Hemangioma and Vascular Birthmark Center (HVBC): What are Hemangiomas and Vascular Birthmarks?
Vascular birthmarks include hemangiomas and vascular malformations . They are the most common benign tumor of infancy, afflict approximately 400,000 babies every year. About 10% are pronounced enough to demand the attention of a doctor.
Hemangiomas and vascular malformations are benign growths ranging from flat, plum-colored port-wine stains to distorting tomato-like protrusions that can obstruct vision and breathing, as well as, threaten lives.
Hemangiomas can be differentiated from vascular lesions by their natural history and appearance.
Hemangiomas tend to be more common in premature
Two growth spurts are typical. The first occurring between 2 and 8 weeks of life and the second at approximately 4 to 6 months of life.
Involution
, or regression, begins at approximately the first year of life and can last anywhere from 18 months to 12 years.
Some hemangiomas involute rapidly and completely and leave an almost perfect result, while others may take several years and still leave a substantial residual deformity that requires surgical correction. On occasion, significant complications can occur such as airway compromist, visual disturbances, ulcerations and cosmetic deformity.
Types of Hemangiomas:
Superficial
Bright red during growth phase, dusky red involution. Involves more superficial skin.

HEMANGIOMAS Por la Dr. Azucena Ríos Ponce Existen cuatro variedades de hemangiomas: capilar, “en araña”, nevo flameo y cavernoso. El primero tiene su origen en un trastorno vascular y está constituido por un crecimiento anárquico de células endoteliales; su característica principal es que se manifiesta con un rojo brillante en la superficie dérmica que suele aparecer en los primeros meses del neonato, aumentando de tamaño muy lentamente pero desapareciendo hacia los 5 a 9 años de edad, quedando simplemente una mancha parda, una cicatriz o una zona de piel rugosa. El hemangioma “en araña” tiene la misma estirpe del primero y es una lesión vascular, también de color rojizo, menos fulgurante que la anterior pero que tiene, a diferencia del primero, un vaso central como arteriola, con proyecciones diminutas alrededor que semejan las patas de una araña. No son congénitos y se suelen encontrar tanto en niños como en adultos. Una de las patologías que generan estas lesiones es la cirrosis hepática, con la particularidad anatómica de que éstas pueden ser muy prominentes; otra identificación es que se encuentran con frecuencia en embarazadas o en mujeres que toman anticonceptivos por mucho tiempo. Estas formaciones son asintomáticas y suelen desaparecer de 5 a 8 meses después del parto o al suspender los fármacos contraceptivos.

She has one under her right eye, one in between her eyes, one on her back and a very small one under her neck. She was born at 25 weeks. She is now 8 mo. old actual age and 4 1/2 mo. adj. age. I was just wondering if anyone else's children were born with these. The big one on her back has already started to shrink. I was wondering how long before they are gone. Everytime we go out in public I have to explain what happened to her eye everytime someone wants to look at the babies. It's really starting to get on my nerves. When I look at her now I don't even see them. All I see if the beautiful little face behind them. I just worry about other kids teasing her if they do take years to go away.Any help or experience is greatly appreciated.

MANAGEMENT OF LIFE THREATENING HEMANGIOMAS Hemangioma of infancy is an angiomatous disorder characterized by the proliferation of capillary endothelium, with multilamination of the basement membrane and the accumulation of mast cells, fibroblasts, and macrophages. In this angiogenic disease, the abnormal growth of capillaries results in tissue damage, sometimes with fatal consequences. Hemangioma is the most common tumor of infancy, occurring in 10-12% of whites and in up to 22% of preterm babies weighing less than 1000g. The female to male preponderance is 3:1. Cutaneous hemangiomas are the most common, they usually appear as a single tumor, although multiple cutaneous lesions also occur, often with involvement of other organs (generalized hemangiomatosis). The cutaneous hemangiomas are rarely fully grown at birth. They usually appear during the first few weeks of life as an erythematous macule, a blanched spot, or localized telangiectasia. However they have rapid postnatal growth for 8 to 18 months (proliferative phase), followed by very slow but inevitable regression for the next 5 to 8 years (involutive phase). There is complete resolution of hemangiomas in over 50% by the age of 7 years, with continued improvement in the remaining children until the age of 10-12 years. The rate of regression is not related to the sex or age of the infant, the duration of the proliferative phase, or the site, size, or appearance of the hemangiomas. Most hemangiomas are small, harmless birthmarks that involute to leave either normal or slightly blemished skin. However, even small hemangiomas can obstruct the airway or impair vision. A few may behave aggressively and destroy a vital structure and they may cause pulmonary hypertension, congestive heart failure by diverting a considerable volume of blood and producing high output failure, disfigurement or loss of a function as a result of direct invasion or necessary surgical intervention. The increased capillary endothelial surface that characterizes a giant hemangioma can also trap platelets and may cause a thrombocytopenic coagulopathy (Kasabach-Merritt syndrome), microangiopathic hemolytic anemia and bleeding may also occur.

    Hemangiomas
    Hemangiomas are classified according to their location. They are either deep superficial or compound . They can also grow internally: GI tract, liver, kidneys, adrenals, brain and lungs.
    Deep - occur in lower dermis or subcutaneous tissue below the collagen layer. [
    Superficial - occur in upper dermis above the collagen layer. [
    Compound - combination of both deep and superficial lesions. [ Vascular Malformations
    Vascular malformations are classified according to the type of blood vessels involved.
    Midline Venular (Capillary) - flat, macular stains such as stork bite, salmon patch, and angel kiss. The very small veins (venules) of this lesion are always situated in the midline. They usually fade within the first year of life. Venular - known as portwine stains, previously referred to as capillary malformations. The venules of this lesion are located in the superficial layer of the dermis. Venous - erroneously referred to as cavernous hemangiomas. Comprised of large dilated veins in the subcutaneous tissue.

    HEMANGIOMAS
    Hemangiomas are strawberry-colored "birthmarks". They are not rare, and vary from tiny blebs to large and multiple tumor-like growths. They are not true birthmarks since they are mostly not seen at birth, but start in infancy and then begin to grow. Hemangiomas first appear from birth up to 18 months, then slowly shrink. Port wine stains and other true birthmarks are fully formed at birth and do not grow wider. Doctors disagree over how hemangiomas should be dealt with. The answer may depend on whether you see a Dermatologist, Plastic Surgeon or other specialist. Because many of the smaller birthmarks resolve on their own with no intervention, most doctors agree that you should leave small hemangiomas that are not growing alone, especially if they are on skin normally covered by clothing. Hemangiomas that require early aggressive treatment include those that are cosmetically deforming, growing rapidly or obstructing vision, hearing, breathing, eating or any other body function. Hemangiomas on the lower face and neck may later block internal airways. Large facial hemangiomas may cause psychosocial impairment. Also, larger hemangiomas that are left alone to regress (shrink away over years) will eventually look better if the resulting saggy, stretched out skin and fatty tissue is surgically removed.
