General Welcome/Mission Health Centers Services Departments ... Links/Resources After food leaves the stomach, it enters the small intestine, the beginning of the lower gastrointestinal (GI) tract. As it passes through, the major portion of digestion and absorption takes place. Undigested food residue, debris, and some water then pass into the large intestine, or colon, where almost all of the water absorbed, and the solids are compacted as feces. This fecal mass is stored until distention causes a reflex muscle movement that propels the mass into the rectum for defecation and excretion through the anus. Normally, ingested food passes through this system and its waste products are excreted within 35 – 70 hours, resulting in a normal bowel movement 3-12X/week. However, the defecation reflex is rather weak, and easy to ignore. We ignore the reflex for many reasons, including inconvenience or lack of private, clean, or comfortable bathroom facilities. After a few minutes, the reflex goes away, and will not begin again for several hours. During this time, the fecal mass becomes drier and harder, resulting in constipation. Constipation Constipation describes bowel movements that involve excessive straining, difficulty, or discomfort; or that occur less than twice a week. Although occasional constipation is common, chronic or persistent constipation and associated straining at bowel movements can lead to more serious problems. Complications of chronic constipation may include hemorrhoids, anal fissures, and damage to the muscles and supportive tissues of the anus and perineum. The most common cause of constipation is a diet lacking fluid and fiber; in combination with poor bowel habits, such as routinely delaying defecation. Other causes of constipation may include:

Causes
Hemorrhoids, also known as "piles," are enlarged veins located in the lower rectum or anus that are caused by blood blockage in the veins of the hemorrhoidal complex. The hemorrhoids begin above the internal opening of the anus and may become large enough to protrude. The subsequent squeezing of them while sitting brings on pain. Hardened stools, or constipation, is one of the primary causes of hemorrhoids. Constipation may be due to diet, lack of fluids or medication, among other things. When someone has constipation, the wave-like motion (peristalsis) that moves the bowels through the intestines, is nearly non-existant, and the straining to have a bowel movement causes hemorrhoids. Traditional Treatment
Standard conventional medical treatment consists of lubricating the surface of the veins with medication to kill the pain and shrink the hemorrhoid, sitz baths, and hot and cold packs put on the vein to soothe the irritated area. In conventional allopathic treatment, the hemorrhoids may sometimes be hardened by injection, tied off (the preferred method), or removed by surgery.

But just because they're so common doesn't mean that you should ignore them or the bleeding they often cause. Blood in the stool can result in anemia and may be caused by something more serious than hemorrhoids. What causes hemorrhoids?
Straining during elimination, usually because of constipation, increases pressure in the veins of the anal canal. Eventually, the veins become weak, stretched, and enlarged. These enlarged veins are hemorrhoids. They may occur inside or bulge outside the anal canal. How can I relieve the discomfort of hemorrhoids?
First, get rid of the constipation to make passing stools easier. Eat plenty of high-fiber foods (whole-grain breads, bran cereals, fresh fruits and vegetables). Drink lots of water and juice. Exercise regularly, and lose excess weight. Try some of these methods to relieve discomfort:
  • For the first day, apply cold compresses or ice packs to ease swelling, especially if hemorrhoids extend outside the anal canal. Apply cold for just a few minutes at a time.
  • Try bed rest if you have a lot of pain.

External hemorrhoids represent distended vascular tissue in the anal canal distal (outside) to the dentate line (the junction between the rectal mucosa and the specialized skin of the anal canal, called the anoderm). External hemorrhoids are covered by anoderm and perianal skin richly innervated with somatic pain fibers. Diseases affecting the anal canal or the external hemorrhoidal vessels can be extremely painful. A simple incision and removal of thrombosis, as opposed to hemorrhoidectomy, is associated with a significant rate of rethrombosis.

Hemorrhoids are enlarged and inflamed veins in the lower rectum and anus, which sometimes occur outside the anus and can be seen (external type); at other times they occur inside the rectum and cannot be seen (internal type). WHAT CAUSES THEM?
Many things are known to be related to hemorrhoids: straining to have a bowel movement; passing hard, dry stools (constipation); sitting for long periods of time; and pregnancy (the pressure it puts on the bowel). Other factors include diet, overuse of laxatives, inflammation or infection of the rectal area, heredity, and other problems of the rectum. Hemorrhoids are a very common problem. It is not known exactly how many people have them, because often they do not cause symptoms. We do know that the largest number of symptoms from hemorrhoids occur in people between the ages of 20 and 50. WHAT ARE THE COMMON SYMPTOMS?
Pain, itching, and bleeding are common symptoms. Hemorrhoids inside the rectal opening are usually painless, but often bleed and cause a feeling of fullness, which may cause an urge to have a bowel movement even when there is no stool present. Straining to have a bowel movement can aggravate the problem. Bleeding from internal hemorrhoids usually occurs at the end of the bowel movement. You may notice that the stool is streaked with blood, blood may drip into the toilet bowl, or it will appear on the toilet paper when you wipe. Rectal bleeding can be a signal of a more severe problem and should be evaluated. Other symptoms include increased moisture or itching around the rectum, or finding a small bulge that is very tender to the touch outside the rectum.

Most people who think they have hemorrhoids are actually mistaken. We've all been convinced by clever advertising for "hemorrhoid creams" that any skin irritation around the anus is a symptom of hemorrhoids. The truth is hemorrhoids rarely cause significant symptoms and when they do, over-the-counter remedies are not going to solve the problem. So, what about that itching, burning, soreness, dampness in the anal area? If you have these symptoms, they are almost certainly not from hemorrhoids. Instead, they are symptoms of a skin condition in the anal area called " pruritis ani " which literally means "itchy anus." What are hemorrhoids? Hemorrhoids are enlarged "anal cushions." Anal cushions are the tail end of the anal canal. They are like "lips" at the other end of your intestinal tract. The job of anal cushions is to provide a complete seal of the anus when it is closed. Sometimes - due to excessive straining, or pregnancy, or a family condition - anal cushions become enlarged and can begin to cause symptoms. We call enlarged anal cushions "hemorrhoids."
