Hirsutism Excess Facial and Body Hair Hirsutism is the growth of long, coarse hair on the face and body of women in a pattern similar to that found in men. Besides being cosmetically distressing, hirsutism may also signal the presence of a hormone imbalance or a hormone-producing tumor. Normal hair growth Each hair grows from a follicle deep in the skin. As long as these follicles are not completely destroyed, hair will continue to grow even if the shaft, which is the part of the hair that appears above the skin, is plucked or removed. Adults have two types of hair, vellus and terminal. Vellus hair is soft, fine, colorless, and usually short. In most women, vellus hairs grow on the face, chest, and back and give the impression of "hairless" skin. Terminal hairs are the longer, coarser, darker, and sometimes curly hairs that grows on the scalp, pubic, and armpit areas in both adult men and women. The facial and body hair in men is mostly of the terminal type. What causes hirsutism? Most often, excess facial and body hair is the result of abnormally high levels of androgens or male hormones in the blood. Androgens are present in both men and women, but men have much higher levels. These hormones cause hairs to change from vellus to terminal. Once a vellus hair has been transformed to the coarser terminal hair, it usually does not change back. Androgens also cause terminal hairs to grow faster and thicker. Both the ovaries and the adrenals produce androgens. To some degree, estrogens and progesterone, female hormones, prevent the effect of androgens.

  • Hirsutism is when male-patterned hair growth occurs in women. This includes hair growth on the face, chin, abdomen, and chest.
  • Virilization is when frontal balding occurs, muscles increase in size, and the clitoris becomes enlarged and/or thickened.
  • The cause of both Hirsutism and Virilization is excess androgens (male hormones). These hormones are androstenedione, dehydroepiandrosterone sulfate, and testosterone. Androstenedione is made in about equal amounts by the ovaries and the adrenal glands. Dehydroepiandrosterone sulfate (DHEAS) is secreted by the adrenal glands. 60 % of the testosterone in the body is made by the ovaries, while 40% is made when androstenedione in the blood is converted to testosterone. Testosterone is the active hormone (the other two act as precursors, but do not have a direct effects by themselves).

Hirsutism is a condition in which there is excessive growth of hair on the face or body. Although hirsutism can occur in both men and women, it is usually only a problem for women. Women with hirsutism have dark, thick hair on their face, chest, abdomen and back. This thick, dark hair is different from the hair that some women have on their upper lip, chin, breasts or stomach, or the fine "baby" hair all over their body. What causes hirsutism? Hirsutism can be caused by hair follicles that are overly sensitive to male hormones (called androgens), or it can be caused by abnormally high levels of these hormones. A very high level of male hormones may be caused by tumors. Some medicines can cause excessive hair growth. These medicines include birth control pills, hormones and anabolic steroids. Women of Mediterranean ancestry, Middle-Eastern women, Hispanic women and African-American women tend to have more body hair than women from other ethnic groups. Hirsutism seems to run in families. Your family doctor may want to do some tests to find out what is causing the hirsutism. When should I see my doctor about hirsutism?

Hirsutism (hairiness)

This is usually due to above average levels of androgens, the male hormones that are normally present in women at low levels. Some women do not find the excess hair a problem, particularly if it does not affect their face, or it is blonde in colour. Sometimes excess hairiness is not abnormal and is a racial or genetic variation. Initial treatments include bleaching and electrolysis. If these do not produce an acceptable result, drugs may be used to reduce the high androgen levels, if that is the cause. The contraceptive pill contains oestrogen , which reduces androgen levels and will improve hirsutism. A formulation is available which includes a specific drug to reduce these further, known as 'Dianette'. The other component of Dianette is called cyproterone acetate, and this is the next drug to try if hirsutism persists. It is used in a higher dose than contained in the Dianette pill, but must be combined with adequate contraception, as it can cause fetal abnormality if taken during early pregnancy. Spironolactone is another alternative, but this frequently causes erratic periods, so is often given with a low dose contraceptive pill. A newer drug is called flutamide , which appears promising, though its safety profile is less clear. Side effects of the anti-androgens include tiredness, mood changes and reduced sex drive. Both flutamide and higher dose cyproterone acetate have a rare but serious side effect of causing problems with liver function, and so regular blood tests are advised.

Excessive growth of facial or body hair in women is called hirsutism. Description Hirsutism is not a disease. The condition usually develops during puberty and becomes more pronounced as the years go by. However, an inherited tendency, over-production of male hormones (androgens), medication, or disease, can cause it to appear at any age. Women who have hirsutism usually have irregular menstrual cycles. They sometimes have small breasts and deep voices, and their muscles and genitals may become larger than women without the condition. Types of hirsutism Idiopathic hirsutism is probably hereditary, because there is usually a family history of the disorder. Women with idiopathic hirsutism have normal menstrual cycles and no evidence of any of the conditions associated with secondary hirsutism. Secondary hirsutism is most often associated with polycystic ovary syndrome (an inherited hormonal disorder characterized by menstrual irregularities, biochemical abnormalities, and obesity ). This type of hirsutism may also be caused by:

Hirsutism can be a mild cosmetic problem that requires only reassurance and cosmetic therapy or it can have considerable psychologic or medical impact. In the latter women, the goals of therapy are to slow or stop hair growth and to ameliorate any related problems, such as menstrual irregularity and infertility. These goals may be accomplished by reducing androgen secretion or androgen action or by specific therapy directed at the cause of the hirsutism. Hirsutism is the term used for excess facial and body hair growth in women. It refers to a male pattern of hair i.e. over the moustache and beard areas, or more hair growth than usual on the arms and legs. Hairs may also grow on the chest or extend from the groin area on to the abdomen and thighs. What is considered normal for a woman and what is considered hirsute depends on factors such as culture and race. Why is hirsutism important? Hirsutism is not a disease and it is rarely caused by a serious illness. Unfortunately in our society, excess hair growth is considered abnormal and this can cause distress or embarrassment. At least 25% of normal middle-aged women have unwanted facial hair and many women do not realise the treatment options that are available. In some cases, hirsutism may be as a result of an underlying medical disorder. This can easily be ruled out with a medical history and some simple blood tests. The body is completely covered with hair, except the lips, the palms of the hands and the soles of the feet. The number of hair follicles is fixed before birth and is dependent on genetic (inherited) factors. There are 2 types of hairs:

HIRSUTISM (TOO MUCH HAIR) Gabe Mirkin, M.D. Five to 10 percent of North American women have hirsutism, a condition that causes women to have excess hair on their faces and bodies. Women have the same number of hairs on their bodies and faces as men do, but the male hormone, testosterone, makes hairs thicker, darker and longer, making them more noticeable. Some women with noticeable body hair have normal amounts of testosterone and inherit this tendency. Others have a tumor or medical condition that causes their bodies to produce larger amounts of testosterone. Women with noticeable body and facial hair should get blood tests for testosterone to measure male hormones made by the ovaries and DHEAS to measure male hormones produced by the adrenal glands. They should also get a sonogram of their ovaries to test for polycystic ovaries (PCO syndrome) in which the ovaries do not release eggs each month. PCO syndrome is treated like diabetes with drugs (Glucophage, Avandia, Actose) to lower blood insulin levels and severe restriction of foods with added sugar and flour. Over the last 20 years, doctors have treated hirsutism with birth control pills and spironolactone, a drug to block male hormones. Now they also have leuprolide (brand name Leupron) which stops the brain from stimulating the ovaries to produce testosterone (1) and Proscar, the drug to shrink a man's enlarged prostate.(2,3,4) The most effective treatment may be the anti-male hormone, flutamide (brand name Eulexin) to stop the body from responding to testosterone (5,6,7,8,9,10,12). Doctors can measure the benefit of these drugs by drawing PSA blood tests in women (11)

Hirsutism
Hirsutism is defined as the excessive growth of hair on the face, chest, back, abdomen, upper arms and legs of women. Besides being a cosmetic concern, hirsutism may be a sign of an underlying hormonal disorder and a prompt evaluation for the cause of hirsutism is recommended. Excessive hair growth may be due to high levels of male hormones in the blood or increased sensitivity to the male hormones. Once a hair follicle has started to grow thicker and darker it usually can not be changed back and requires electrolysis for permanent removal. Hirsutism is often treated with a combination of hormonal therapy to lower male hormone levels, medications that inhibit the action of male hormones at the hair follicle and cosmetic treatments such as electrolysis, shaving and bleaching and laser.
