Temporal association between obesity and hyperinsulinemia in children, adolescents, and young adults: the Bogalusa Heart Study.
- Srinivasan SR, Myers L, Berenson GS Metabolism 1999 Jul;48(7):928-34. Obesity is generally associated with hyperinsulinemia. However, whether obesity precedes or follows hyperinsulinemia is not clear. The present study examined the temporal nature of the association between obesity and hyperinsulinemia in a biracial (black-white) community-based population enrolled in the Bogalusa Heart Study. Three longitudinal cohorts of children (n = 427; baseline age, 5 to 7 years), adolescents (n = 674; baseline age, 12 to 14 years), and young adults (n = 396; baseline age, 20 to 24 years) were selected retrospectively, with a follow-up period of approximately 3 years. In general, longitudinal changes in the mean body mass index (kilograms per meter squared), an

Let's go to something totally different, a lady with severe osteoporosis. She is almost three standard deviations below the norm in both the hip femeral neck and the cervical vertebrae, and she is very worried about getting a fracture. A fairly young woman and she was put on a high carbohydrate diet and told that would be of benefit, and placed on estrogen, which is a fairly typical treatment. They wanted to put her on some other medicines and she didn't want to, she wanted to know if there was an alternative. Although we didn't have as dramatic a turn around, we got her to one standard deviation below the norm in a year, taking her off the estrogen she was on, anyway.

Frequently Asked Questions and Answers
on Hyperinsulinemia
Q: Why should I be concerned about high insulin levels? A: When we first came to realize that high insulin levels somehow played a role in weight problems and that high insulin levels depended on eating carbohydrates frequently, we were excited about the prospect of correcting the problem by changing the frequency of carbohydrate intake during the day. And so the Carbohydrate Addict's Diet was born. Then came the thousands of letters and calls in response to the diet. There was invaluable information that readers supplied. Some reported not only weight-loss but also reductions in blood pressure. Others reported lowering of cholesterol and triglycerides in the blood even before there was significant weight-loss. Still others wrote to tell us of relief from migraine head aches, relief from gout, relief from tiredness or chronic fatigue, reductions in the type II diabetic state, and much more. Our curiosity about possible connections between hyperinsulinemia and many medical conditions got the best of us and so we began to hit MED-LINE, a tool used to search medical and scientific research. To our amazement, we found so many connections that we can not begin to describe them here. For anyone interested, the entire first part of HEALTHY FOR LIFE is devoted to our findings.

Bret, one possibility is that you could be in the middle of a long process of reversing a condition of hyperinsulinemia, where your body produces too much insulin in response to a meal. Here is the text of a comment I put on the Zone List a few weeks ago. Possibly it could apply to your situation if you imagine that you are still overproducing insulin. Remember, insulin production is linked both to carb intake and to total calories and, if there is any hint that you might have developed Type II diabetes or have been borderline for that disease, protein ingested with carbs will stimulate insulin production, not glucagon production as it does in other people. Therefore, if you have this condition (which is undiagnosed in many people) your only solution for controlling your insulin/glucagon ratio is to limit total calories at a meal. You can't depend simply on P/C ratio. You also produce insulin in response to just putting food in your mouth, or smelling an appetizing aroma. So it is possible to see your blood sugar start going down before you even begin to eat, or shortly after you start eating. If you have this problem you might try eating a very small unfavorable carb (yes, something sugary) about 20-30 minutes before your meal just to give your insulin something to work on. You'll have to experiment with this to see what, if anything, works for you. I have also tried eating a small handful of nuts 30 mintues before a meal and it seemed to work pretty well.

BACKGROUND Insulin-mediated glucose disposal varies widely in apparently healthy human beings. When insulin resistant individuals cannot secrete enough insulin to overcome this defect, glucose intolerance supervenes. However, the ability of hyperinsulinemia to prevent decompensation of glucose tolerance appears to be a mixed blessing. Individuals that are insulin resistant and hyperinsulinemic are more likely to be hypertensive and have a dyslipidemia characterized by a high plasma triglyceride (TG) and low high-density lipoprotein (HDL) cholesterol concentration. All of these have also been shown to increase risk of coronary heart disease (CHD), and in 1988 these relationships were explicitly addressed and designated as comprising a Syndrome (X). Although there is evidence that variations in insulin action are under genetic regulation, probably as much as 50% of the variability in insulin-mediated glucose disposal within a population of normal volunteers is secondary to difference in life-style. In this context, obesity and decreased physical activity play particularly crucial roles, each contributing approximately equally to the variability in insulin-mediated glucose disposal seen in the population at large. It is also worth noting that powerful ethnic differences exist, with essentially every other ethnic group studied being more insulin resistant than individuals of European ancestry.

Question and Answer Column Understanding the "H" word.. Question:
What is hyperinsulinemia ?
Answer:
We are all born with a genetic palate. Some of us were born with a tendency towards certain diseases such as heart, diabetes, certain cancers, circulatory diseases, high blood pressure, stroke, yo-yo weight gain & a variety of addictions from alcohol to smoking to food. This "tendency towards" is a biochemical factor that can cause you to either over-produce a hormone known as insulin or develop a resistance to it. Most people have both problems....over-production followed by resistance on the cell borders.
Insulin is important to life & we must have it to live. However, if the pancreas wears out from the over-production because of the ineffectiveness of the resistant cell borders, then what started out as a tendency towards has moved into hyperinsulinemia or reactive hypoglycemia, and is on it's way to adult diabetes. In a way the disease term "adult diabetes" is a misnomer, because we are now seeing children diagnosed with this killer disease at early ages.
