Case Fifteen - Myelomeningocele

Radiologic Findings: Myelomeningocele of lumbar spine, with neural placode present posterior to enlarged thecal sac. Ventral nerve roots course through CSF-filled sac to exit neural foramina. Tethered spinal cord. Diagnosis: Myelomeningocele of lumbar spine. Discussion: A myelomeningocele is a neural tube closure defect. The embryonic neural folds fail to flex and fuse into a tube, which persists as a flat plate, the neural placode. The superficial ectoderm does not disjoin from the neural ectoderm, remaining in a lateral position. Thus the neural placode is not covered with skin. The spinal cord remains tethered to the lesion. Spinal dysraphism is present, usually extending over 4-6 vertebral levels. Most myelomeningoceles are lumbar, with 70% present below L2. A myelomeningocele contains nerve roots, and spinal cord within a thecal sac, with herniation through the dysraphic spine, causing the neural placode to be elevated and everted. This differentiates this lesion from a meningocele which contains no neural tissue, and the neural placode remains flush with the surface. A Chiari II malformation is present with virtually 100% of myelomeningoceles. Other CNS abnormalities that are associated are hydrocephalus (80%), corpus callosum agenesis, hydrosyringomyelia, and diastematomyelia. Non-CNS abnormalities include other vertebral malformations, hip deformities, and bladder/bowel dysfunction.

The clinical picture in hydrosyringomyelia is independent of the aetiology and is a function of the sectional and vertical extent of the cavity. The classical clinical syndrome is marked by segmental anaesthesia of the dissociative type (loss of sense of pain and temperature with preservation of the sense of touch) associated with segmental weakness, absence of tendon reflexes and atrophy of the upper limbs and back, and spastic paresis in the lower limbs. The complete spectrum occurs relatively infrequently. About 80% of patients present with complaints of sense of stiffness in the legs and weakness in the legs or hands. Pain is also, despite the anaesthesia, a fairly frequent complaint.

HYDROSYRINGOMYELIA: ETIOLOGY, PATHOGENESIS, CLINIC AND NEUROSURGICAL ASPECTS OF TREATMENT V.V. Kopanitza Summary. Today there is still much unclear in the origin of hydrosyringomyelia. Different conceptions exist as to its etiology and pathogenesis, and a number of suggestions are under discussion. The dysontogenetic theory dominant until the mid-century, attributed the gliosis origination and the spinal cord cavities formation to the embryonal nervous tissue developmental defects. Within the last decades, however, the new research methods came into being, making it possible to detalise the symptomatology and extend the knowledge of the pathogenesis of this severe and progressive central nervous system disease. Key words: hydrosyringomyelia, etiopathogenesis, classification, clinic, surgical treatment.

The word syringomyelia derives from two components, syringo- and myelos. The former denotes a tubular structure and the latter term refers to marrow. Early anatomists thought that the spinal column was much like a long bone in that it contained marrow but the marrow of the spinal canal is, of course, very different indeed from long-bone marrow being, in fact, the spinal cord. The term syringomyelia therefore denotes a tubular cavity within the spinal cord. In practice the term refers to a fluid filled cavity inside the cord. The term syringo- derives from the name Syrinx, a character from Greek mythology. If you are interested you can read a little more about this. We can therefore define syringomyelia as a condition where there is a fluid filled cavity within the spinal cord. A syringomyelia cavity (or syrinx for short) may be anything from a short narrow cavity to one which extends along the whole length of the spinal cord and is under considerable tension from the build up of fluid pressure within.

Vol 33 - ABSTRACT 22 The congenital Cranio-Cervical Anomalies and Their Role in The Pathogenesis of Congenital Hydrosyringomyelia: A Radiological Study. Mohammed Yasser Metwally. Department of neurology and Psychiatry, Ain Shams University ABSTRACT In the present study 20 patients with the clinico-radiological diagnosis of congenital syringomyelia are included. All patients were examined by MRI and CT myelography. Special focus of attention was made on the associated congenital cranio-cervical anomalies with the aim of establishing possible cause-effect relationship between some of the associated cranio-cervical anomalies and the development of hydrosyringomyelia. The single congenital anomaly that was demonstrated in all patients was hindbrain herniation below the level of the foramin magnum (Chiari I malformation was demonstrated in 70% of cases and chiari II malfomation in 30% of cases). Hindbrain herniation induced the following anatomical aberrations; 1) stenosis of the foramin of magnum and the foramin of magendi, and 2) the foramin of magendi was demonstrated below the level of the calamus scriptorious.
