X-linked dominant inheritance is where the gene is located on the X chromosome, but the gene acts in a dominant manner. This means that both males and females can display the trait or disorder, by only having one copy of the gene. Depending on the disorder, often times a gene that is X-linked dominant causes a male pregnancy not to survive. Another way scientists and physicians refer to this process is to say that “there is lethality in males”. This means that the gene is passed from mother to daughter, and, if passed to a son, the pregnancy miscarries. Only those sons without the gene survive to be born and are born healthy. One example of an X-linked dominant condition is called incontinentia pigmenti (IP).

Incontinentia Pigmenti (IP) on harvinainen vallitsevasti periytyvä sairaus. Sen aiheuttaa X-krosomin geeni, mahdollisesti kaksikin. Geenikartoissa mainitaan alueet Xp11.21 ja Xq28. IP-tauti aiheuttaa iho-oireita, minkä lisäksi siihen liittyy silmien, keskushermoston ja hampaiston vaurioita. Myös kynsien ja hiusten vaillinaisuuksia esiintyy usein. Niinikään pienipäisyydestä mainitaan sairautta koskevissa raporteissa. Iho-oireet ilmenevät vaiheittain. Vastasyneen iho on punanainen ja rakkuloiva. Seuraavalla tasolla esiintyy käsien ja jalkapohjien rupia. Kolmannessa vaiheessa muodostuu ylipigmentoituneita ihoalueita, ja lopuksi ihon kalvakoitumista, kuivumista ja arpeutumista. Keskushermostovauriot ilmenevät epilepsiana tetraplegiana ja kehitysvammaisuutenakin IP-tauti on yleisempi naisilla kuin miehillä. Sen esiintyvyydestä ei ole tarkkoja tietoja. Incontinentia pigmenti (Ook syndroom van Bloch-Sulzberger of syndroom van Bloch-Siemens genaamd) is een zeldzame (ca. 1:30.000) erfelijke (x-gebonden dominante ) huidziekte die wordt gekenmerkt door onregelmatige pigmentatie van de huid, met name bij jonge kinderen. De huidmanifestaties verdwijnen vaak zonder behandeling in de adolescentie. Naast huidafwijkingen kunnen er ook neurologische en oogsymptomen en tand- en skeletafwijkingen bij voorkomen. De ziekte komt meestal bij vrouwen/meisjes voor omdat aangedane mannelijke vruchten meestal al voor de geboorte doodgaan. De plaats van de genetische afwijking is sinds 1999 bekend en ligt op de locatie Xq28 in het zgn. NEMO gen.

Incontinentia Pigmenti
Incontinentia pigmenti (IP) is a rare, genetic disorder characterized by unusual patterns of discolored skin. The disorder is caused by excessive deposits of melanin (normal skin pigment). IP is divided into 4 stages, which frequently overlap or appear together. During the first stage, which begins between birth and 6 months of age, there is inflammation accompanied by skin redness and spiral lines of small fluid-filled blisters. The second stage gradually develops with rough, warty skin growths which appear on the arms or legs and, sometimes, on the head or trunk. These growths, which are often arranged in the same spiral or linear pattern as in the first stage, usually resolve during infancy or early childhood. The third stage begins between 3 months and 2 years of age and is characterized by discolorations appearing in unusual patterns. The fourth stage consists of diminished pigmentation or atrophy in areas of previous discoloration. In rare cases of IP, hair loss with scarring and non-dermatological symptoms such as dental problems (delayed tooth growth or decay, missing or malformed teeth), diminished vision, seizures, muscle spasms, or slight paralysis may occur. Developmental abnormalities including dwarfism or short stature, club foot, spina bifida, skull and ear deformities, cleft lip or palate, atrophy on one side of the body, abnormal development of cartilage, congenital dislocation of hip, incomplete development of one side of the spinal bones, and extra ribs or webbed fingers may occur with the disorder but are not characteristic. In a few cases of IP, extremely wooly or kinky hair and an immune system dysfunction may also appear.
