Prevalence
Table 199-1 ). Hypertension occurs more often in black adults (32%) than in white (23%) or Mexican American (23%) adults, and morbidity and mortality are greater in blacks. Diastolic BP increases with age until age 55 or 60.
Etiology and Pathogenesis
Primary hypertension: Primary (essential) hypertension is of unknown etiology; its diverse hemodynamic and pathophysiologic derangements are unlikely to result from a single cause. Heredity is a predisposing factor, but the exact mechanism is unclear. Environmental factors (eg, dietary Na, obesity, stress) seem to act only in genetically susceptible persons. Isolated, perfused kidneys from Dahl salt-sensitive rats (which are genetically prone to hypertension when fed a high-salt diet) do not excrete water or Na as rapidly as those from Dahl salt-resistant rats, even before hypertension develops. The pathogenic mechanisms must lead to increased total peripheral vascular resistance (TPR) by inducing vasoconstriction, to increased cardiac output (CO), or to both because BP equals CO (flow) times resistance. Although expansion of intravascular and extravascular fluid volume is widely claimed to be important, such expansion can only raise BP by increasing CO (by increasing venous return to the heart), by increasing TPR (by causing vasoconstriction), or by both; it frequently does neither.

Primary pulmonary hypertension (PPH) is when pulmonary hypertension occurs in the absence of a known cause. It is a rare disease. Pulmonary hypertension is much more common as a part of or a consequence of other diseases like chronic bronchitis, emphysema, collagen vascular diseases such as systemic lupus erythematosus (SLE) or CREST syndrome, thromboembolic diseases, and congenital heart disease involving the heart valves. Diagnosis is often delayed because the initial symptoms of fatigue, difficulty breathing on exertion, and fainting are nonspecific and definitive diagnosis of pulmonary hypertension requires invasive procedures. Pulmonary hypertension is extremely rare, occurring in about 2 people in every million. The average life expectancy after diagnosis of PPH is two to three years, with death usually due to progressive right heart failure.

Alcohol Linked To Decreased Hypertension Risk In Young Women
Moderate alcohol consumption can lead to a reduced risk of developing hypertension in young women, according to researchers from the Massachusetts General Hospital (MGH) and Brigham and Women's Hospital (BWH). The study results, published in the March 11 Archives of Internal Medicine, use data from the Nurses' Health Study II, which is based at BWH. "For women in their 20s to 40s, we found that alcohol intake at moderate levels was beneficial to blood pressure and at high levels it was harmful," says Ravi Thadhani, MD, MPH, of the Renal Unit at MGH and the Channing Laboratory at BWH, the paper's lead author. Thadhani and his colleagues found that the association between alcohol consumption and risk of chronic hypertension in young women follows what is called a J-shaped curve: Light drinkers had a decreased risk compared with nondrinkers, but heavier drinkers had an increased risk. Thadhani and his team gathered data from over 70,000 women aged 25 to 42 years old at the study's outset in 1989, who did not report having hypertension during the study's early years. After eight years of follow-up, the scientists found that women who drank about two or three drinks a week had a risk of developing hypertension about 15 percent lower than that of nondrinkers. However, women who drank on average more than 10 or 12 drinks per week had a 30 percent increased risk of developing the condition.

Blood pressure is the pressure of the blood flowing through your blood vessels against the vessel walls. It depends on your blood flow (how much blood is pumped by your heart) and the resistance of your blood vessels to blood flow. The causes of high blood pressure can vary, and most of the time, the cause isn't known. This form of the condition is called "essential hypertension." It might be due to a narrowing of the arteries, more blood than normal, or the heart beating more forcefully or faster than it usually should. Any of these conditions can raise the force of the blood against the artery walls. Sometimes high blood pressure can be caused by another medical problem, such as kidney disease. When this happens, the condition is called "secondary hypertension." As the name indicates, treating the main problem makes the blood pressure go down. Hypertension crusade now - or wait until the year 2037?
Modern medicine regards itself to be an exact natural science, based on facts as results of scientific experiments. Indeed, students of medicine learn a complete doctrine of modern medicine; and they assume that this doctrine is composed of facts. They "believe" their teachers, and they believe in this doctrine (as most patients do). Too, they don't have any other choice, if they want to pass their examination and get their license as a physician. The inner conflict starts, when they find a fact which doesn't fit into the official medical doctrine. What to do in such a case? Ask the professor? This could look as if you have doubts, like not believing fully what the teacher is teaching. Dangerous for a student who will be examined by this professor. Better to live with the inner conflict until the examination could be passed successfully.
