Gonorrhea is a common infection of the genitourinary tract (specifically the urethra and cervix) caused by the bacteria, Nisseria gonorrhoeae . Causes, incidence, and risk factors: Gonorrhea is one of the most common infectious bacterial diseases and is most frequently transmitted during sexual activity, including vaginal intercourse and both oral and anal sex. Gonorrhea is a reportable disease and all state governments require that cases of diagnosed gonorrhea be reported to the health authorities (State Board of Health). This allows for adequate follow-up and testing of sexual contacts.
There are almost 400,000 cases of gonorrhea reported to the CDC (Centers for Disease Control and Prevention) each year in the United States. There are probably many cases that are treated but not reported. The total number of cases is thought to approach 1,000,000; if unreported cases are included, some estimates reach 2,000,000. A rate of 400,000 cases per year means that 1 out of every 687 Americans has gonorrhea. A rate of 2 million means that 1 in every 130 Americans are infected.
Gonorrhea is more common in large metropolitan areas, inner-city areas, populations with lower overall levels of education and people with lower socioeconomic status. Gonorrhea is most prevalent in people 15 to 29 years old. Risk factors include having multiple sexual partners, having a partner with a past history of any STD, and having unprotected sex (sex without the use of a condom).

Gonorrhea, also known as "the clap," is most common in sexually active people between the ages of 20-24, according to San Diego State University Student Health Services.
The good news is, according to the Center for Disease Control, cases of gonorrhea have been on the decline. From 1994 to 1995, the rate decreased from 165.1 cases per 100,000 to 149.5.
According to Sean Kaufman, a health educator for SHS, there was a time when gonorrhea was fairly common, but it is much less common now.
"HPV and chlamydia are more common now," he said.
Gonorrhea is most commonly transmitted by unprotected vaginal, anal and oral sex. Because it is a bacteria, it grows best in warm, moist areas.
Katie (not her real name) was diagnosed with gonorrhea after having unprotected sex with her current partner one time. She said she didn't have any symptoms, but she had heard her partner was infected.
Katie was tested at SHS, and the gonorrhea test was positive.

  • Bacteria transmitted during sex cause gonorrhea.
  • Women often have no symptoms at all or very mild ones. If symptoms appear, they may include burning during urination (peeing) and/or a vaginal discharge.
  • Men may have a yellow discharge from the urethra, opening in the penis. It may also burn or hurt to urinate (pee). Symptoms vary from person to person from being mild to severe. Men also may not have any symptoms.
  • A gonorrhea infection during pregnancy can cause problems such as stillbirth or blindness for the baby. If a mother is infected, she can pass gonorrhea on to her baby during delivery.
  • Antibiotics or penicillin can easily cure gonorrhea if treated early. If it is left untreated in women, gonorrhea (like chlamydia) can progress into a more serious illness called pelvic inflammatory disease (PID) and can keep women from being able to have children in the future. If it is left untreated in men, it can make them sterile (unable to have children).
  • Tell any sex partners right away if you find out you have gonorrhea. They should also be treated even if they have no symptoms.

The spectrum of disease is far greater than simple "clap." Overall, up to 10 percent of men may have asymptomatic infections. Women seen in family practice and VD clinics are typically asymptomatic, but up to 90 percent of women hospitalized for gonorrhea have symptoms. Urethritis is the most common gonococcal disease. It occurs after two to eight days incubation, although occasionally as long as 18 days are required. Most cases have abrupt onset, severe dysuria, and copious purulent discharge, but mild cases occur and may mimic nongonococcal urethritis (NGU). Gram stain is positive in 85 percent of cases in men, and the presence of gram negative diplococci (GNDC) is essentially diagnostic. (The old requirement, that the GNDC be intracellular, has little validity and should be discarded). Ideally, four hours should have elapsed since the patient last urinated. Advantages of pretreatment cultures:
1. No need to obtain a repeat specimen (for culture) if the Gram stain is nondiagnostic.

What is gonorrhea?
Gonorrhea, also known as the clap or g.c., is one of the most common sexually transmitted diseases . The disease can affect all parts of the body, although it usually shows up first in the genital areas. Although the disease is completely curable, it's important to seek treatment early, because gonorrhea can cause serious consequences such as infertility and arthritis. It's possible that the gonococcal bacteria may spread to the heart and brain, and in rare cases, cause death. Unfortunately, the disease can easily go undetected until it progresses and causes painful complications. What causes gonorrhea? Gonorrhea is caused by bacteria and is highly contagious. The bacteria can enter the body through any body opening (mouth, vagina, rectum). Gonorrhea is most frequently transmitted through sexual intercourse. In men, the infection usually starts as an infection of the urethra (the tube that urine passes through). In women, the bacteria usually first infect the cervix (opening of the uterus). The bacteria can infect the throat and rectum after oral and anal sex. A baby can be infected during childbirth if the mother has gonorrhea. When the baby passes through the birth canal, the bacteria can get into and infect the baby's eyes.
