Heel Spurs and Plantar Fasciitis
Heel spurs are the best known heel problems. A heel spur is a point of excess bone growth on the heel. The bone growth usually extends forward towards the toes. Heel spurs are visible on Xray. The most common heel problems are actually caused by a painful tearing of the plantar fascia connecting the toes and the heel. (Plantar refers to the bottom of the foot and fascia is a type of dense fibrous connective tissue.) This may result in either a heel spur or plantar fasciitis. If your foot flattens or becomes unstable during critical times in the walking or running cycle, the attachment of the plantar fascia into your heel bone may begin to stretch and pull away from the heel bone. This will result in pain and possibly swelling. The pain is especially noticeable when you push off with your toes while walking. Since this movement stretches the already inflamed portion of the fascia. Without treatment the pain will usually spread around the heel. The pain is usually centered at a location just in front of the heel toward the arch. When the tearing occurs at the bone itself, a the bone may attempt to heal itself by producing new bone. This results in the development of a heel spur. Without the spur the condition is called plantar fasciitis. The pain of this condition may cause you to try to walk on your toes, or alter your running stride and gait which will cause further damage and may cause a problem to develop in your healthy foot. Gait changes in running may also lead to ankle, knee, hip or back pain.

Plantar fasciitis
(heel spurs) is an excruciatingly painful foot condition caused by the inflammation of the plantar fascia , a broad ligament-like structure that runs along the sole (plantar) of the foot between the toes and the heel bone.
Most people who have plantar fasciitis know they have it and know what it is.
Some doctors compare the pain of heel spurs to the feeling of walking with a pebble in your shoe, just beneath the heel. Ha! Those doctors have never had plantar fasciitis . Those who have will tell you that the pain is far, far worse than that innocuous little analogy suggests. It is so bad, in fact, that it drives many people to submit to horrendously painful corticosteroid shots in their heels, in hope of reducing the pain.
I had a cortisone shot in my right heel the second time I had
plantar fasciitis
. I am not a screamer, but let me tell you, that shot caused me to let out a shriek of pain that was heard in the parking lot of the podiatrist's office. It was so painful that I was unable to force myself to undergo the shot in my left heel that was scheduled for two weeks later. Furthermore, though many people do get relief from such shots, for me the shot did no good at all.
Another fairly drastic option for treating plantar fasciitis is surgery to loosen the plantar fascia by making a small cut in it. Certainly this treatment is effective for many people, but few would choose either surgery or those devilish cortisone shots if they had other less invasive treatment options.

Of the 80 percent of Americans with foot problems at some time during their lives, more than half complain of heel pain. A heel spur, one of the most painful foot disorders, can be so disabling that even getting out of bed is an effort. High impact exercise, excess weight, arthritis - even standing on concrete for long periods - all can lead to a heel spur. With every step, the foot absorbs tremendous force. The heel strikes the ground with 2,000 pounds of force per 100 pounds of body weight. Most of the shock is absorbed by a muscle called the plantar fascia, an elastic-like band that stretches along the bottom of the foot and protects it from the stress of day-to-day activities.

    (Plantar Fascitis) Description Sharp pain, aching or stiffness on the bottom of one or both heels is a very common ailment. The pain is often at its worst upon awakening in the morning (or after sitting down for an extended period and then resuming activity), causing hobbling or limping for a few minutes before a comfortable stride can be resumed. As weight continues to be applied during walking or standing, mild or severe pain may persist. Adults whether office worker, laborer or athlete develop the problem most frequently, although children, too, can be affected if the growing bone becomes irritated. Causes of Heel Pain Heel pain originates deep within the foot, directly on the heel bone or within the foot's connective tissues, called thePlantar Fascia. Several layers of fatty tissue surround the heel bone, softening the impact or walking and running and protecting the bones and muscles of the foot. Beneath this padding, a fibrious band of connective tissue extends from the heel bone, supports the arch and reaches across to the toes. Pain can result when these tissues become irritated or inflamed, or when small spurs grow on the heel bone. Inflamation Most cases of heel pain are characterized by inflammation. First, the fascia begins to pull in the bone and the tissues become irritated, then inflamed. Inflammation of the fascia is called fascitis.

    Are high heels bad for your feet? What else do I need to consider when buying women's shoes? High heels may well be the rage in fashion magazines, but from a medical standpoint they are literally a walking health hazard. According to Edward Hurwitz, D.P.M., staff podiatrist at Boston Medical Center, almost nothing can be said in defense of high heels. "There isn't a high heel in existence that is good for the average woman, and the higher the heel, the worse the potential problems," he says. "Especially when a woman wears nylons, high-heeled shoes become a sliding board, shoving the foot to the front of the shoe and compressing the toes together. This leads to a host of problems that affect not only the toes, but the arch, the ankle, the knee, and possibly even higher body parts." A small lift in the heel a little less than half an inch is necessary for the foot to grab the ground well when walking. The toes are designed to act as small shock absorbers when the foot hits the ground. However, when women "ratchet up" on high heels, the toes become locked into place and can no longer absorb the force of each stride. One possible result is a condition known as hammer toes, where the toes are permanently bent in the middle joint, causing them to be shaped like claws. In addition, because the toes are pressed together, ingrown toenails are a major risk.

It is most often seen in middle-aged men, but can be found in all age groups. The condition is diagnosed with the classic symptoms of pain well localized over an area of the bottom of the foot near the heel. Often the pain is most severe when you first stand on the foot in the morning. The condition is sometimes, but not always, associated with a sudden gain of weight. The plantar fascia is a thick ligamentous connective tissue that runs from the heel bone to the ball of the foot. This strong and tight tissue contributes to maintaining the arch of the foot. It is also one of the major transmitters of weight across the foot as you walk or run. Therefore, the stress placed on the plantar fascia is tremendous, and when there is inflammation of the fascia it can be quite painful. When the plantar fascia becomes inflamed due to excessive stretching, too much pressure, or trauma, the condition of plantar fasciitis results.     Plantar fasciitis is the inflammation and possible tears of the plantar fascia ligament that attaches to the base of the heel bone on the bottom of the foot. The ligament extends to the metatarsal heads in the forefoot area. The plantar fascia ligament is very much like a guitar string when it is pulled tight.
    Plantar fasciitis not only creates pain in the heel region of the foot; it can cause arch pain. It can be triggered by a number of different situations.
    One of the most common situations that triggers plantar fasciitis is over-pronation within the foot. If the foot pronates, collapses, and elongates it actually pulls on this ligament and stretches it. It is this pulling motion and stretching motion that takes place from 8,000 to 12,000 steps a day as we walk that puts stress on the plantar fascia ligament and triggers it to become inflamed and possibly start to tear.
    There are certainly other situations that can trigger plantar fasciitis, like wearing a shoe that may not be the right shape, or offer the right amount of support, or certain types of impacts to the bottom of the foot. Those impacts may put pressure on the plantar fascia ligament causing it to become bruised or stressed.
    Plantar fasciitis is commonly treated with a footbed or orthotic in the beginning to try to stabilize the bone structure of the foot so that the plantar fascia ligament can stay more relaxed so undue stress is not put on the ligament.

    A: Heel spurs are small deposits of bone-like calcium on the heel bone (calcaneus), usually on or near the bottom of the foot where the long tendons of the arches (plantar fascia) insert into the bone.
    Q: What causes heel spurs?
    A: Some experts believe that a spur is the body's attempt to strengthen its foundation. Being overweight, having abnormal foot biomechanics, or running regularly on hard surfaces can cause excessive stress on the foot, which may lead to the production of a spur. Other authorities contend that spurs are a normal part of the aging process: some studies have shown that calcaneal spurs are more frequently found in people after age 50.
