Heartburn is a discomfort or pain caused by the stomach contents regurgitating from the stomach up into the gullet (oesophagus). The oesophagus is not made to withstand acid and is thus irritated and inflamed when acid from the stomach refluxes into it. This is known as reflux oesophagitis.
Symptoms
The pain caused by the irritation of the oesophagus is a burning pain which one feels in the centre of the chest. There is often an associated awareness of acid and stomach contents in the back of the throat or mouth. Lying down or bending (eg in the garden) tend to increase the symptoms.
Causes
The whole of the intestinal tract is in the form of a tube with muscles in the wall which move the food along (peristalsis). At the bottom of the oesophagus the muscles encircling the tube work rather like a valve to stop food going backwards. This is not a perfect valve, but in some people works less well than others:
  • Obesity leads to a higher pressure in the abdomen.
  • Pregnancy causes a higher pressure in the abdomen and there is also evidence that the changing hormone levels cause relaxation of the muscle valve.

More than a third of Americans suffer from heartburn at least once a month. Around the holidays, they suffer more often. "Kind of like a fullness you can't breathe and you feel a burning sensation," said Amy Wison, who comes from a family of heartburn sufferers. They all say it gets worse around the holidays, when there's so much food around. Meet brother Andy. "It's not like an upset stomach, but it's a burning sensation in the chest." That burning sensation is caused by food going back up the swallowing tube, or esophagus. "It may be a genetic predisposition, but certain foods can make it worse," explained Dr. Stephen Brunton of the Long Beach Memorial Medical Center. "Things like spicy foods, onions, chocolate, peppermint or just eating a lot will prevent the stomach from emptying and will allow the food to back up into the esophagus." So what's a heart-burn sufferer to do? Here are a few tips from the experts:

Heartburn is the most common symptom of a condition called gastroesophageal reflux or acid reflux. A sphincter (specialized muscle), known as the lower esophageal sphincter (LES), is located at the end of the esophagus and opens during swallowing to allow food to pass into the stomach. The LES muscle then closes quickly to prevent the return (reflux) of food and stomach juices back into the esophagus. However, the LES muscle does not always work perfectly. Gastroesophageal reflux occurs when the LES muscle either relaxes inappropriately or is weak. This allows stomach juices to back up, or reflux, into the esophagus, creating heartburn. When the acid contents from the stomach regularly back up into the esophagus, a chronic condition called gastroesophageal reflux disease, or GERD, occurs. Heartburn is sometimes called acid indigestion and usually occurs after meals. In addition to heartburn, symptoms of acid reflux may include persistent sore throat, hoarseness, chronic cough, asthma, heart-like chest pain and a feeling of a lump in the throat.

In the United States, almost 50% of the population has heartburn at least once a month; 7% have it daily. Heartburn is even more common during pregnancy, with about 1 in 4 women reporting it daily at some point during their pregnancy. Statistics on acid indigestion are difficult to find because the definition is vague, but it's probably almost as common as heartburn. Heartburn is a burning feeling in the middle of your chest caused by acid leaking upwards from the stomach into the esophagus, or swallowing tube. Doctors call this reflux esophagitis. Normally, the muscular esophagus acts like a one-way valve allowing food to enter your stomach after swallowing, but not letting anything go back up. When your esophagus is too loose where it connects to the stomach, strong stomach acid seeps back through the opening and causes heartburn. Because your esophagus doesn't have a protective lining like the stomach, it can be burned by the acid causing pain and sometimes damage. Heartburn can also cause night time cough, wheezing, and in some cases difficulty swallowing food due to scarring of the esophagus. Acid indigestion is a similar burning discomfort, but in the pit of your stomach. It can be caused by acid irritating the stomach lining or duodenum. Acid indigestion is believed to happen not just because of too much acid, but also because of too little protection from the mucus lining the stomach. Acid indigestion can burn so deeply into your stomach or duodenum that a canker sore-like crater forms (peptic ulcer disease).

Each time you swallow food, it travels down the esophagus to the stomach where it is mixed with stomach acid and other powerful digestive enzymes. The stomach is lined with cells that secrete mucus to protect it from this potentially destructive cocktail. A muscular ring at the base of the esophagus, called the lower esophageal sphincter (LES), relaxes to allow food to pass into the stomach, and then tightens to prevent backflow or reflux of acidic stomach contents into the more vulnerable esophagus. Unfortunately, a number of factors can interfere with this protective mechanism. Reflux is common, and often occurs without causing any symptoms because the body responds by quickly neutralizing and clearing the acidic contents from the esophagus. In some individuals, however, the protective mechanisms are not adequate to meet the challenge for reasons that we don't understand well.     Posted by Amanda at 08:14 AM Comments (0) TrackBack (0) March 24, 2003 I had a pretty yucky weekend - I had a allergic reation to something I ate and started to break out in hives Friday. They were'nt actually that bad on Friday or Saturday but on Sunday it spread to my face and the ones on my stomach were pretty swollen. Bob drove me to the minor emergency clinic (he was worried it was chicken pox! ha ha.). The doctor gave me a shot of Benadryl and a pack of steroids to take. I am still pretty itchy but I think its getting better. My face looks ALOT better today and my ears are no longer purple.
    The only thing we can figure may have triggered the attack is the sourdough bread that we bought last week. I had some on both Thursday and Friday. The only other thing that I ate differently last week was a pear (maybe the pesticides?). Needless to say, I will avoiding both for a quite some time.
    Please send all thoughts of non-itchiness my way.

    Heartburn and Hiatus Hernia What is the Heartburn? Potentially dangerous confusion can arise when someone neglects their heart pain believing it to be heartburn. What causes Heartburn? Acid is present in the stomach to digest food. Heartburn occurs when small amounts of this acid rise up into the gullet (oesophagus) - the tube which carries food from the mouth to the stomach. This is called reflux. The gullet, unlike the stomach, does not have a protective lining. So when it is exposed to the acid, it can become inflamed and painful. Why doesn't everyone get Heartburn? Some known factors that can lead to heartburn include:
    pregnancy
    smoking
    eating large meals, especially near bedtime
    being overweight
    bending a lot
    wearing tight clothing around the waist. In most of these cases, it is the increased pressure which overcomes the normal muscle contraction and causes heartburn. Smoking, on the other hand, relaxes the muscle around the gullet, with the same result. What is Hiatus Hernia?
