Excess facial hair is rarely caused by virilisation. If there is associated acne, voice change, amenorrhoea or androgenic hair loss, or enlarged clitoris, referral to an Endocrinologist is appropriate. Treatment is by bleaching or depilatory products or shaving. Electrolysis needs an expert to obtain good results without scarring. Laser removal will increasingly become available as probably the best treatment for hirsutism. No treatment is available on the NHS for hair removal. Index August 1999

Finasteride cream in hirsutism
ABSTRACT
Objective: To determine, in a preliminary study, whether women with hirsutism attributable to various causes would benefit from treatment with finasteride cream. Methods: Finasteride cream (0.25%) and placebo cream were administered to eight women with various degrees of facial hirsutism. The two creams were used on opposite sides of the face in an area of excessive hair growth. The side chosen for the finasteride cream versus placebo was randomized and blinded. In a 1 cm2 area on each side of the face, hair counts were done every 2 months throughout the 6-month study period. Hair thickness was also measured. Conclusion: In this study of women with facial hirsutism, topically applied finasteride significantly decreased hair growth and thickness, and no adverse effects were noted. INTRODUCTION
Finasteride, a 5a-reductase inhibitor, has been extensively studied and clinically used for treatment of prostatic hyperplasia (1,2). Currently, it is being assessed for use in prostate cancer (3). Because both the prostate and the hair follicle contain this enzyme, finasteride has also been used to treat male pattern baldness (4,5). The mechanism of action of this medication is to block the conversion of testosterone into its active metabolite dihydrotestosterone (DHT). By inhibiting 5a-reductase, the effect of testosterone on the prostate is blunted, and the growth of the prostatic tissue is diminished. By decreasing the levels of DHT and not testosterone, finasteride has not been shown to affect luteinizing hormone, follicle-stimulating hormone, or testosterone levels substantially over time. Only 4 to 5% of men have any change in potency or libido (6,7).

  • What Is It? Normal patterns of hair growth and hair distribution vary widely, determined to a large extent by racial background. For example, whites as a group have more facial and nonscalp hair growth than do blacks or Asians, and white women of Mediterranean heritage normally have more hair growth than do women from Nordic countries. Hirsutism in women means that hair follicles are being over-stimulated by testosterone or other androgen hormones. Androgens are the dominant sex hormones in men, but low androgen levels are normally present in women. Excess hair growth in women may be caused by abnormally high levels of androgens, or by the hair follicles becoming more sensitive to the effects of androgens. In the vast majority of cases, hirsutism is benign and responds to straightforward cosmetic treatment. Occasionally, when additional symptoms suggest that androgens are being overproduced, it is important for a woman to be evaluated by a physician. Overproduction of androgen hormones can signify an abnormality in the ovaries, the adrenal glands or the pituitary gland of the brain. Some excessive hair growth does not fit the pattern of growth triggered by androgen hormones (for example, hair between the eyes, on the forehead, on the temples or high on the cheeks of the face).     Hirsutism is not a disease. The condition usually develops during puberty and becomes more pronounced as the years go by. However, an inherited tendency, over-production of male hormones (androgens), medication, or disease, can cause it to appear at any age. Women who have hirsutism usually have irregular menstrual cycles. They sometimes have small breasts and deep voices, and their muscles and genitals may become larger than women without the condition. Types of hirsutism Idiopathic hirsutism is probably hereditary, because there is usually a family history of the disorder. Women with idiopathic hirsutism have normal menstrual cycles and no evidence of any of the conditions associated with secondary hirsutism. INTRODUCTION – Hirsutism refers to the growth of coarse, dark hair in areas where women typically grow fine hair or no hair at all above the lip, and on the chin, chest, abdomen, and back . This growth is caused by an excess of any of the hormones called androgens (male hormones), which all women have in them normally. Hirsutism warrants medical attention, especially if it develops or worsens rapidly in a woman aged 20 years or older, or if her relatives have comparatively less hair growth. In the vast majority of cases, hirsutism is not caused by serious or malignant medical conditions; however, the cause of hirsutism should be established and underlying conditions that pose related health problems should be treated. Sometimes, the decision to treat hirsutism is a matter of personal choice. When it comes to the amount of body hair, there is a wide range of normal among women. Race and ethnicity play a major role in the growth of body hair: for example, Asian and Native American women tend to have little body hair, whereas Middle Eastern and Mediterranean women tend to have moderate to large amounts of body hair.     A case of Unwanted hair growth treated successfully
    A Girl aged 23 years consulted me for unwanted facial hair growth. She was also suffering from irregular menses for about 6 years. Her mother took her to a Gynaecologist. Doctor put her on Hormone therapy and this continued for 4 years, without any permanent relief. Meanwhile she had been asked to undergo a pelvic scan which studied tiny follicles on both sides of ovaries. Her hormonal count for testosterone wa s very high. They were fed up with the treatment and finally approached me.
    After a thorough study, I asked her to take hormonal count once more. To my utter surprise the count was abnormally higher than other PCOD cases; the count was 727 ng. The size of the ovary is 4.2 cms. As she was very much depressed about her facial hair growth, I advised her to apply some cosmetic measures. She had facial or bleaching every week. I explained the real nature of the disease. She took treatment without any break. In the meantime she told me after 4 months her regular periods had started.
    I asked her to take another blood test and scan. The result was surprising as the Blood reported 0.43 ng. What a drastic change! Everybody were surprised. The scan result was also normal and studied with normal sized ovaries. After a period of three or four months she informed me that " My hair growth has considerably reduced and I go for bleaching occasionally." I asked her to continue the treatment for few more months.

The condition varies from being mild to obvious dark hair growth. Hair growth is moderate or dense which causes understandable social embarrassment.     Excessive growth of facial or body hair in women is called hirsutism.
    Description
    Hirsutism is not a disease. The condition usually develops during puberty and becomes more pronounced as the years go by. However, an inherited tendency, over-production of male hormones (androgens), medication, or disease, can cause it to appear at any age. Women who have hirsutism usually have irregular menstrual cycles. They sometimes have small breasts and deep voices, and their muscles and genitals may become larger than women without the condition.
    Types of hirsutism
    Idiopathic hirsutism is probably hereditary, because there is usually a family history of the disorder. Women with idiopathic hirsutism have normal menstrual cycles and no evidence of any of the conditions associated with secondary hirsutism. Secondary hirsutism is most often associated with polycystic ovary syndrome (an inherited hormonal disorder characterized by menstrual irregularities, biochemical abnormalities, and obesity ). This type of hirsutism may also be caused by:
