1. Gross hematuria: initial - at onset of urinary stream, suggests distal urethral source terminal - at end of urinary stream, suggests source at bladder neck /trigone, or proximal urethra total - throughout entire urinary stream, suggests bladder or upper tract source 2. Asymptomatic hematuria: occurring in the absence of symptoms or abnormal exam findings. 3. Isolated hematuria: occurring in the absence of other urinary abnormalities.

    Hematuria
    What is hematuria? Hematuria is the presence of red blood cells (RBCs) in the urine. In microscopic hematuria, the urine appears normal to the naked eye, but examination under a microscope shows a high number of RBCs. In gross hematuria, the urine is red or the color of cola, which can be seen with the naked eye. What causes hematuria? Most of the causes are not serious, but, because hematuria may be the result of a tumor or other serious problem, a doctor should be consulted. Several conditions can cause hematuria. For example, exercise may cause hematuria that goes away in 24 hours. Many people have hematuria without having any other related problems. How is hematuria diagnosed?

    A patient information handout on microscopic hematuria, written by the authors of this article, is provided on page 1154. M icroscopic hematuria is defined as the excretion of more than three red blood cells per high-power field in a centrifuged urine specimen. Because the degree of hematuria bears no relation to the seriousness of the underlying cause, hematuria should be considered a symptom of serious disease until proved otherwise. Microscopic hematuria is defined as the excretion of more than three erythrocytes per high-power field, but the degree of hematuria does not correlate with the seriousness of the underlying cause of the bleeding. The widespread use of dipstick urinalysis in clinical practice and health screening has resulted in increased recognition of microscopic hematuria and has raised concerns about the appropriate diagnostic investigation. The prevalence of asymptomatic microscopic hematuria in adult men and postmenopausal women has been reported to range from 10 percent to as high as 20 percent. Routine screening of all adults for microscopic hematuria with dipstick testing is not currently recommended because of the intermittent occurrence of this finding and the low incidence of significant associated urologic disease.

The literature agrees that gross hematuria warrants a thorough diagnostic evaluation. By contrast, microscopic hematuria is an incidental finding, and whether physicians should test for hematuria in asymptomatic patients remains at issue. No major organization currently recommends screening for microscopic hematuria in asymptomatic adults, even though bladder cancer is the most commonly detected malignancy in such patients.

    Blood in the urine is usually caused by kidney and urinary tract diseases. However, there are a couple of exceptions:
    • In women, the blood may appear to be in the urine when it's actually coming from the vagina In men, the urethra carries both urine and semen out of the body and what may be mistaken for urinary bleeding is sometimes a bloody ejaculation usually due to a prostate problem. In children, coagulation disorders (such as hemophilia ) or other hematologic problems such as sickle cell disease renal vein thrombosis , or the thrombocytopenias can be underlying reasons for newly discovered blood in the urine.

    What is hematuria?
    Hematuria means that red blood cells are in the urine. Urine does not normally contain red blood cells because the filters in the kidney prevent blood from entering the urine. In hematuria, the filters or other parts of the urinary tract allow blood to leak into the urine. Microscopic hematuria means that the blood is detected only with a microscope, while gross hematuria means the urine appears red or the color of tea or cola. Microscopic hematuria in an otherwise healthy child does not usually need to be investigated unless it is present in at least three urine tests over several months. However, if the child has high blood pressure, chronic kidney disease or protein in the urine, then hematuria should be investigated promptly What causes hematuria?

Often, if a child complains of painful urination or abdominal pain, his doctor may want to test the child's urine using a paper test strip. Sometimes the blood is obvious like a clot and this is called gross hematuria . It is important to always test the urine even when it appears red because not all obviously red urine has blood in it. For example, certain foods like beets and certain medicines like metronidazole (an antibiotic) can make the urine turn red.

    I am 40 - 10 months ago I experienced my first UTI - antibx cleared the infection but I continued and continue to have persistant asymptomatic microscopic hematuria - IVP:negative cystology:negative cystoscopy:negative.
    What would the usual algorithm say to do next?
    My mother died 2 years ago of renal transitional cell carcinoma.
    Would an abdominal CT scan be in order? What are the chances of a CT scan detecting anything, given the other tests were negative? Thanks

    Dear Beth, The work up that has been done in your case is the gold standard , and should be adequate. There is no preponderance of evidence suggesting a familial predisposition in the case of transitional cell cancer. The best follow up for you would be urine cytology checks, probably on a yearly basis to be certain that if a conversion occurs , it is dealt with expeditiously. A CT scan at this point is unnecessary. It is unlikely to add much to the diagnostic process. The best of luck to you. This information is provided for general medical education purposes only.
