HEART MURMURS What causes heart murmurs? Heart murmurs can be caused by defective heart valves or holes in the heart walls. A stenotic heart valve can't open completely, so blood is ejected through a smaller-than-normal opening. A valve may also be unable to close completely, leading to regurgitation of blood back through the valve when it should be closed - such as aortic or mitral regurgitation. Murmurs also can be caused by conditions such as pregnancy, fever, thyrotoxicosis (a diseased condition resulting from an overactive thyroid gland) or anemia. A diastolic murmur occurs during heart muscle relaxation between beats. A systolic murmur occurs during heart muscle contraction. Systolic murmurs are graded by intensity (loudness) from one to six. For example, a grade 1/6 is very faint, heard only with a special effort. A grade 6/6 is extremely loud and heard with stethoscope slightly removed from the chest.

In fact, they're so common that in any group of children, a large percentage is likely to have had one at some time. Innocent murmurs also may disappear and then reappear. Most innocent murmurs disappear when a child reaches adulthood, but some adults still have them. When a child's heart rate changes, such as during excitement or fear, the innocent murmurs may become louder or softer. This still doesn't mean that the murmur is abnormal. Sometimes, when a doctor first hears the murmur through a stethoscope, he or she may want to have other tests done to be sure the murmur is innocent. After that, there's no need for a cardiac reevaluation unless the patient or doctor has further questions. In addition, the child doesn't need medication, won't have cardiac symptoms, and doesn't have a heart problem or heart disease. A parent doesn't need to pamper the child or restrict his or her diet or activities. The child can be as active as any other normal, healthy child.

It does not necessarily mean there is an abnormality, just that it is detected. (Similar to hearing water going through a pipe – you hear it – and just because you hear it does not mean it is abnormal). The term "murmur" should not be confused with the term "abnormal murmur" just as the word "temperature" should not be confused with the term "fever". However, people mistakenly use both terms interchangeably. Murmurs are not always significant. – they may be normal or innocent . Innocent murmurs are usually found incidentally during a physical exam. They require no further evaluation. Pathologic murmurs are due to abnormal blood flow, usually as a result of a cardiac abnormality. These ought to be referred to a pediatric cardiologist (even prior to testing to avoid duplication and the improper performance of tests). How does it occur?

Bush, Dick Cheney has already spent a lot more time in the Oval Office than the man he would like to serve as Vice President. If the younger Bush eventually enters the White House, Cheney promises to be an even more influential Vice President than, well, Al Gore. As head of the informal transition team, Cheney has been making regular trips to Bush's ranch outside Austin, Texas, to start shaping a potential Cabinet. In the excitement about the vote in Florida, he and James Baker are George W.'s chief strategists. It was Cheney who brought in Baker in the first place, and the two men hold a daily conferencef call with Bush that is George W.'s first order of business each morning. Bush aides say Cheney is far more involved than Bush with both the transition and the postelection recount struggles. Protocol for patients found to have a murmur prior to elective surgery
The vast majority of children found to have a murmur pre-operatively have an innocent murmur. These innocent murmurs are characterised by being soft, early systolic murmurs with no thrill or abnormal cardiac impulses and are not associated with cardiac signs or symptoms. If a child is found to have a murmur prior to elective surgery a history and cardiac examination should be performed and an ECG done.
  • In the presence of a murmur an echocardiogram and cardiac opinion
    is indicated prior to surgery if:
  • The child is younger than a year The murmur fits pathological criteria There are cardiac signs or symptoms Evidence of LVH or RVH (see following tables) Criteria for pathological murmurs
  • All diastolic murmurs All pansystolic Late systolic Very loud murmurs Continuous murmurs (other than a venous hum) Associated with cardiac signs or symptoms

    July 15, 2002 - Zargis Medical Tests New Technology, Developed At Siemens, to Improve Diagnosis of Heart Murmurs; Clinical Studies Under Way At NYC's St. Francis Hospital Zargis Medical Corporation Corporation announced today it has begun testing its alpha system, ZACsmtm (Zargis Acoustic Cardiac) solution, through the recording of clinical data at St. Francis Hospital, The Heart Center in Roslyn, New York. St. Francis Hospital, New York's only specialty designated cardiac center, has the highest cardiac caseload in the Northeast and is one of the highest volume cardiac centers in the United States. Zargis Medical Corp. is developing decision support products and services for the screening and diagnosis of congenital and valvular heart disease. The technology behind ZAC was developed by Siemens Corporate Research over the last six years and is based on a proprietary algorithm and system that analyzes heart sounds and murmurs, using a digital stethoscope, and identifies potential heart abnormalities. This technology was subsequently transferred to Zargis Medical Corp. upon its formation in January 2001, when Siemens Corporate Research Inc. and Speedus Corp. announced their co-investment in Zargis to further develop and commercially exploit this opportunity in the medical arena. Speedus, in addition to helping Zargis in raising capital, is under contract to design and develop wireless applications, as well as provide transaction processing to support the commercial rollout of Zargis' cardiac diagnostic products. Speedus has demonstrated, using a combination of wireless and wired technology, the ability to transfer heart sound data from a physician's office to the Data Center which will enable Zargis Medical to pursue a business model based on a fee per usage basis.
