What are viral hemorrhagic fevers?
Researchers wearing protective clothing investigate the 1993 HPS outbreak Viral hemorrhagic fevers (VHFs) refer to a group of illnesses that are caused by several distinct families of viruses. In general, the term "viral hemorrhagic fever" is used to describe a severe multisystem syndrome (multisystem in that multiple organ systems in the body are affected). Characteristically, the overall vascular system is damaged, and the body's ability to regulate itself is impaired. These symptoms are often accompanied by hemorrhage (bleeding); however, the bleeding is itself rarely life-threatening. While some types of hemorrhagic fever viruses can cause relatively mild illnesses, many of these viruses cause severe, life-threatening disease. The Special Pathogens Branch (SPB) primarily works with hemorrhagic fever viruses that are classified as biosafety level four (BSL-4) pathogens. A list of these viruses appears in the

RNA viruses from several different viral families: the Filoviridae, which consists of Ebola and Marburg viruses; the Arenaviridae, including Lassa fever, Argentine and Bolivian hemorrhagic fever viruses; the Bunyaviridae, including various members from the Hantavirus genus, Congo-Crimean hemorrhagic fever virus from the Nairovirus genus, and Rift Valley fever from the Phlebovirus genus; and Flaviviridae, such as Yellow fever virus, Dengue (hemorrhagic fever) virus, and others. Signs and Symptoms: VHFs are febrile illnesses which can be complicated by easy bleeding, petechiae, hypotension and even shock, flushing of the face and chest, and edema. Constitutional symptoms such as malaise, myalgias, headache, vomiting, and diarrhea may occur in any of the hemorrhagic fevers. Diagnosis: Definitive diagnosis rests on specific virologic techniques. Significant numbers of patients with a hemorrhagic fever syndrome should suggest the diagnosis of a viral hemorrhagic fever. Treatment: Intensive supportive care may be required. Antiviral therapy with ribavirin may be useful in several of these infections. Convalescent plasma may be effective in Argentine hemorrhagic fever.

What is Ebola hemorrhagic fever?

Ebola hemorrhagic fever (Ebola HF) is a severe, often-fatal disease in humans and nonhuman primates (monkeys, gorillas, and chimpanzees) that has appeared sporadically since its initial recognition in 1976. The disease is caused by infection with Ebola virus, named after a river in the Democratic Republic of the Congo (formerly Zaire) in Africa, where it was first recognized. The virus is one of two members of a family of RNA viruses called the Filoviridae. There are four identified subtypes of Ebola virus. Three of the four have caused disease in humans: Ebola-Zaire, Ebola-Sudan, and Ebola-Ivory Coast. The fourth, Ebola-Reston, has caused disease in nonhuman primates, but not in humans. In general, the term "viral hemorrhagic fever" is used to describe a severe multi-system syndrome, where multiple organ systems in the body are affected. VHFs include four families of viruses: filiviruses such as the Ebola and Marburg viruses, arenaviruses such as Lassa, bunyavirus such as Rift Valley Fever and flaviviruses such as yellow fever and dengue. They can all cause serious, life-threatening illnesses. Some, including Ebola, Marburg and Lassa, are contagious. Although the Ebola virus gained notoriety in various films such as Outbreak and books, VHFs aren't expected to be prime bioweapon candidates since people die so quickly from them and often don't get a chance to infect others. During World War II, the Japanese army fed botulinum toxin to prisoners of war in Manchuria, with lethal results. How Is It Spread? VHFs naturally occur in humans only after contact with an infected insect, rodent or larger mammal. Transmission can happen via touching fecal matter, receiving an insect bite or handling contaminated meat. It's possible VHFs can be manufactured for aerosol dissemination but the bacterium generally don't fare well in this form.

  • Filoviridae: Ebola and Marburg viruses
  • Arenaviridae: Lassa fever virus and a group of viruses referred to as the New World arenaviruses
  • Bunyaviridae: Crimean Congo hemorrhagic fever virus, Rift Valley fever virus, and a group of viruses known as the 'agents of hemorrhagic fever with renal syndrome'
  • Flaviviridae: dengue, yellow fever, Omsk hemorrhagic fever, and Kyasanur Forest disease virus Hemorrhagic fever viruses are all capable of causing a clinical diseases associated with fever and bleeding disorder, classically referred to as viral hemorrhagic fever (VHF). None of these viruses occurs naturally in the United States. Risk factors for these diseases include travel to certain geographic areas where these diseases may naturally occur (such as certain areas of Africa, Asia, the Middle East, and South America), handling of animal carcasses, contact with sick animals or people with the disease, and arthropod bites. The Working Group for Civilian Biodefense identified a subset of these viruses that pose particularly serious threats as biological weapons, based on, among other characteristics, their infectious properties, morbidity and mortality, transmissibility by way of aerosol dissemination, and prior research and development as biological weapons. Specifically, these viruses are: Ebola, Marburg, Lassa fever, New World arenaviruses, Rift Valley fever, yellow fever, Omsk hemorrhagic fever, and Kyasanur Forest disease. Isolation measures and barrier nursing procedures are indicated.
