On the basis of limited information from four randomised studies, botulinum toxin appears to be effective for hyperhidrosis Several strains of botulinum toxin exist, the most commonly used in medicine are botulinum toxin A and B. Botulinum toxin A is commercially manufactured as Botox and Dysport. These are different agents, with one mouse unit of botox being approximately equivalent to three mouse units of dysport. The toxin is licensed as a treatment for a number of indications, mainly dystonias and spasm, but is also commonly used for cosmetic reasons to reduce the effects of ageing. Anticholinergic drugs or topical creams are inconvenient, unpleasant and the relief is temporary. Iontophoresis (placing the hands in a bath through which an electrical current is passed) provides only short term relief (6 hours to 1 week). Botox (botulinim toxin) treatment requires several injections in the palms or underarms to provide relief for 1 to 6 months. Surgical Treatment
The surgical treatment of Hyperhidrosis involves cutting the sympathetic trunk. This is accomplished by a procedure called endoscopic thoracic sympathectomy (ETS) or thoracoscopic sympathectomy. General anesthesia is required for this procedure. Once asleep, 3 small incisions are made below the armpit. A telescope attached to a miniature video camera is passed through one incision. The sympathetic chain is identified. The other incisions are utilized to accommodate the surgical instruments. The lung is collapsed to allow the surgeon to maneuver. When the procedure is completed the lung is re-expanded and the incisions are closed. A small tube may be kept in place to allow evacuation of air from the chest, but this is usually removed within a few hours of surgery. After one side is completed the second side is approached in the same manner.

Allgemein zum Schwitzen siehe hier! Vor Schweiss triefende Hände und Füsse... Ca. 10% der Bevölkerung leidet an einem emotionalen stärkeren Schwitzen, meist der Fusssohlen, Handflächen sowie der Achselregion. Feuchte Hände vor einer Prüfung kennt wohl jeder. Einen Hyperhidrotiker lässt emotionaler Stress jedoch geradezu in Schweiss schwimmen.
Hyperhidrosis sollten Sie als Alarmsignal ernst nehmen: Offensichtlich harmonisieren Ihr Seelenleben und die täglichen Anforderungen nicht miteinander. Die wirksamste Massnahme ist zugleich die schwierigste. Sie besteht darin, die Ursache für Unausgeglichenheit, ev. Auch Schlafstörungen und Ruhelosigkeit ausfindig zu machen und zu korrigieren. Nicht immer gelingt dies, oder es dauert eine Zeit, bis Änderungen herbeigeführt werden können. Deshalb muss häufig auch symptomatisch therapiert werden. Bei der Achselhöhlen -Hyperhidrosis erzielt man gute Erfolge mit Aluminiumchlorid-Hexahydrat. Der Apotheker mischt zunächst eine Grundlage aus 1 bis 2% Methylzellulose (bei der Galenik dieser Lösung ist es besonders wichtig, die Zellulose 24 Stunden vor Zugabe des Aluminiumchlorids quellen zu lassen) und destilliertem Wasser an, und fügt dann 15 bis 20% Aluminiumverbindung dazu. Das so erhaltene Gel muss man vor der Nachtruhe auftragen und sanft einmassieren. Nachts, wenn in der Regel kein Schweiss produziert wird, kann der Wirkstoff in die Ausführungsgänge der Schweissdrüsen eindringen und dort als festpappenden Pfropf die Drüse verschliessen. Allerdings sollte teure Nachtwäsche während des Behandlungszeitraums nicht getragen werden, da die Textilfasern angegriffen werden können.
