It was repaired when he was two days old. Lately, however, his rectum seems to hold more stool than he is able to push out comfortably. He struggles and strains and cries, but the stool is soft. When I watch him move his bowels on the changing table, he looks like he is giving birth because the stool is so very wide around. We did the dilatator thing after the surgery. He doesn't need to control his bowels because he is only 21 months old. However, I don't believe being in pain is normal when you are moving your bowels. Has anyone else had this problem and what did you do? Thanks! The Question: Does folic acid supplementation before and during early pregnancy reduce the risk of imperforate anus in their newborns? Answer in a nutshell: According to this cohort study from China, daily maternal consumption of 400 µg of folic acid before and during early pregnancy may reduce the risk for imperforate anus in their offspring. Maternal consumption of folic acid before pregnancy and during early pregnancy is associated with a reduced risk for some birth defects. Whether folic acid can reduce the risk for imperforate anus is unknown. As part of a public health campaign conducted in China from 1993 through 1995, the outcomes of pregnancies of 20 weeks' gestation were evaluated among women using folic acid supplements. The women were asked to take one pill containing 400 µg of folic acid (without other vitamins) every day from the time of their premarital examination until the end of their first trimester of pregnancy. Rates of imperforate anus and risk ratios for imperforate anus among the offspring of these women were calculated according to folic acid use. Among the offspring of women who took folic acid and women who did not take folic acid, 20 and 30 infants with imperforate anus were identified, respectively. The rate of imperforate anus was 3.1 per 10,000 among the offspring of women who did not take folic acid and 1.6 per 10,000 among the offspring of women who took folic acid; adjusted for maternal age, the risk ratio was 0.59 (95% confidence interval: 0.33, 1.07). Folic acid supplementation appears to protect against imperforate anus in the newborns.

He had a colostomy from birth to 8 months. At 8 months he had a "pull-thru" operation, and the colostomy was closed. He has struggled with incontinence off and on over the years - the operation was not "perfect". Now, at the age of 25, he still suffers from bouts of incontinence, and of course self-esteem problems. He no longer trusts any doctor to give him any useful advice or assistance, and I am concerned that he won't care for his condition properly. In addition, the self-esteem problem and his occasional incontinence has made him avoid pursuing a conventional education or conventional job - he simply is afraid of exposing himself to embarassing situations. Are there any parents out there who have grown children dealing with this ? Complete absence of an anal opening requires emergency surgery for the newborn. Surgical repairs are done while the baby is deep asleep and pain-free (using general anesthesia). Surgery for a high type imperforate anus defect usually involves creation of a temporary opening of the large intestine (colon) onto the abdomen to allow passage of stool (this is called a colostomy ). The baby is allowed to grow for several months before attempting the more complex anal repair. The anal repair involves an abdominal incision, loosening the colon from its attachments in the abdomen to allow it to be repositioned. Through an anal incision, the rectal pouch is pulled down into place, and the anal opening is completed. The colostomy may be closed during this stage or may be left in place for a few more months and closed at a later stage. Surgery for the low type imperforate anus (which frequently includes a fistula ) involves closure of the fistula, creation of an anal opening, and repositioning the rectal pouch into the anal opening.

Introduction : The standard approach to management of high imperforate anus is colostomy in the newborn period followed by future definitive repair. The purpose of this study was to study the advantages of primary PSARP (Group A) over the three stage correction (Group B). This study also compared the post operative complications between the two groups. Materials and Methods : 30 male neonates were enrolled in this study 15 in either group 15 underwent primary PSARP and 15 underwent colostomy, PSARP and then colostomy closure as a three stage procedure. The mode of presentation, associated congenital anomalies, type of fistula were evaluated in both groups. The post operative complications and mortality in either group were evaluated. The average duration of hospital stay and the number of hospital admissions were measured. The babies were followed up for a period of one year. Results : 2 Babies in group A and 7 babies in group B had associated anomalies. It was observed that a majority of patients in both groups had recto urinary fistulas. All 15 patients in group A underwent the one stage correction procedure (Primary PSARP). In group B, out of 15 patients who underwent the preliminary procedure sigmoid colostomy, only 12 of them underwent subsequent PSARP and closure of colostomy. One patient was lost for follow up and two patients expired. The incidence and variety of post operative complications were statistically significantly more in Group B. (Three stage correction) when compared to Group A (one stage correction). The average duration of hospital stay, number of admissions and mortality (2 patients in Group B) were more in Group B as compared to Group A.
