Professor Michael Bailey
As Professor of Surgery and Consultant Surgeon at the Royal Surrey County Hospital and the University of Surrey in Guildford, Michael Bailey is at the forefront of modern abdominal surgery. A pioneer of Laparoscopic surgery and its safe training, Professor Bailey performed one of the first laparoscopic gallbladder removal operations in the UK in 1990 and has since become internationally recognised for his technical and teaching expertise. He has evolved techniques for operating on many other conditions, including inguinal hernia repair and gastro-oesophageal reflux disease. In Guildford he has established and is Director of the Minimal Access Therapy Training Unit, which teaches and researches keyhole surgery. He is also President-Elect and Education Officer of the Association of Endoscopic Surgeons of Great Britain and Ireland, and Minimal Invasive Tutor at the Royal College of Surgeons of England. Apart from laparoscopic surgery, Professor Bailey's clinical interests are in gastrointestinal malignancy and thyroid surgery.

Manuscript preparation The manuscripts must be written in English, and the authors are urged to take special care in ensuring clarity, brevity and accuracy of information and language. Authors whose mother tongue is not English should have their papers checked for linguistic accuracy by a native English speaker. STOMA ('Parastomal') HERNIAS A Hernia is a weakness or split in the muscle wall of the abdomen which allows the abdominal contents (usually some part of the intestine) to bulge out. The bulge is particularly noticeable upon tensing the abdominal wall muscles - such as occurs when coughing, sneezing, straining or simply standing. Stomas pose an additional problem. When a stoma is brought out to the surface of the abdomen it must pass through the muscles of the abdominal wall, thus a potential site of weakness is immediately created. In the ideal situation the abdominal wall muscles form a snug fit around the stoma opening. However, sometimes the muscles come away from the edges of the stoma thus creating a hernia - in this case, an area of the abdominal wall adjacent to the stoma where there is no muscle. Factors that can contribute to causing a stoma hernia to occur include coughing, being overweight or having developed an infection in the wound at the time the stoma was made. The development of a stoma hernia is often a gradual phenomenon, with the area next to the stoma stretching and becoming weaker with the passage of time. This weakness, or gap, means that every time one strains, coughs, sneezes or stands up, the area of the abdomen next to the stoma bulges, or the whole stoma itself protrudes as it is pushed forwards by the rest of the abdominal contents behind it. As with all hernias the size will increase as time goes by. Stoma hernias are rarely painful, but are usually uncomfortable and can become extremely inconvenient.

What is the Heartburn? Potentially dangerous confusion can arise when someone neglects their heart pain believing it to be heartburn. What Causes Heartburn? Acid is present in the stomach to digest food. Heartburn occurs when small amounts of this acid rise up into the gullet (oesophagus) - the tube which carries food from the mouth to the stomach. This is called reflux. The gullet, unlike the stomach, does not have a protective lining. So when it is exposed to the acid, it can become inflamed and painful. Why Doesn't Everyone get Heartburn? Some known factors that can lead to heartburn include:
pregnancy
smoking
eating large meals, especially near bedtime
being overweight
bending a lot
wearing tight clothing around the waist. In most of these cases, it is the increased pressure which overcomes the normal muscle contraction and causes heartburn. Smoking, on the other hand, relaxes the muscle around the gullet, with the same result. What is Hiatus Hernia?
