It is attached by a stalk to the hypothalamus , an area of the brain that controls the function of the pituitary gland.
The hormones secreted by the pituitary and their functions are:
  • growth hormone (GH), which stimulates growth of tissues and bone
  • thyroid stimulating hormone (TSH), which stimulates the thyroid gland to secrete hormones that affect body metabolism
  • adrenocorticotropic hormone (ACTH), which stimulates the adrenal gland to secrete hormones that affect metabolism
  • prolactin , which stimulates female breast development and milk production
  • luteinizing hormone (LH), which controls sexual function in males and females
  • follicle stimulating hormone (FSH), which controls sexual function in males and females

Autoantibodies to human pituitary cytosol proteins were determined by immunoblotting in sera from patients with hypopituitarism and their relatives. Hypopituitarism, also called an underactive pituitary gland, is a condition that affects the anterior lobe of the pituitary gland usually resulting in a partial or complete loss of functioning of that lobe. The resulting symptoms depend on which hormones are no longer being produced by the gland. Because the pituitary gland affects the other endocrine organs, effects of hypopituitarism may be gradual or sudden and dramatic.

What are the symptoms of hypopituitarism
Symptoms vary depending on what hormones are insufficiently produced by the
pituitary gland. The following are common symptoms associated with reduced
production of certain hormones: insufficient gonadotropins production
(luteinizing hormone and follicle-stimulating hormone) In premenopausal women, this leads to absent menstrual cycles, infertility, vaginal dryness, and loss of some female characteristics. In men, this deficiency leads to impotence, shriveling of testes, decreased sperm production, infertility, and loss of some male characteristics. insufficient growth hormone production This usually produces no symptoms in adults. In children, this deficiency can lead to stunted growth and dwarfism.

The pituitary gland is located inside the skull. When this gland becomes less active than normal for any reason, hypopituitarism is said to occur. What is going on in the body? The pituitary gland is sometimes called the master gland of the body. It secretes many important hormones that have different effects on other glands in the body. When the pituitary gland is not working correctly, many things can go wrong in the body. The danger of hypopituitarism is that levels of other key hormones may become too low. When thyroid hormone and adrenal hormone are extremely low, death may occur. The underlying cause of this condition, such as a tumor, may also have risks. What are the signs and symptoms of the condition?

A 21 year-old woman of Indian descent, but UK born and bread presented with a two-year history of intermittent headaches, hot flushes, polydipsia, nocturia and amenorrhoea. One year previously she had been diagnosed with primary hypothyroidism with a TSH of 7.3 and FT4 of 6.5. She was treated with thyroxine but remained amenorrhoeic. Subsequent evaluation revealed TSH, LH, and FSH deficiency with evidence of cranial diabetes insipidus. An MRI scan of the pituitary gland showed an enlarged pituitary with a necrotic centre and pituitary stalk thickening. Subsequent trans-sphenoidal exploration revealed a sterile abscess, which was drained. PCR with m. tuberculosis specific amplimers from the abscess was positive. No acid-fast bacilli were isolated and culture remained sterile. Histological sections showed an avascular tissue containing a dense reticulin nework with lymphocytes and CD68 positive macrophages. Serum ANCA, ACE and ANA were all negative.

DISCUSSION: The prevalence of ACTH and/or TSH deficiency in men with macroprolactinomas is 32% at diagnosis. If interval reassessments are performed over several years of treatment, recovery of corticotropic and/or thyrotropic function is demonstrable in more than half of these patients. Without adequately reassessment, such patients might be unnecessarily maintained on long-term thyroxine and/or glucocorticoid therapy. Potential complications of glucocorticoid therapy include osteoporosis, weight gain, insulin resistance and adrenal suppression, so consigning a patient to indefinite therapy should not be undertaken lightly.
