Dear Sue, Thank you for your question. You question brings up the topic of 'tachycardia'. There are many causes of tachycardia (fast heart beat) and they can be divided roughly into sinus (originating from the sinus node or heart's natural pacemaker) and non-sinus tachycardias. Nonsinus tachycardias are either supraventricular (coming from the upper chambers of the heart) or ventricular (coming from the lower chambers of the heart). Supraventricular tachycardias include: paroxysmal supraventricular tachycardia, atrial flutter, atrial fibrillation and AV nodal tachycardia. Ventricular tachycardias are more serious in nature and are due to a rapid depolarization of the ventricles. Sinus tachycardia is defined as a heart rate of greater than 100 beats per minute originating from the sinus node. Sinus tachycardia is classified as either appropriate or inappropriate. There are many causes of appropriate sinus tachycardia such as exercise, anxiety, panic attacks, dehydration, deconditioning, volume loss due to bleeding or other loss of body fluids, electrolyte abnormalities and many other conditions such as hyperthyroidism which you were experiencing. In general thyroid levels should be checked in everyone with unexplained tachycardia and then followed during therapy.

Dear Ms. Macgowan, Many of the symptoms you describe could result from hyperthyroidism (though depression is more often a symptom of hypothyroidism). The condition can have an influence on many of the body's systems - central nervous system; skin, hair and nails; heart and blood circulation; respiratory system; digestive system; bone, muscle and joints; reproductive system). To review some of the basic symptoms of both of these conditions, refer to the reply to the note that Tiffany posted on March 9, 1999. Now that the diagnosis has been determined, I'm assuming that a course of treatment has been recommended. If treatment is successful, you will probably note a change in the better in many of the symptoms you are observing in your daughter. This information is provided for purposes of general medical education. Please consult your health care providers for diagnostic and treatment options that pertain to your specific condition or situation.

Hortense the Hyperthyroid Cat
Hortense's owners noticed that their cat was eating more then ever but was losing weight. When they brought Hortense in to see us, she was skin and bones. She was bright and alert and very responsive. Her owners felt that she was very active and somewhat anxious when she was at home. Our examination showed a rapid heart rate and when we took the blood pressure, it was over 190. We consider 160 or under normal. We palpated the neck and we felt a bump on one side of the windpipe just below the larynx. This "bump" was an abnormally enlarged thyroid gland. We did a blood panel and found that Hortense's kidney function was normal, but there were some elevations in her liver enzymes and her thyroid level was 7. (Normal thyroid levels in the cat should be under 3.5.) Hortense was hyperthyroid. Hyperthyroidism has recently become a common ailment in cats. The thyroid glands become hyperactive and start producing more hormones than normal. This disease often increases the blood pressure, which can, over time, cause cardiac and renal damage. Thyroid hormones are directly related to the rate of metabolism. When the thyroid hormone level increase, so does the body's metabolism. Increased metabolism means increased food absorption, heart rate, blood pressure and activity level, which would explain all of Hortense's symptoms.

General Description
Hyperthyroidism, also called thyrotoxicosis or Grave's Disease, is an over production of thyroid hormone. The hormones produced by the thyroid controls the body’s temperature, ability to burn calories, and energy usage. An over production of these hormones is a hyperthyroid condition, while an underproduction creates a hypothyroid condition. An overproduction of these hormons causes the body to be in an overactive metabolic state, thus speeding up the body's processes. It is estimated that hyperthyroidism affects mostly women. One in eight women may develop a thyroid condition at some time in their life usually between the ages of 30 and 50.

Possible Symptoms
A blood test that measures the levels of various thyroid hormones or hormones from the pituitary gland (TSH) that stimulate the thyroid. A consistently high body temperature reading may also indicate an over active thyroid. Hyperthyroidism may be the cause of many different recurring ailments or conditions. The following are possible symptoms of an over active thyroid.
  • intolerance of heat
  • always feeling hot
  • nervousness
  • irritability
  • goiter
  • excessive perspiration
  • insomnia
  • frequent bowel movements
  • less frequent, irregular menstruation

    I had experienced a range of symptoms including weight loss (pleased about this one!), faster heartbeat, tiredness and sore throat but was finally laid low by 5 days of vomiting uncontrollably which my usually astute GP could not explain, so I was admitted to hospital due to dehydration. The specialist in charge of the ward took one look at me and immediately observed my enlarged thyroid, so tested my hormone levels - with a T In retrospect I wonder about the impact of the stresses in my life at that time as I had recently moved from the country to a new job in the city, so trying to settle myself and my two teenagers into new schools, a new location and adjust to the demands of city living were all taking their toll.
    My condition had stabilised and with medication of one Neo-Mercazole tablet (5 mg) daily I felt quite well and was free of symptoms with T , T and TSH in the normal range, however my endocrinologist at my three monthly visits always raised the issue of a more permanent solution, due to my gland still being enlarged with an obvious bruit (noise in the gland due to more blood than normal rushing through it). I had some concerns about radioactive iodine treatment but felt it was the better of the two options as surgery is much more invasive, so in October 1999 I agreed to proceed and took my 12 millicurie dose of Iodine 131, with the necessary precautions of avoiding children and pregnant women for 2-3 weeks.
