Plantar fasciitis refers to the symptoms of pain that people with this condition experience; a heel spur is a spur of bone that can form on the heel bone (calcaneus) as a result of this condition. About 50% of patients with plantar fasciitis have an actual bony spur. Plantar fasciitis is most often seen in middle-aged men, but can be found in all age groups. The condition is diagnosed with the classic symptoms of pain well localized over an area of the bottom of the foot near the heel. Often the pain is most severe when you first stand on the foot in the morning. Pain often subsides quite quickly, but then returns after prolonged standing or walking. The condition is sometimes, but not always, associated with a sudden gain of weight. Plantar fasciitis can be confused with a condition called

Introduction
Plantar Fasciitis is a condition that is sometimes called a heel spur . This condition causes pain on the bottom of the heel when putting weight on the foot. There are probably many underlying causes of heel pain, and some physicians feel that it is probably more accurate to simply make a diagnosis of heel pain rather than try and define an absolute cause in every instance of heel pain.
Anatomy
The Plantar Fascia is a structure that runs from the front of the heel bone (calcaneus) to the ball of the foot. This dense strip of tissue helps to support the longitudinal arch of the foot, by acting similar to the string on a bow. As you can imagine, when the foot is on the ground a tremendous amount of force is concentrated on the plantar fascia. This can lead to stress on the plantar fascia where it attaches to the calcaneus. Small tears of the tendon can result and are repaired by the body.

ANATOMY
The plantar fascia is a thick, broad, inelastic band of fibrous tissue that courses along the bottom (plantar surface) of the foot. It is attached to the heel bone (calcaneus) and fans out to attach to the bottom of the metatarsal bones in the region of the ball of the foot. Because the normal foot has an arch, this tight band of tissue (plantar fascia) is at the base of the arch. In this position, the plantar fascia acts like a bowstring to maintain the arch of the foot.
Figure 1 : Lateral (Side ) View of Foot Plantar fasciitis refers to an inflammation of the plantar fascia. The inflammation in the tissue is the result of some type of injury to the plantar fascia. Typically, plantar fasciitis results from repeated trauma to the tissue where it attaches to the calcaneus. Figure 2 : Plantar (Bottom) View of Foot This repeated trauma often results in microscopic tearing of the plantar fascia at or near the point of attachment of the tissue to the calcaneus. The result of the damage and inflammation is pain.
If there is significant injury to the plantar fascia, the inflammatory reaction of the heel bone may produce spike-like projections of new bone called heel spurs. The spurs are not the cause of the initial pain of plantar fasciitis, they are the result of the problem. Most heel spurs are painless. Pain can result when these tissues become irritated or inflamed, or when small spurs grow on the heel bone. Inflammation Most cases of heel pain are characterized by inflammation. First, the fascia begins to pull on the bone and the tissues become irritated, then inflamed. Inflammation of the fascia is called fascitis.

Heel Spurs
Definition
The heel bone is the largest bone in the foot and absorbs the most amount of shock and pressure. A heel spur develops as an abnormal growth of the heel bone. Calcium deposits form when the plantar fascia pulls away from the heel area, causing a bony protrusion, or heel spur to develop. The plantar fascia is a broad band of fibrous tissue located along the bottom surface of the foot that runs from the heel to the forefoot. Heel spurs can cause extreme pain in the rearfoot, especially while standing or walking.
Cause
Heel spurs develop as an abnormal growth in the heel bone due to calcium deposits that form when the plantar fascia pulls away from the heel. This stretching of the plantar fascia is usually the result of over-pronation (flat feet), but people with unusually high arches (pes cavus) can also develop heel spurs. Women have a significantly higher incidence of heel spurs due to the types of footwear often worn on a regular basis.
Treatment and Prevention
The key for the proper treatment of heel spurs is determining what is causing the excessive stretching of the plantar fascia. When the cause is over-pronation (flat feet), an orthotic with rearfoot posting and longitudinal arch support is an effective device to reduce the over-pronation, and allow the condition to heal.

I have been a reader of your column for a long time and have always enjoyed it, but I am concerned with your most recent article addressing heel spurs on April 22. Being a physical therapist, I was very much surprised to see that you neglected to use physical therapy as an intervention for heel spurs. You mentioned exercising and stretching, but who provides this care? Having many friends who are orthopedic surgeons, I find it highly unlikely that they would have the time to sit with a patient and explain to them the mechanics of stretching. I also believe you missed out a large portion of treatment in terms of modalities like phonophoresis and iontophoresis. Phonophoresis drives hydrocortisone solution into the tissue using ultrasound, and iontophoresis uses electrical current to drive anti-inflammatory medication in. I mean no disrespect by this letter, but as a therapist who treats three to five of these patients per month with very good results, I was slightly offended at not having my profession mentioned in your article. Thank you for your consideration.
