The gallbladder is a small pouch near the liver where bile is stored. Bile helps to break down food, especially fats. The gallbladder can become irritated and swollen. A common cause of gallbladder disease is gallstones. Pain from gallstones is also called ""biliary (BILL-ee-air-ee) colic.'' Many people have gallstones but do not know it. Often, gallstones stay in the gallbladder or pass through it without causing problems. However, if one gets stuck on its way out of the gallbladder, intense pain can result.
Causes
You are more likely to have gallstones if your parents had them. Your chances of developing gallstones also increase if you weigh too much, have many children, take birth control pills, drink too much alcohol, and have a high-fat diet. Whenever the gallbladder cannot completely empty, gallstones may develop. For instance, infection and swelling of the tubes that carry bile out of the liver can produce gallstones.

What are gallstones?
Gallstones are cholesterol crystals or pigment material that form in the gall bladder.
What is the gall bladder and what does it do?
The gall bladder is a pouch that sits beside the liver and stores bile, which is a greeny-yellow fluid produced by the liver. During digestion, the gall bladder releases bile into the small intestine where it helps in dissolving fats.
How are gallstones formed?
Some bile components (such as cholesterol) are not very soluble. When there is too much of these bile components, they separate and form a solid. This process causes the formation of gallstones - also known as cholelithiasis.
Are all gallstones the same?
No. There are different types of gallstones, depending on what component of the bile has solidified. Also, the stones can vary in size ranging from tiny, sand like particles less that one millimetre in diameter to more than four centimetres in diameter. Almost 90 per cent of gallstones are composed of cholesterol; the remainder consist of pigment material (bilirubin). The reason for the formation of pigment stones is not yet fully understood; however, some people with blood disorders such as homolysis (eg. sickle cell anemia) are at risk for developing pigment stones since the excessive destruction of red blood cell produces bilirubin, a bile pigment.

The liver produces a nasty substance called bile that helps metabolize fatty foods. This substance passes through a duct into the gallbladder which in turn forces the contents into the intestines through a spasmodic action. When there is interference with this process, usually due to congestion and/or blockage, there is often acute pain on the right side of the body, under the ribs, burning sensations as some of the bile moves up the esophagus, and often queasiness and nausea. This condition is called "gallbladder disease" but unlike some diseases, there is really no outside organism causing the problem. However, doctors are taught that this disease is incurable and the solution is to remove the gallbladder. There are many modern methods for treating this condition, including laproscopic surgery and even some drugs.

Bile and the Gallbladder
The formation of gallstones is a complex procedure that starts with bile , a fluid composed mostly of water, bile salts, lecithin (a fat known as a phopholipid), and cholesterol. (Most gallstones are formed from cholesterol.)
  • Bile is first produced by the liver and then secreted through tiny channels that eventually lead into a larger tube called the common bile duct , which leads to the small intestine.
  • Only a small amount of bile drains directly into the small intestine, however. Most flows into the gallbladder through the cystic duct , which is a side extension off the common duct. (The system of ducts through which bile flows, including the common bile duct is called the biliary tree).
  • The gallbladder is a four-inch sac with a muscular wall that is located under the liver. Here, most of the fluid (about two to five cups a day) is removed, leaving a few tablespoons of concentrated bile.
  • Bile is important for the digestion of fat. The gallbladder serves as a reservoir until bile is needed in the small intestine for this function.

Dietary Advice There is little evidence to support the use of a low fat diet in the management of existing gallstones, except when bile ducts are obstructed (Madden 1992). Such patients may require nutritional support until the condition can be treated by surgery or other means.

Gallstones form in the gallbladder, an organ located next to the liver whose purpose is to store bile, a digestive fluid produced by the liver to aid in the digestion of fat. Some components of fat, like cholesterol, do not respond well to bile and rather than dissolve as they should, they solidify and begin to form crystals. These crystals are gallstones. About 90% of all gallstones are formed this way.
There are also some gallstones made of bilirubin, and the reason that these form is not certain. However, those with certain blood diseases are at higher risk for this type of stone.
The risk of developing gallstones increases with age. Women who have had multiple pregnancies are at a higher risk, as are obese people or those who lose a great deal of weight quickly.
The largest symptom of gallstones is severe abdominal pain. However, four out of five people with gallstones experience no symptoms. These people are said to suffer from "silent gallstones." Symptomatic gallstones can cause complications such as inflammation of related organs like the liver or pancreas.

About three-quarters of the gallstones found in the U.S. population are formed from cholesterol. Cholesterol makes up only five percent of bile; it is not very soluble, however, so in order to remain suspended in fluid, it must be properly balanced with bile salts. If the liver secretes too much cholesterol into the bile, if the bile becomes stagnant because of a defect in the mechanisms that cause the gallbladder to empty, or if other factors are present, supersaturation can occur. Cholesterol may then precipitate out of the bile solution to form gallstones a condition known as cholelithiasis. The process is very slow and most often painless. Gallstones can range from a few millimeters to several centimeters in diameter. The other 25% of gallstones are known as pigment gallstones. They are composed of calcium bilirubinate, or calcified bilirubin, the substance formed by the breakdown of hemoglobin in the blood. These black stones often form in the gallbladders of people with sickle cell anemia, hemolytic anemia, or cirrhosis.

What are gallstones?
Gallstones (cholelithiasis) are stones that form in the gallbladder. The gallbladder is a pear-shaped organ that lies underneath the liver on your upper right abdomen. The gallbladder stores bile, a substance that helps digest fat and also carries away waste products from the liver. Bile is produced by the liver and contains many compounds including cholesterol. Most gallstones are made of cholesterol, but other compounds can also cause gallstones. Usually acids in bile keep cholesterol from forming into stones. If the amount of cholesterol in the bile increases beyond the ability of acids to maintain the cholesterol in suspension, the cholesterol crystallizes and forms a stone. You thus have a stone in the gallbladder or "gallstones." If you are overweight, diabetic, have high blood levels of cholesterol and/or fats (triglycerides), you increase your risk of developing gallstones.

Definition: The presence of gallstones in the gallbladder without any associated symptoms.

Causes, incidence, and risk factors: Cholelithiasis is usually incidentally discovered by routine X-ray study, surgery, or autopsy. Virtually all gallstones are formed within the gallbladder, an organ that normally functions to store bile excreted from the liver. Bile is a solution composed of water, bile salts, lecithin, cholesterol and some other small solutes. Changes in the relative concentration of these components may cause precipitation from solution and formation of a nidus, or nest, around which gallstones are formed. While these stones may be as small as a grain of sand, they may become as large as an inch in diameter depending on how much time has elapsed from their initial formation. In addition, depending on the main substance that initiated their formation (e.g., cholesterol), they may be yellow or otherwise pigmented in color. Cholelithiasis is a common health problem worldwide with an approximate incidence of 1 out of 1,000 people. The prevalence is greater in women, Native Americans, and people over the age of 40.
