Hyperhidrosis is a condition in which a person experiences excessive sweating in their hands (palmar hyperhidrosis), underarms (axillary), face and/or feet (plantar). Everyone perspires more when they get nervous or excited or when they exercise, but people with hyperhidrosis perspire excessively, often for no apparent reason. The sympathetic nervous system, which controls involuntary responses such as sweating, blushing, and salivation, simply fails to regulate sweating in the body’s extremities. Consequently, people with hyperhidrosis often experience sweating so severe that it becomes a source of embarrassment, making them reluctant to shake or touch hands. It also can interfere with everyday activities such as writing, driving, taking tests, making presentations or even holding or grasping objects. Hyperhidrosis, or excessive sweating, is a relatively common condition which potentially interferes with personal and professional life. Focal hyperhidrosis commonly affects the underarms (axilla) or palms. Medications are not usually very effective for this condition and until recently only surgery could be offered. Surgery involved removing a group of nerve cells located near the first rib called the superior cervical ganglion. This surgery can have significant side effects. More recently, it has been recognized that botulinum toxin can easily and effectively treat this common condition. At Diagnostic Clinic of Houston, Dr. Loftus treats primary axillary hyperhidrosis and palmar hyperhidrosis with botulinum toxin type A and B. Botulinum toxin inactivates acetycholine nerve endings. The axillary area (underarm) contains mostly eccrine sweat glands that generate most of the sweat. These glands are innervated by acetycholine nerve fibers. The injection itself is mildly uncomfortable in the axilla but moderately uncomfortable in the hand. Topical anesthetics can be used to reduce the pain. It is rare to get any systemic side effects except that botulinum toxin type B commonly causes a mild dry mouth. The treatment will not last forever but commonly lasts for 8 months or more. The expense for underarm treatment varies somewhat by body size but is approximately $1100. The expense for palm treatment is approximately $1100 for each palm.

Axillary Hyperhidrosis, or excessive sweating in the armpits can cause embarrassing wet marks on shirts. Plantar Hyperhidrosis refers to excessive sweating of the feet and is a known cause of foot odour. Less commonly, patient may have excessive sweating of the trunk or thighs SYMPTOMS OF PRIMARY Hyperhidrosis Facial sweat may be so profuse that it causes the person to be insecure or anxious. The person may appear overly nervous when this is not the case. Patients may also experience excessive facial blushing. Sweaty hands is usually the most distressing manifestation of primary Hyperhidrosis. The amount of hand sweating varies from moisture to dripping.
