Heart murmurs and other sounds
Overview Treatment Definition: Murmurs are blowing, whooshing, or rasping sounds produced by turbulent blood flow in or near the heart. Often, they are caused by turbulent blood flow through the heart valves.
Alternative Names: Chest sounds - murmurs; Heart sounds - abnormal
Considerations: Heart sounds are evaluated by listening with a stethoscope ( auscultation ). They can be further evaluated with an echocardiogram to see what the exact cause of the murmur is.
The heart has four chambers two upper chambers (called atria) and two lower chambers (ventricles). The heart has valves that temporarily close to permit blood flow in only one direction. The valves are located between the atria and ventricles, and between the ventricles and the major arteries from the heart.
Normal heart sounds are called S1 and S2. They are the "lubb-dupp" sounds that are thought of as the heartbeat. These sounds are produced when the heart valves close.

... Women at Risk Understanding Heart Murmurs How Serious Are They? Simply stated, a heart murmur is an abnormal sound in the heart beat. Generally, during a routine office visit, the doctor listens to your heartbeat using a stethoscope an can detect a murmur if present. In most cases, heart murmurs do not affect your health and result in some form of irregular blood flow through the heart. Some, however, are due to underlying defects in the heart or its valves and may require treatment. What are the Underlying Defects? Two common defects that can cause a heart murmur are valvular defects and septal defects. Valvular defects refer to abnormalities of the valves that keep blood from flowing "backwards" into the heart chambers. A defective valve may allow blood to back up in the wrong direction or it may block the passage of blood, thereby interrupting the normal flow. Septal defects are small breaks in the partition that separates the left and right sides of the heart (septum). These breaks can allow blood to flow irregularly between the two sides. Septal defects are more commonly found in children.

Heart murmurs: Which are innocent?
Many children have a heart murmur, especially during febrile illness. Most of these are "innocent", or "functional" murmurs, but some are caused by organic heart defects that are asymptomatic. When a heart murmur is detected during routine physical exam, a careful review of the child's appearance and history often provides clues about the murmur's significance. The classical innocent murmur is the vibratory systolic murmur referred to as Still's murmur. It is soft and of low frequency with a somewhat musical quality, like plucking a string or rubber band. This murmur is of short duration and occurs between the two heart sounds (mid-systole) and is often referred to as "physiologic", or "not significant." Other innocent murmurs are soft, quiet and "blowing" in character. Innocent murmurs often come and go. Also they may disappear if the child sits up or lies down. Once a murmur is discovered, (provided it's not a classical Still's murmur), observation of the child is important, along with noting vital signs. Is the color of mucous membranes and nailbeds pink or dusky (cyanotic)? Is exercise intolerance present? By history, have there been syncopal episodes or palpitations? All these findings suggest an organic murmur.

What are the different types of murmurs? Your child's physician will evaluate a murmur based on several factors. Murmurs are analyzed for pitch, loudness, and duration. A new hand-held computer may detect murmurs as accurately as more expensive tests. Four-month-old Jacob is getting a high-tech check-up of his heart. "We're looking for the heart to speak to us," said Curt DeGroff, M.D. Doctor Curt Degroff uses an artificial neural network or "Ann" to diagnose potentially dangerous heart murmurs in children. It's a computer program that can recognize complex sound patterns. Doctor Degroff teamed up with Roop Mahajan, a mechanical engineer, to develop this device.

What is an innocent heart murmur?
Heart murmurs are extra noises heard with each heartbeat. Some murmurs occur even though the heart and blood vessels are completely normal. These murmurs are called innocent murmurs. There are several different types of innocent murmurs that occur in childhood. Some occur when the child is very young, and others are heard later in childhood.
What causes innocent heart murmurs?
The heartbeat has two distinct sounds that you can hear with a stethoscope. These two thumping sounds are the sounds of the heart valves closing. A heart murmur is heard in between these two normal valve sounds. The murmur sound is caused by blood flowing turbulently through the heart, like the sound of a stream flowing into a whitewater pool. A murmur can occur in several different parts of the heart and blood vessels even though the heart is completely normal.
Are innocent murmurs common?
Innocent murmurs occur in about half of all children at some time during childhood.

Heart murmurs can generally be divided into two categories:
  • Murmurs that are not due to structural problems in the heart also called "innocent murmurs" or "functional murmurs."
  • Murmurs which are due to a structural problem of the heart (such as a hole in the heart, a narrowed heart valve or a leaky heart valve).
