Too Little Parathyroid Hormone is Called Hypo-parathyroid-ism.
Hypoparathyroidism is a very rare parathyroid disease.
Hypoparathyroidism is the combination of symptoms due to inadequate parathyroid hormone production (Hypo-parathyroid-ism). This is a VERY rare condition, and most commonly occurs because of damage or removal of parathyroid glands at the time of parathyroid or thyroid surgery. This bears repeating: When people have too little parathyroid hormone, it is almost always because they had thyroid or parathyroid surgery and all four parathyroid glands were removed or injured. If this is the first parathyroid page you have read from our site, we strongly recommend that you read our introduction to the parathyroid gland first! Remember, hypER-parathyroidism is much more common than hypOparathyroidism. Hypoparathyroidism is the state of decreased secretion or activity of parathyroid hormone (PTH). This leads to decreased blood levels of calcium (hypocalcemia) and increased levels of blood phosphorus (hyperphosphatemia). There are no symptoms of too little parathyroid hormone other than the symptoms due to having a blood calcium that is too low. These symptoms can range from quite mild (tingling in the hands, fingers, and around the mouth) to more severe forms of muscle cramps leading all the way to tetany (severe muscle cramping of the entire body), and convulsions (this is very rare!).

Hypoparathyroidism is caused by underactive parathyroid glands. Underactive parathyroid glands produce too little parathyroid hormones, which, in turn, causes low levels of calcium in the blood stream. The low levels of calcium lead to tetany, an increased excitability of the nerves. What causes hypoparathyroidism?
Causes of hypoparathyroidism include accidental removal of a parathyroid gland when the thyroid is removed, or when part of the parathyroid tissue is removed during surgery. Other causes include absent parathyroid glands from birth or the sudden stop of functioning due to unknown reasons (idiopathic). It is often associated with cardiac defects, such as DiGeorge. Disease Injury ... Prevention Hypoparathyroidism Definition: a deficiency of parathyroid hormone that causes abnormal metabolism of calcium and phosphorus. Causes, incidence, and risk factors: Calcium and phosphorus are important body minerals. They form the mineral component of bones, and they exist as charged particles called ions in the blood and inside cells. A hormone called parathyroid hormone (PTH) regulates the amount of calcium and phosphorus in bone and blood. Four small glands called parathyroid glands make PTH. The parathyroid glands are located behind the thyroid gland in the neck.
Hypoparathyroidism is the clinical condition caused by a lack of parathyroid hormone. Calcium levels in the blood fall, and phosphorus levels rise. Low blood calcium levels may cause symptoms such as tingling of the lips, fingers and toes, and muscle cramps, or spasms. The most common cause of hypoparathyroidism is injury to the parathyroid glands during head and neck surgery. Rarely, hypoparathyroidism is a side effect of radioactive iodine treatment for hyperthyroidism. PTH secretion may be impaired when blood levels of the element magnesium are low or when blood pH is too high, a condition called metabolic alkalosis.

The patient showed hypocalcemia and was treated with calcium supplements and calcitriol. When the serum calcium level became normal, the pustules disappeared and erythroderma completely resolved. Histopathological features consisted of the formation of intraepidermal pustules including spongiform pustules underneath the stratum corneum, and acantholysis was observed in the epidermis. It was suggested that generalized pustular psoriasis may have been induced by hypocalcemia due to hypoparathyroidism in this case, and that acantholysis may be caused by hypocalcemia, since intercellular junctional components such as cadherins are highly dependent on calcium in the epidermis.

Parathyroid glands consist of four pea-shaped glands located on the back and side of the thyroid gland. The gland produces parathyroid hormone which, along with vitamin D and calcitonin, are important for the regulation of the calcium level in the body. Hypoparathyroidism affects both males and females of all ages.
The accidental removal of the parathyroid glands during neck surgery is the most frequent cause of hypoparathyroidism. Complications of surgery on the parathyroid glands is another common cause of this disorder. There is the possibility of autoimmune genetic disorders causing hypoparathyroidism such as Hashimoto's thyroiditis, pernicious anemia, and Addison's disease. The destruction of the gland by radiation is a rare cause of hypoparathyroidism. Occasionally, the parathyroids are absent at birth causing low calcium levels and possible convulsions in the newborn. Symptoms in the advanced and continuous stages of hypoparathyroidism include splitting of the nails, inadequate tooth development and mental retardation in children, and seizures. Abnormal low levels of calcium result in irritability of nerves, causing numbness and tingling of the hands and feet, with painful-cramp like muscle spasms known as tetany. Laryngeal spasms may also occur causing respiratory obstruction.

The conventional treatment for hypoparathyroidism, is associated with chronic increased levels of calcium in the urine (hypercalciuria) due to the kidney's inability to retain calcium. This side effect usually occurs even while maintaining the level of calcium in the blood at a normal range. As a result the kidneys may become damaged due to calcium deposits (nephrocalcinosis) and work ineffectively (renal insufficiency). This study will compare the long term effects of twice-daily injections of parathyroid hormone (PTH) with twice-daily doses of oral calcitriol in patients with hypoparathyroidism. Researchers believe, based on previous studies, that injections of PTH will provide better control of hypoparathyroidism. In addition, PTH appears to avoid the side effects typically associated with conventional therapy for hypoparathyroidism.

Al-Harakati, MD, FRCPC; Asim Hassan, MBBS, MRCP Ibrahim Al-Fawaz, MD, FRCPC Background: This is a retrospective analysis of case records of b -thalassemia major patients who developed hypoparathyroidism (HPT). The objective of this study was to assess the prevalence of hypocalcemia and hypoparathyroidism in b -thalassemia major patients being followed at King Khalid University Hospital (KKUH), Riyadh, Saudi Arabia. Patients and Methods: Diagnosis was based on low serum calcium (S/Ca), high serum phosphate (Po ), normal serum magnesium and alkaline phosphatase, and low serum parathyroid hormone levels. Other parameters analyzed included age, sex, serum ferritin levels, age of onset of HPT, any symptoms of hypocalcemia, and presence of other complications in these patients. Results: Out of 40 patients, eight (20%) were diagnosed to have HPT. The mean age at diagnosis was 13.6 years (range 11-16 years), mean serum calcium was 1.88 mmol/L (range 1.58-2.04), mean serum ferritin was 7490 m g/L (range 2000-23,064) and mean serum phosphate was 1.88 mmol/L (range 1.50-2.73). Serum parathyroid hormone (PTH) levels were low in most of the patients. Only two patients (25%) had mild symptoms of hypocalcemia. Growth retardation was present in all patients, while four patients had liver dysfunction, two had diabetes mellitus and two had cardiac dysfunction.
