Insulin, a hormone, regulates the transport of sugar from the blood into cells for use as fuel. Insulin resistance occurs when insulin is less active than it should be, or when cells are for some reason unable to respond to insulin. This leaves higher than normal levels of sugar circulating in the blood, which can lead to diabetes and many other health problems. Physicians and researchers are seeing an increase in cases of insulin resistance in their HIV-positive patients, particularly those taking protease inhibitor medications, and are working to find out why. Insulin is a kind of hormone. It helps your body use the sugar in the food you eat as fuel. In some people, the tissues stop responding to insulin. Doctors refer to this condition as insulin resistance. If you have insulin resistance, your body will make more and more insulin, but because the tissues don't respond to it, your body won't be able to use sugar properly. Insulin resistance often goes along with other health problems, such as diabetes, high cholesterol, high blood pressure and heart attack. When a person has many of these problems together, doctors call it insulin resistance syndrome. How do I know I have insulin resistance syndrome? No simple test can tell that you have insulin resistance syndrome. If one or more of the following is true for you, your doctor may suspect you have insulin resistance syndrome. How to check your waist-to-hip ratio To find your waist-to-hip ratio, measure the smallest part of your waist with a tape measure. Don't hold in your stomach while you measure! There is an increasing recognition that there are 2 important factors contributing to the insulin resistance seen among treated patients, that due to the protease inhibitors and that due to fat redistribution itself. Both need to be considered in practice and in future research into potential interventions. Lesser progress has occurred with specific therapies that address insulin resistance, but insight into mechanisms is nonetheless very important into deciding what types of interventions are likely to be effective.

Dr. Currier and colleagues (Abstract 677-T) examined Medi-Cal (California Medicaid) claims from July 1994 to June 2000 for DM incidence rates in men and women over age 18 with and without HIV diagnoses. A total of 7219 (61% male) individuals with HIV and 2,792,971 (30% male) individuals without HIV were studied. A total of 7,101,180 person-years were examined. To assess only incident cases of DM diagnoses, individuals had to be free of DM related diagnoses and oral anti-diabetic pharmacy claims for at least one year prior to study inclusion.
HIV-infected persons were 3.32 times more likely to have diabetes in this study. Overall incidence of DM per 100 person-years in HIV infected individuals is 10.68 (95% CI 10.15-11.23) and 2.91 (95% CI 2.89-2.92) in non-HIV infected individuals, odds ratio 3.32 (p <0.0001). Insulin Resistance Syndrome Also indexed as: Metabolic Syndrome, Syndrome X The insulin resistance syndrome (IRS) is a group of health risk factors that increase the likelihood of heart disease and perhaps other disorders, such as diabetes and some cancers The risk factors that make up IRS include insulin resistance, which refers to the reduced ability of the hormone insulin to control the processing of glucose by the body. Other major risk factors often associated with IRS include high blood sugar and high blood triglycerides cholesterol high blood pressure , and excessive body fat in the abdominal region. People with IRS do not always have every one of these risk factors, but they usually have many of them. THE PROBLEM: Incidence of adult- onset diabetes varies depending on recency of a population's exposure to agriculture.

This section is based on Miller and Colagiuri [1994]. The carnivore connection hypothesis they elucidate provides an explanation, based on evolution, of the following phenomenon:
  • Populations that have a relatively "long" history of agriculture, e.g., those of European descent, have a relatively low incidence of NIDDM, that is, non- insulin- dependent diabetes mellitus, also known as adult- onset diabetes.
  • Populations that adopted agriculture more recently or, in other words, discontinued the evolutionary hunter- gatherer diet more recently, have relatively higher incidence rates of NIDDM. In some former hunter- gatherer populations, incidence levels of NIDDM range from epidemic levels (Nauruans; the Pima tribe of the U.S.), to rates that are "only" several times that of Europeans.

Insulin is a hormone that carries glucose (sugar) from your blood into your cells so that it can be burned for energy. The pancreas produces insulin. Insulin resistance is a state in which your cells are not responding to insulin appropriately, so the sugar in your blood cannot get into your cells. To compensate, your pancreas pumps out more insulin to try to get the sugar out of your blood and into your cells. The hyperinsulinimia (high blood insulin) that results is able to maintain normal blood sugar levels and delay the onset of diabetes. Diabetes Type 1 diabetes is usually diagnosed in childhood and occurs when the pancreas stops producing insulin. Insulin injections are required for life. Type 2 diabetes is usually diagnosed in adulthood. It occurs when the cells stop responding to insulin (often due to insulin resistance), and the pancreas is unable to keep compensating by producing more and more insulin to maintain normal blood sugar levels. Scientists Close In On Trigger Of Insulin Resistance
Extra sugar can cause insulin resistance in cells. Now scientists have an explanation.
In experiments with fat cells, Johns Hopkins scientists have discovered direct evidence that a build-up of sugar on proteins triggers insulin resistance, a key feature of most cases of diabetes.
The results underscore the importance of glycosylation - attachment of a sugar to a protein as a way cells control proteins' activities, the scientists report in the April 16 issue of the Proceedings of the National Academy of Sciences. The scientists found that at least two proteins involved in passing along insulin's message were unlikely to work properly when coated in extra sugar.
Type 2 diabetes, the most common form in adults, occurs when muscle, fat and other tissues stop responding to insulin's signals to mop up sugar from the blood. The resulting high blood sugar, if uncontrolled, can lead to blindness, amputation and death. Understanding sugar's precise influence on insulin's activity may help improve treatment and prevention, scientists hope. "Cells don't respond to insulin itself. Instead, a whole cascade of events, set in motion by insulin, eventually causes cells to take in sugar," explains Gerald Hart, Ph.D., professor and director of biological chemistry in the school's Institute for Basic Biomedical Sciences. "We now have an explanation of how sugar can affect these signals, and even a hypothesis for how high blood sugar could cause tissue damage in diabetes by improperly modifying proteins."
