In this case, the opening of the penis, the urethral meatus, is on the underside of the penis instead of at the tip. In mild cases, the opening is near the tip, and in severe cases, the opening may be down near the scrotum. The foreskin is also usually not fully formed on the underside of the penis, and there may be a downward bend of the penis, called chordee. Boys who are suspected of having hypospadias should not be circumcised , since the foreskin may be needed to repair the problem. Definition
Hypospadias is a congenital defect, primarily of males, in which the urethra opens on the underside (ventrum) of the penis. The corresponding defect in females is an opening of the urethra into the vagina and is rare. Epispadias (also called bladder exstrophy) is a congenital defect of males in which the urethra opens on the upper surface (dorsum) of the penis. The corresponding defect in females is a fissure in the upper wall of the urethra and is quite rare.
Description
In a male, the external opening of the urinary tract (external meatus) is normally located at the tip of the penis. In a female, it is normally located between the clitoris and the vagina. In males with hypospadias, the urethra opens on the inferior surface or underside of the penis. In females with hypospadias, the urethra opens into the cavity of the vagina. In males with epispadias, the urethra opens on the superior surface or upper side of the penis. In females with epispadias, there is a crack or fissure in the wall of the urethra and out of the body through an opening in the skin above the clitoris.

Hypospadias
Hypospadias is a relatively common anomaly of the male external genitalia, occurring in approximately 1 in 300 male infants. Hypospadias results from the failure of the urogenital folds to close due to inadequate androgen production or inadequate hormone receptors. The result of the urogenital folds not closing completely is that the spongy urethra does not form completely. The exact form of the anomaly is dependent upon where the closure fails to occur. The different locations at which failure occasions give hypospadias a variety of "types". Glandular hypospadias is one of the more common types and occurs when the external urethral orifice is on the ventral surface of the glans. Penile hypospadias is also relatively common and occurs when the external urethral orifice is on the ventral surface of the penis itself. Indications Hypospadias is a congenital anomaly in which the urethral opening is at the base of the penis, or along the inferior aspect of the shaft of the penis. Hypospadias is usually associated with an abnormal curvature of the penis, called "chordee". Hypospadias is one of the most common birth defects seen in infant boys. Surgery is necessary to allow normal urination, to correct the penis deformity, and to ensure normal sexual functioning. The repair is performed before the child reaches school age.

The goals of the surgery are to repair the abnormal curvature of the penis, and to create a new urethra at the tip of the penis. A small piece of foreskin can be used to create a tube to extend the urethral length to allow placement of the urethral opening at the tip of the penis. Classification: Browne (1936) Barcat (1973) - post-orthoplasty position The location of the meatus after correction of curvature provides the most practical classification of hypospadias. The glanular and coronal types constitute anterior hypospadias, which accounts for 50% of all cases. The distal penile midshaft and proximal penile types account for 30% of cases. Embryology Formation of the genital tubercle and lateral genital folds (indifferent) occurs during 6/40. DHT dependent elongation of the phallus and urethral groove forms between lateral folds (during 10/40), and the epithelial lining of the groove forms the urethral plate. The folds then coalesce in the midline (during 12/40), forming the urethra proximal to the glans. The glanular urethra results from canalisation of the solid cord of epithelial cells. Hypospadias results when fusion is incomplete. However, pathogenesis of the chordee is disputed, it may be due to fibrous tethering or differential growth.

Hypospadias
Epispadias, which is less common than hypospadias, may also be seen in children with spina bifida or exstrophy of the bladder (a condition where the bladder may appear to be inside out and the abdominal wall is partially open leaving the bladder exposed to the exterior of the body), occurs when the urethral opening is in an abnormal location. In contrast to hypospadias, with epispadias, the urethral opening in male is usually located on the topside (not the underside or tip) of the penis.

Causes, incidence, and risk factors: Hypospadias is a congenital defect that affects up to 3 in 1,000 newborn boys. The condition varies in severity. In most cases, the opening of the urethra is located near the tip of the penis on the glans. More severe forms of hypospadias occur when the opening is at the midshaft or the base of the penis.Occasionally the opening is located in the scrotum or the perineum (beneath the scrotum). This anomaly is often associated with chordee, a tight fibrous band that results in a downward curvature of the penis seen with erection. (Erections are common with infant boys.) Some cases are inherited; others result from unknown causes.

The foreskin is also almost always affected, being imperfectly formed beneath, and this deformity may be more obvious than the hypospadias itself In a proportion of cases, there is also a downward bend on the shaft of the penis (chordee) which becomes exaggerated during penile erections and as a general rule the more severe the hypospadias the greater the chance of significant chordee. The position of the urethral opening may result in a downwardly deflected urinary stream, a problem for boys once they are toilet trained, while chordee may cause difficulties with sexual intercourse during adult life. Hypospadias is one of the most common congenital deformities, affecting approximately one in two hundred new born boys. Except in a small number of cases where there is a positive family history, the cause is unknown. In the most severe cases, which are fortunately rare, the gender of the child may not be certain in which event further investigation will be required.
