    KLIPPEL-TRENAUNAY-WEBER SYNDROME
    DEFINITION:
    A disorder classically characterized by the triad of cutaneous hemangiomas, hemihypertrophy, and varicose veins.
    EPIDEMIOLOGY:
    • incidence: over 700 cases worldwide
    • age of onset:
      • the cutaneous lesions and hemihypertrophy are present at birth or appear soon thereafter
    • risk factors:
      • sporadic but some cases may be familial
      • M = F
      PATHOGENESIS:
      1. Background
      • first reported in 1900 by Klippel and Trenaunay
      • also called Angio-Osteohypertrophy Syndrome
      • if associated with arteriovenous fistulae then called Parker-Weber Syndrome
      2. Pathogenesis
      • unknown etiology
      • may be related to the abnormal regulation of soft tissue growth factors
      • the hemihypertrophy is due to hyperplasia of both bone and soft tissue +/- vascular malformations
      • atresia or hypoplasia of the deep veins may increase blood flow through the more superficial veins resulting in varicose veins

    Although Wilms tumor occurs sporadically in children, 8% of Wilms tumors will develop in individuals who have a recognizable pattern of malformation. High-risk Wilms tumor phenotypes can generally be grouped as "overgrowth" syndromes or "non-overgrowth" syndromes. 1. Overgrowth Syndrome The most common disorders in the overgrowth category are Beckwith-Wiedemann syndrome and isolated hemihypertrophy. A. Beckwith-Wiedemann Syndrome Beckwith-Wiedemann Syndrome (BWS) is characterized by pre and postnatal overgrowth, omphalocele, macro-glossia, nephromegaly and ear lobe creases. Somatic hemihypertrophy may or may not be present. While 80% of BWS cases are sporadic, 20% show a complex familial inheritance pattern explained by alteration of genes at 11P15. For BWS in general, over-expression of the insulin-like growth factor II gene located at 11P15 is believed to cause the characteristic overgrowth and perhaps confer increased risk of Wilms tumor. The overall risk of neoplasia with BWS may approach about 8%, and specifically the Wilms tumor risk appears to be in the range of 3-5%. B. Isolated Hemihypertrophy

      GENERAL INFORMATION
      This summary on Wilms' tumor is an overview of prognosis, diagnosis, classification, and treatment. The National Cancer Institute created the PDQ database to increase the availability of new treatment information and its use in treating patients. Information and references from the most recently published literature are included after review by pediatric oncology specialists.
      Cancer in children and adolescents is rare. Children and adolescents with cancer should be referred to medical centers that have a multidisciplinary team of cancer specialists with experience treating the cancers that occur during childhood and adolescence. This multidisciplinary team approach incorporates the skills of the primary care physician, pediatric surgical subspecialists, radiation oncologists, pediatric medical oncologists/hematologists, rehabilitation specialists, pediatric nurse specialists, social workers, and others in order to ensure that children receive treatment, supportive care, and rehabilitation that will achieve optimal survival and quality of life. Guidelines for pediatric cancer centers and their role in the treatment of pediatric patients with cancer have been outlined by the American Academy of Pediatrics. ] At these pediatric cancer centers, there are clinical trials available for most of the types of cancer that occur in children and adolescents, and the opportunity to participate in these trials is offered to most patients/families. Clinical trials for children and adolescents with cancer are generally designed to compare potentially better therapy with therapy that is currently accepted as standard. The majority of the progress made in identifying curative therapies for childhood cancers have been achieved through clinical trials.     • My friend's baby was just diagnosed with a disease where half of his body will grow faster than the other half. What causes this condition? What are treatment options?
    Hemihypertrophy (say: hem-ee-hi-per-troh-fee) is a rare condition in which one side of the body grows faster than the other. It can run in families, or occur by chance. Most people with hemihypertrophy look fairly normal, but might have to get different size shoes or gloves. Some people with hemihypertrophy have different leg lengths, and will need surgery or shoe lifts to correct this when they reach adulthood.

    Your son's leg pain may be due to his growth spurts. Children with BWS do most of their growing it seems when they are younger. Be sure his doctor is watching out for asymmetrical leg growth (one leg growing faster then the other one). Warm towels or soaking in a warm bath may help. If it still hurts then talk to his doctor about ibuprofen. Tip #3
    There is a loose connection between bws and heel cord tightness (which causes the toe walking). Physical therapy is one the treatments. Tip #4
    I have noticed people talking about how they buy their shoes. My son is not there yet, but I know that I will be buying different sizes so I did some research. Someone gave me a list of websites for parents of children with special needs and I found the site listed below. They have a seperate listing for footwear and if you click on it, it will take you to a list of stores that sell special footwear including shoes that are different sizes.     ©P¨Îº¡ ³¯«p¥þ ±i®¶©÷ Hemihypertrophy clinically manifestates asymmetric growth of carnium, face, trunk, limbs, with or without visceral involvement. It could be an isolated finding in an normal individual or occur in several syndromes such as Beckwith-Wiedemann syndrome. Hemihypertrophy usually occurs sporadically and 1:86,000 in incidence. The clinical feature of hemihypertrophy is associated with neoplasm, most frequently with Wilms tumor, adrenocortical carcinoma, hepatoblastoma and rarely with rhabdomyosarcoma, neuroblastoma, pheochromocytoma or undifferentiated sarcoma of lung. The overall incidence of abdominal neoplasm associated with hemihypertrophy is 5.9% and average annual incidence in the first 4 years of life is 0.027 per person-year. In this article, we describe 2 patients with congenital asymmetry of limbs who presented with hepatoblastoma and were underwent surgical intervention. The postoperative recovery were smooth and the patients were doing well at present follow up.
