the ribs to the breastbone), with resulting inward deformity of the sternum.
If pectus excavatum is severe, it may affect the underlying organs (heart and lungs) leading to exercise intolerance or other problems. In addition, the cosmetic appearance of a pectus excavatum may pose a psychological difficulty.
Pectus excavatum may occur as the only abnormality, or in association with other syndromes.

Answer The medical term for this is pectus excavatum and the lay term is funnel chest. This is usually an isolated abnormality of the skeleton, but can be associated with certain connective tissue diseases such as Marfan's syndrome . Rarely, it is associated with rickets. There is an occasional link between poor exercise tolerance and funnel chest, but this is usually because people with this condition are led to believe that they shouldn't exercise. In many people with the condition, the heart is shifted to the left, and there may be some problems with the functioning of the heart.

However, in all techniques, correction of the excavation region of the sternum and prevention of flail bony thorax after fixation of the separated sternum and costal cartilage(plastron) are considered important issues. In particular, unstable plastron causes flail chest, and is considered the most important factor in postoperative respiratory management. In this case, we report 1 surgical case in which plastron was more firmly stabilized by overlapping and conjugating the sternum and costal cartilage(Ravitch's fixation method ) for fixation of the plastron, resulting in a good course without postoperative flail chest.

    What is pectus excavatum (sunken or funnel chest)? One of every 1,000 children has a chest disorder known as the pectus excavatum. This congenital deformity is characterized by a concavity of the sternum. It is often referred to as sunken or funnel chest and may appear as though someone has punched in the chest. What are the symptoms?
    The inward-facing sternum can apply pressure to the chest organs, which can cause shortness of breath and restricted growth of heart and lungs. Mildly present at birth, Pectus Excavatum usually becomes more serious through childhood. It can magnify considerably through the teenage growth years. Other problems, which may occur with pectus excavatum include chest pain, floppy heart valve, mitral valve prolapse, rapid heartbeat palpitations, and asthma-like symptoms and respiratory disease.     Even with severe deformities, the heart and lungs have normal growth and development. However it is documented that pectus deformities affect heart and lung function (Haller and Loughlin, 2000, Fonkalsord and Bustorff-Silva, 1999). In pectus excavatum, it is believed that the heart is displaced to the left side of the chest, and there is a restriction of movement of the heart and lungs. Patients complain of a decrease in stamina and endurance during strenuous exercise (67%), frequent respiratory infections (32%), chest pain (8%) and asthma (7%) (Fonkalsord EW et al, 2000). However recognition of these symptoms remains controversial amongst physicians as the evidence that exists does not reliably and consistently demonstrate physiological limitations. Whilst the physiological effects of pectus deformities remains debatable, the psychological difficulties facing patients are evident and yet there is sparse published information regarding these. Some patients are able to accept and live happily with the shape of their chest; this is an important point as health care professionals frequently assume a person with a physical disfigurement has a negative image of himself (Anderson, 1982). However many patients with pectus deformities are affected with a negative self-image and low self-confidence. 'An unwillingness to be seen without a shirt while swimming and participating in sports or social activities,' is the most frequently quoted complaint. In fact living with a pectus deformity affects all areas of life (Table 1).

    Definition
    Pectus excavatum is a chest wall deformity in which the central portion of the chest, or sternum, is pushed toward the spine.
    How often is it seen?
    Funnel chest is the most common deformity of the chest and is seen 3 to 10 times more often in boys than girls. It is usually diagnosed in the first year of life and rarely improves without therapy.
    Why is it a problem?
    There are two orders of suffering involved associated to PE: the physiological and the social.
    Physiological or medical problems:
    The sternum or breastbone is so far displaced in severe PE as to push the heart to one side or compress it while the lungs are compressed in a smaller chest cavity.
    Social problems:
    These affect each individual differently, in severe cases the deformity will significantly alter the child's life. Self-esteem issues are very prominent and you may notice that as they come closer to adolescence many activities will be abandoned if they involve removing their tops. What is the therapy? Mild PE can be improved with exercise and posture programs but severe cases require surgery for correction.

Description While the child is deep asleep and pain-free (using general anesthesia), an incision is made over the breastbone (sternum). The deformed cartilages are removed and the rib lining is left in place to allow regrowth of the cartilages. An incision is made in the sternum and it is repositioned. A rib or metal strut may be used to stabilize the sternum in normal position until healing occurs in 3 to 6 months. A temporary chest tube may be placed to reexpand the lung if the lining of the lung is entered.
    Metal struts are removed 6 months later through a small skin incision under the arm. This procedures is usually done as an outpatient. Most repairs are done between 18 months and 5 years of age. Indications Repair of pectus excavatum may be recommended for:
    • improved appearance (cosmetic repair)
