Every year more than half a million hernia repairs are performed in the United States. A hernia occurs when inner layers of abdominal muscle become weakened. The lining of the abdomen then bulges out into a small sac, and part of the intestine or abdominal tissue may enter the sac. Hernias occur most commonly in the groin (inguinal hernia), the navel (umbilical hernia), and at the incision site of a previous surgery. A hiatal, or diaphragmatic, hernia occurs when the lower part of the esophagus and a portion of the stomach slide up through the esophageal hiatus, an opening in the diaphragm through which the esophagus passes before it reaches the stomach. In a small percentage of cases, the junction of the esophagus and stomach remains in place, but a portion of the stomach rolls up and through the esophageal hiatus alongside the esophagus. Some people with hernias remain relatively free from symptoms. But a hernia can cause severe pain and other potentially serious problems (e.g., infection, bowel obstruction). Surgery is the only way to repair them, because they do not resolve on their own.

What is a Hernia? A hernia is a protrusion of an organ or tissue through an abnormal opening in the body. Most hernias occur when a piece of intestine slips through a weakness in the abdominal wall creating a bulge you can see and feel. Hernias can develop around the naval, in the groin, or any place where you may have had a surgical incision. Some hernias are present at birth. Others develop slowly over a period of months or years. There are two main types of hernias: hiatal and inguinal. Hiatal Hernia A hiatal hernia can be caused by many different things. Some of the causes are: obesity, pregnancy, tight clothing, sudden physical exertion, such as weight lifting straining, coughing and abdominal injury. Symptoms of Hiatal Hernia Most hiatal hernias cause no symptoms.

Patients with a hiatus hernia need to understand what it is and what might occur with it. Most importantly, they need to know it is unusual for serious problems to develop from this type of hernia. Anatomy
The diaphragm is a sheet of muscle that separates the lungs from the abdomen. When a person takes a deep breath, the dome-shaped diaphragm contracts and flattens. In doing this, the diaphragm pulls air into the lungs. The left diaphragm contains a small hole through which passes the tube-shaped esophagus that carries food and liquid to the stomach. Normally this hole, called a hiatus, is small and fits snugly around the esophagus. The J-shaped stomach sits below the diaphragm. What Causes a Hiatus Hernia?
In some people, the hiatus or hole in the diaphragm weakens and enlarges. It is not known why this occurs. It may be due to heredity, while in others it may be caused by obesity, exercises such as weightlifting, or straining at stool. Whatever the cause, a portion of the stomach herniates, or moves up, into the chest cavity through this enlarged hole. A hiatus hernia is now present. Hiatus hernias are very common, occuring in up to 60 percent of people by age 60. What Are the Different Types of Hiatus Hernia?

Most commonly, a hernia is a swelling that you or your doctor discover in your child's inguinal ("groin") area. They are much more common in boys than in girls, although inguinal hernias do occur in girls. A hernia usually pops up without warning and is noticed at bath or diaper change time, often when the child is crying. The area they first appear in lies just above the scrotum, on a line drawn from the scrotum up to the nipple on that side. What has happened is that there was a weakness in the floor of the abdomen where the blood vessels, nerve and spermatic cord leading down to the testicle penetrate the wall of the abdominal cavity. Over time, as the baby cries and normally strains to push out his bowel movements, the pressure gradually works this weakness into an opening through the abdominal wall (first smaller and then larger) and ultimately through a sort of tunnel and down into the testicular sac (scrotum). When the hole is large enough, a short section of intestine can slide down into the tunnel and the scrotum.

Rond het begrip 'hernia' bestaat nogal wat verwarring. Veel mensen weten niet precies wat hernia is behalve dat men door deze aandoening 'door de rug gaat'. Om te kunnen verduidelijken wat hernia nu exact is, zullen we eerst aandacht besteden aan de functie van de tussenwervel- schijven in uw wervelkolom.
Wat is en doet een tussenwervelschijf?
Tussenwervelschijven zijn schijven tussen de beweegbare wervels die de schokken of stoten op uw wervelkolom opvangen Ook zorgen zij ervoor dat de wervelkolom soepel blijft. Een tussenwervelschijf is opgebouwd uit een geleiachtige kern die door kraakbeen is omringd (zie tekeningen middenpagina). Een netwerk van stevige uiterst fijne vezels verbindt de wervels en tussenwervelschijven met elkaar tot een sterke constructie. Deze constructie houdt de wervels op hun plaats.
Wat is hernia en hoe wordt deze veroorzaakt?
Er is sprake van hernia wanneer de kern van een tussenwervelschijf een ongewone uitstulping laat zien. Die uitstulping drukt op de zenuwwortel en kan zelfs het ruggenmerg doen irriteren (zie tekeningen). Pijn is hiervan zonder meer de consequentie. Hernia kan zich op twee manieren vormen. Ten eerste kan een acute aandoening de oorzaak van hernia zijn. Ten tweede kan een degeneratieproces ofwel slijtage aan hernia ten grondslag liggen. Een acute aandoening kan ontstaan onder meer door plotseling te draaien in een onjuiste houding te tillen, een schok, een val of zelfs door te hard niesen.

Each year about 600,000 hernia-repair operations are performed in the United States. Until recently, however, all were performed as traditional, "open" procedures requiring a large incision in the lower abdomen. The result was significant pain for patients.
Today, the minimally invasive technique of laparoscopic surgery can be used to repair the most common types of hernias. Although both traditional and laparoscopic hernia surgery are performed on an outpatient basis, patients treated laparoscopically seem to experience more rapid healing and far less pain during recovery. They can return to normal activity, including work, after only a few days, while recovery from traditional hernia repair can be a three- to five week process.
The laparoscopic approach can be used to repair direct and indirect inguinal (groin) hernias and femoral (below the groin) hernias. Some hernias located in other parts of the abdominal wall can be repaired laparoscopically as well.

What is a Hernia?
HERNIA
A hernia is the protrusion of an organ (small bowel, large bowel, ovary, etc..) through a weakness in the abdominal wall. This weakness can be congenital (umbilical hernias, epigastric, inguinal hernias) or acquired (some inguinal hernias), and following abdominal operations. SURGICAL TECHNIQUES
The surgical techniques available for the treatment of hernia are classed in three groups:
    1. The pure tissue repairs (Shouldice operation);
    2. The Tension-free repairs (Lichtenstein, Gilbert, Rutkow);
    3. The laproscopic techniques.
THE SHOULDICE OPERATION
This pure tissue technique was introduced in 1945 by Dr. E.E. Shouldice. Since then, the operation has proved to be the most reliable, safe and cost-effective technique. The operation is carried out under local anaesthesia, contributing to the exceptional safety of the operation. Just as important as the technique is the fact that a team of 12 surgeons have devoted all their time and resources exclusively to the practice of hernia surgery. Shouldice Hospital surgeons will each perform from 400 to 700 operations a year, providing a sixth valuable sense in the recognition and handling of all the problems that surround hernia pathology. The Shouldice Team of Surgeons have performed more than 270,000 operations.
TENSION-FREE REPAIRS
Tension-free repairs demand the use of foreign prosthetic materials (mesh) for all necessary repairs. The actual hernia is left in place and the defect occluded or covered by a prosthesis. Tissue then grows into the mesh. These prosthesis are not without problems. Should infection supervene, all prostheses must be removed and the hernia repaired by a pure tissue repair. In the few cases where prostheses are a must (less than 2%), the Shouldice surgeons have the knowledge and the techniques to use them. In any case, those surgeons who use mesh for the 'tension-free' repairs have not demonstrated better results. Their chief claim is an earlier return to work, and even that claim is being questioned, as our patients have, on average, returned to work in 8.2 days.

What is an umbilical hernia?
